Use of psychotropic drugs in general medical geriatric inpatients. Relationship with various parameters of psychological distress (evaluated 'in blind').

The authors have evaluated the psychotropic drug use patterns and psychological distress (with the Symptom Distress Checklist, SCL-90) amongst 331 elderly medical inpatients. Forty-two percent of the sample took psychotropic drugs during their hospitalization period. The drugs most commonly used were anxiolytics and hypnotics of the benzodiazepine class. Subjects to whom psychotropic drugs were prescribed reported higher psychological distress compared to those not receiving them; however, a score of moderate distress in the depression and sleep disturbances subscales was reported by a relatively high percentage of subjects not receiving psychotropics. Patients taking antidepressants reported scores of psychological suffering higher than those under benzodiazepine treatment: such a difference not only related to the depression subscale, but to the majority of the symptom areas investigated by the SCL-90.

Psychological findings in chronic anal pain.

The psychological characteristics of 21 patients suffering from chronic anal pain were studied. They underwent a clinical interview and completed the Eysenck personality questionnaire, the Zung self-rating depression scale and an analogue scale for the assessment of perceived pain. Data derived from the self-rating scales were compared with those obtained from a matched control group afflicted with fissure-in-ano. The pain patients reported higher scores for neuroticism, psychoticism, depression and perceived pain than the controls. Fifty percent of the experimental group had suffered prior to the onset of pain from depressive disturbances, and 11 subjects reported at the clinical interview somatic symptoms which usually accompany depression. These results support the hypothesis that the pain symptom in some patients with chronic anal pain could represent the manifestation of an underlying depressive disturbance.

Assessment of severity of suicide attempts. A trial with the dexamethasone suppression test and 2 rating scales.

In this study, the dexamethasone suppression test (DST) and 2 rating scales, the 'Echelle d'Evaluation de Risque Suicidaire' and the Pierce modified form of the Suicide Intent Scale, were administered to a group of 37 subjects admitted to the general hospital of Padua for suicide attempts. The purpose of our study was to verify if these tools could be considered useful in assessing the severity of the attempts. 10 subjects were DST-positive, but the test evidenced very low sensitivity and predictive value. The 2 rating scales demonstrated a good degree of concordance in identifying the adjustment disorder group as being at a lower risk for a serious attempt.

Type A behavior pattern and mortality after recurrent myocardial infarction: preliminary results from a follow-up study of 5 years.

The purpose of the current study is to examine the association between the type A Behavior Pattern (TABP) and recurrent myocardial infarction (RMI). Rosenman's Structured Interview was administered to a consecutive series of patients admitted to the hospital for myocardial infarction (n = 88). Incidence and mortality from RMI in relation to TABP categories were evaluated after a follow-up period of 5 years. The number of new episodes of myocardial infarction observed in the extreme categories was nearly the same but the number of subjects who dies was nearly twice as large in the B as in the A1 category (10.3% vs. 5.8%, chi 2 = 9.074, p less than 0.0283). No subject was observed to survive after RMI in the B group. In agreement with other recent studies, our preliminary results failed to confirm the association between TABP and RMI, but showed a protection from death for subjects displaying high TABP levels. This finding is discussed in terms of the possibility for type A subjects to cope better with the acute illness.

Personality pattern of patients with chronic gastric ulcer: a case control study.

Personality factors are thought to play a role in peptic ulcer disease. In this study the psychological profiles of chronic gastric ulcer (GU) patients (n = 28) were compared to matched controls (n = 28) using the Cattell Sixteen Personality Factor Questionnaire Form C. GU patients were characterized by traits of anxiety, dependence, introspection, trustfulness and adaptability. These findings are discussed in the light of the literature on personality and peptic ulcer disease.

Personality profiles of patients with duodenal ulcer.

Cluster analysis of personality profiles determined by the Sixteen Personality Factor Questionnaire separated 79 patients with duodenal ulcer into three homogeneous subgroups: 32 dependent and anxious patients, 31 neurotic and anxious patients, and 16 patients with a balanced personality. No significant differences among these three groups emerged for the variables of age, sex, duration of illness, fasting total serum pepsinogen, fasting serum pepsinogen group I, and number of cigarettes smoked per day, although there was a nonsignificant trend for both pepsinogen values to be higher in the patients with a balanced personality than in the other two groups. These results support the concept of heterogeneity of peptic ulcer disease.

Does viloxazine really improve sex drive? A double-blind controlled study.

The action of viloxazine on libido and sex drive was evaluated with a randomised controlled double-blind trial against placebo on a population of 26 male out-patients affected by primary depression (DSM III). Modifications in the sexual sphere were assessed by the ad hoc inclusion of a series of items in the Zung Self-Rating Depression Scale. Thus modified, the scale was administered at the beginning and the end of a four-week trial period. Viloxazine proved to have a considerable disinhibiting effect, whose principal expression level was a return to pre-depression levels of frequency in sexual relations.

Psychosocial stress and obstetrical complications. The need for a comprehensive model.

The relationship between life events, anxiety, social support, coping style and obstetrical complications was studied in a series of 319 pregnant women; they were interviewed during the sixth month of pregnancy. To allow a prospective study, only obstetrical complications occurring after the interview were considered. 142 of the women proved to be complicated and the remaining 177 uncomplicated. Taken singly, none of the psychosocial variables considered significantly differentiated the two groups of women. However, when the combined effect of some psychosocial variables is considered, numerous significant interactions emerge. The variables which interact most frequently with each other and with the life events variables are: specific anxiety, coping style and general anxiety.

Psychosomatic factors and peptic ulcer disease.

The authors present a review of the role of psychological factors in peptic ulcer disease (PU). Three lines of research have been identified: personality, psychological factors and PU; stressful life events and their relationship to PU; possible interactions between biological parameters, the CNS and psychosocial aspects. The analysis of the studies presented shows that there is a certain level of agreement with regard to personality and psychological aspects; PU patients present a personality with dependence/independence problems and high level of anxiety. Data concerning the role of stress appear to be far from uniform and often even contradictory. Much remains to be done with respect to the possible links between psychological and biological parameters; there have been only few studies in man--which have not been duplicated and were performed on a limited number of patients--but a fair number in animals. However, the results concerning the relationship between psyche and secretory patterns are very interesting, and represent one of the most important lines of future research.

Reexamination of the activation of yeast proteinase B at pH 5: loss of inhibition effect of proteinase B inhibitors.

The activation of yeast proteinase B at pH 5 has been suggested to be due to the degradation of a specific inhibitor for the enzyme, IB, by proteinase A. However, we found that when pepstatin, which completely inhibits proteinase A, was included in the pH 5 activation mixture, the same time-dependent activation of proteinase B was observed. Furthermore, proteinase B preparations that were void of proteinase A activity were still activated by incubation at pH 5. We found that the activation of proteinase B at pH 5 was due primarily to the irreversible loss of inhibitory effect of IB, which can be resolved by isoelectrofocusing into four distinct bands with isoelectric points of 4.6, 6.1, 6.8 and 7.6. These four forms of IB showed varying degrees of stability at pH 5, which may explain some of the differing observations reported in the past.

Cytidine deaminase from Escherichia coli B. Purification and enzymatic and molecular properties.

Cytidine deaminase (cytidine aminohydrolase, EC 3.5.4.5) from Escherichia coli has been purified to homogeneity through a rapid and efficient two-step procedure consisting of anion-exchange chromatography followed by preparative electrophoresis. The final preparation is homogeneous, as judged by a single band obtained by disc gel electrophoresis performed in the absence and presence of denaturing agents. The native protein molecular weight determined by gel filtration is 56 000. Sodium dodecyl sulfate disc gel electrophoresis experiments conducted upon previous incubation of the enzyme with dimethyl suberimidate suggest an oligomeric structure of two identical subunits of 33 000 molecular weight. The absorption spectrum of the protein reveals a maximum at 277 nm and a minimum at 255 nm. The isoelectric point is at pH 4.35. Amino acid analysis indicates an excess of acidic amino acid residues as well as six half-cystine residues. No interchain disulfide groups have been evidenced. According to Cleland's nomenclature, kinetic analysis shows a rapid-equilibrium random Uni-Bi mechanism. Cytidine deaminase is competitively inhibited by various nucleosides. Km values for cytidine, deoxycytidine, and 5-methylcytidine are 1.8 X 10(-4), 0.9 X 10(-4), and 12.5 X 10(-4) M, respectively.

Depression in geriatric and adult medical inpatients.

The authors administered two scales for the evaluation of depression, the Zung SDS and the DFS (Depression Factor Score), derived from the SCL-90, to two groups of medical inpatients: One composed of adult subjects (N = 201) and the other of geriatric subjects (N = 178). The results confirm a high presence of depressive symptoms among patients with medical problems, particularly among geriatric subjects; the prevalence of depression was 20.39% for the adults and 42.13% for the elderly patients. High indices of correlation between the two scales were found for both populations. A multiple regression analysis indicated that the factors most predictive of depressive symptoms are female sex and the presence of somatic symptoms likely to lead to disability for elderly subjects and low social class and female sex for adults.

Initiation, elongation and pausing of in vitro DNA synthesis catalyzed by immunopurified yeast DNA primase: DNA polymerase complex.

Yeast DNA primase and DNA polymerase I can be purified by immunoaffinity chromatography as a multipeptide complex which can then be resolved into its functional components and further reassembled in vitro. Isolated DNA primase synthesizes oligonucleotides of a preferred length of 9-10 nucleotides and multiples thereof on a poly(dT) template. In vitro reconstitution of the DNA primase:DNA polymerase complex allows the synthesis of long DNA chains covalently linked to RNA initiators shorter than those synthesized by DNA primase alone. The SS (single-stranded) circular DNA of phage M13mp9 can also be replicated by the DNA primase:DNA polymerase complex. Priming by DNA primase occurs at multiple sites and the initiators are utilized by the DNA polymerase moiety of the complex, so that almost all the SS template is converted into duplex form. The rate of DNA synthesis catalyzed by isolated yeast DNA polymerase I on the M13mp9 template is not constant and is characterized by distinct pausing sites, which partly correlate with secondary structures on the template DNA. Thus, replication of M13mp9 SS DNA with the native primase:polymerase complex gives rise to a series of DNA chains with significantly uniform termini specified by the primase start sites and the polymerase stop sites.
