[Hysteroscopic diagnosis and follow-up of adenomatous hyperplasia and therapeutic consequences].

63 cases of adenomatous endometrial hyperplasia (1st-3rd degree) were diagnosed by hysteroscopy and D&C at the Department of Obstetrics and Gynaecology of the University of Greifswald from January 1991 to December 1994. In 40 patients with adenomatous hyperplasia (2nd-3rd degree) a hysterectomy was performed. 23 patients presenting with 1st degree hyperplasia were treated continuously with gestagenes for a period of 6 months. In those cases we performed a repeated diagnostic hysteroscopy and D&C to check the success of hormonal treatment. In 92.5% of tall cases the histological diagnoses of a lower degree adenomatous hyperplasia, cystic glandular hyperplasia or of a normal endometrium were made after hysterectomy. Only in one patient (2.5%) an endometrial carcinoma developed from an adenomatous hyperplasia (3rd degree). There was no progression to observe of adenomatous hyperplasia after hormonal therapy with gestagenes. We conclude that in those cases a hysterectomy has not necessarily to be carried out. However, in adenomatous hyperplasia of 3rd degree hysterectomy has to be the treatment of choice, because there is an increased risk of invasion.

[Proliferating leiomyoma after hysteroscopic myoma resection].

A proliferative leiomyoma, which had possibly been removed only subtotally by hysteroscopic resection, grew postoperatively so that vaginal hysterectomy had to be carried out. The histology of the leiomyoma showed an increased expression of the proliferative marker Ki-67.

Partitioning of streptokinase in aqueous two-phase systems.

The partitioning of streptokinase in aqueous two-phase systems, containing poly(ethylene glycol) and dextran or poly(ethylene glycol) and salt was investigated. The protein partitions in favour of the upper, PEG-rich phase, if PEG of low molecular weight and potassium phosphate or ammonium sulphate were used. This property was found to be independent of the degree of purity and was exploited for the partial purification of streptokinase from crude material. The protein was shown to exhibit negligible affinity to diverse triazine dyes applied in affinity partitioning experiments.

[The general practice course catalog--an attempt at quality assurance in graduate general practice education].

Additionally to the training in the hospital and in the practice, the rules for continuing education for general practise, established at the meeting of medical German physicians in 1992 demand the participation in theoretical seminars. The content and duration of these seminars is content and duration of these seminars is laid down in recommendations of the chamber of the physicians of Germany to ensure the quality of the courses. According to this, the first recommendations consists of the Course Book for General Medical Practise regarding a course of 240 hours duration. This is a nation-wide standardized curriculum which was accepted from all German chambers of physicians as the basis of the further education in family medicine. The standardization and the structuring of the content, goals, and methods of teaching and learning as well as the organization of the seminars, the working materials for the teachers, regular training for the presenters and teachers, methods for a nation-wide evaluation of the seminars, and independence from commercial interests are the features to ensure the quality of the courses.

How reliable is lumbar nerve root sheath infiltration?

To determine the reliability of lumbar nerve root sheath infiltration, a prospective study was performed. Ninety-four patients were randomized into three groups. In the first group of 33 patients, 0.5 cc of dye (Telebrix N, 30 g) was applied at nerve root L4, in the second group of 30 patients, 1.0 cc, and in the third group of 31 patients, 2.0 cc. The infiltration was guided by computer tomography. The diffusion of the dye was documented with computed tomography of the affected segment L4-5. The images were evaluated by an unbiased observer. The results showed that in the first group the dye diffused to the adjacent ipsilateral nerve roots L3 and/or L5 in nine patients. In the second and third groups this diffusion was seen in 9 and 11 patients, respectively. A diffusion into the psoas muscle was documented in 4, 10 and 22 patients, respectively. These latter differences were statistically significant (P < 0.01). Diffusion into the psoas muscle is especially important because the nerve roots converge in this muscle to become a plexus, and they are no longer surrounded by their dural sheaths. Diagnostic lumbar nerve root sheath infiltration should be performed by an experienced examiner. To guarantee high reliability, the tip of the needle should be placed as near as possible to the affected nerve root. The amount of local anaesthetic should be as small as possible, 0.5 cc or preferably less.

[Mesenchymal chondrosarcoma of the maxilla].

Mesenchymal chondrosarcoma is a variant that shows some peculiarities in comparison with classical chondrosarcomas. On the basis of a case report on a young man with a mesenchymal chondrosarcoma of the maxilla, the clinical symptomatology and diagnostics, morphological findings and actual therapy are discussed. In addition, problems of differential diagnosis are also discussed.

Sexual functioning in HIV+ and HIV- injected drug-using women.

As part of a natural history study of HIV disease in injected drug users, 38 HIV positive (HIV+) asymptomatic or low-symptomatic women and 37 HIV negative (HIV-) women, mostly of minority inner-city background, underwent a comprehensive survey of sexual functioning. At study baseline, the sample as a whole showed a relatively high prevalence of problems in all major phases of the sexual response cycle and in vaginismus and vaginal pain. Statistically significant group differences indicate higher rates of problems in sexual functioning in HIV+ women even at an early stage of disease progression.

[Intra-abdominal migration of a Kirschner wire from the femoral neck].

A epiphysiolysis capitis femoris in a child was treated bei Kirschner-wire fixation. Postoperatively one of these wires broke and presumably migrated through the acetabulum intraperitoneally into the true pelvis. The Kirschner-wire fragment was removed by laparotomy. We discuss the technique of application of Kirschner-wires and complications of the procedure, in particular migration and dislocation.

[Histotopographic studies of the intramural coronary arteries in the Trabecula septomarginalis of the right cardiac ventricle in swine (Sus scrofa domesticus) and dwarf goats (Capra aegagrus f. domestica)].

In the Trabecula septomarginalis (Moderator band) of pig and pygmy goat regularly till to 6 arteries are found, which traverse from the interventricular septum to the M. papillarius magnus. These intramural coronary arteries (diameter 50-300 microns)--without any exception-musculoelastic intimal thickenings are recognizable, which often marked extensive and formed in the whole vessel length. It is concluded that coronary arteries enlarge in response to increasing intimal thickening and that such enlargement can prevent narrowing of the lumen. The importance of the specificities in the wall structure of the arteries in the Trabecula septomarginalis are discussed as an adaptational reaction of the vascular wall to the extraordinary stress of these small vessels.

[Histotopographic and morphometric studies of the intramural coronary arteries in the trabecula septomarginalis of swine and pigmy goats].

Up to six arteries traverse from the interventricular septum to the M. papillaris magnus in the trabecula septomarginalis (moderator band) of swine and pygmy goat. Musculo-elastic intimal thickenings, many of them quite extensive, are recordable from along the entire length of all these intramural coronary arteries which are between 50 microns and 300 microns in diameter. The conclusion may be drawn from the results of morphometric analysis that coronary arteries undergo enlargement in response to increased intimal thickening and that such enlargement does not lead to narrowing of the lumen. Peculiarities in wall structure of the arteries in the trabecula septomarginalis are interpreted as adaptive processes of the vascular wall to the extraordinary stress on those vessels.

[Fibrous hamartoma in the infant].

Fibrous hamartoma is a rare example of juvenile fibromatoses. We report a case of an 11 month old infancy with presenting of clinical picture, morphological findings and therapy. Biological behaviour, differential diagnosis and pathogenetic connections to other fibromatoses were discussed.

Interaction of bacterial glucose-6-phosphate dehydrogenase with triazine dyes: a study by means of affinity partitioning and kinetic analysis.

The interaction of glucose-6-phosphate dehydrogenase (EC 1.1.1.49) from Pseudomonas W6 with 33 triazine dyes bound to poly(ethylene glycol) has been screened by means of affinity partition in an aqueous two-phase system composed of poly-(ethylene glycol) and dextran. Regarding the strength of interaction expressed by the affinity partitioning effect three groups of dyes can be distinguished showing high, medium or low interaction to the enzyme. The different influence of the structurally related dyes Cibacronblue F3G-A, Procion Blue MX-3G and Procion Blue MX-R on the partitioning of glucose-6-phosphate dehydrogenase is discussed. Both coenzymes of this glucose-6-phosphate dehydrogenase, NADP+ and NAD+, are able to diminish the dye-enzyme interactions but to a different extent. In the case of Procion Red HE-3B less than 0.2 mM of NADP+ abolished completely the dye-protein interaction indicating that the dye binding site might be closely related to the nucleotide binding site. This does not hold for a series of other dyes revealing strong interactions to the enzyme. In these cases higher NADP(+)-concentrations are necessary to weaken the dye-enzyme interactions. Only small influences on the interaction between the dye and glucose-6-phosphate dehydrogenase were found with 1 mM NAD+. The different effects of the two coenzymes are discussed with respect to separate binding sites for them and to different conformational states induced. The competition of Procion Red HE-3B with NADP+ and NAD+ for the binding site of glucose-6-phosphate dehydrogenase was also demonstrated by inhibition studies. The Ki-values of Procion Red HE-3B with NAD+ and NADP+, respectively, were calculated.

[Correlative morphological studies of the heart of guinea pigs].

Investigations were conducted to record the following morphological parameters from hearts of guinea pigs (alb/Jena, inbred strain): absolute and relative heart weight as well as weights of left and right ventricles and of the interventricular septum. Correlations are described and discussed between the values recorded as well as between those values and body weight and changes in the course of ageing. The point is made that investigations of this kind are helpful in describing heart development in its totality and may provide better prerequisites for appraisal of pathological conditions.

[Quantitative morphology of the intramural coronary arteries of the trabecula septomarginalis of pigmy goats and swine].

There are musculo-elastic intimal thickenings in the intramural coronary arteries of the Trabecula septomarginalis, which results in a stenosing grade of 69% in average in 6 month old pigs, of 43% in 1-6 months old pygmy goats and of 35% in average in 4-7 years old pygmy goats. The degree of intimal thickenings is related to the arterial diameter (r = -0.60); the strongest of which are found in small vessels of 100-200 microns. With increasing arterial diameter during ageing the stenoses decrease. The role of the intramural coronaries of the Trabecula septomarginalis is discussed.
