[Cranio-maxillo-facial fibrous dysplasia].

The fibrous dysplasia of the bones is a big disease, but it has to be considered as an expansive process with local malignancy in the cranio-maxillo-facial localizations. After a review of the literature, 7 cases of fibrous dysplasia are presented. The only curative treatment is surgical, generally mutilating, it has to be completed by a restoring intervention in same time. The abstention and the modeling surgery can be indicated exceptionally, it requires a regular X ray examination, and a clinical follow-up.

[The vallecula: an anatomic reality?].

The vallecula epiglottica is a borderline area between the pharynx and the larynx, though covered by the epiglottis, and a functionally strategic site and breeding ground for epidermoid carcinoma, but it has remained unappreciated by anatomists. The borders of the vallecula within the oropharynx are represented by the anchoring systems of the epiglottic cartilage; the basilingual margin is sometimes inaccurate. We would like to emphasize the topographical relationships in the vallecular region, in particular: --the proximity to the base of the tongue, which is the preferred are of extension of epitheliomas, --and the features of the hyoepiglottic membrane, which is a veritable barrier for neoplastic infiltration. The valleculae epiglotticae are a definite oropharyngeal anatomical entity, and deserve the attention of maxillofacial surgeons, who should be able to suggest a surgical treatment of cancers that would be practicable both from a functional and from an oncological point of view.

[Accessory salivary tissue and deceptive ectopia].

Histopathologic diagnosis in three patients presenting with a cervicofacial salivary tumoral mass was respectively an inflamed accessory salivary glands, two small islets of ectopic salivary glands and an ectopic salivary duct.

[Post-fracture osteitis of the mandible and gentamicin acrylic cement pellets (author's transl)].

The authors propose a method for osteosynthesis of mandibular fractures in a stable and even solid manner, to avoid the inconveniences of mandibulomaxillary fixation. These include: -- the duration of the operative period, -- the necessary close supervision to avoid secondary displacement, -- desmodontous lesions, -- the adverse effects on mouth hygiene, -- the nutritional disturbances with subsequent loss of weight. There is also the ever-present risk of infection, particularly in the dentulous zone where a fracture line crossing a dental alveolus is permanently exposed to the risk of infection from the buccal flora. Three cases are described in which gentamicin acrylic cement pellets were employed to treat bone infections after osteosynthesis of mandibular fractures, with subsequent healing of the septic foci.

[Prevention of caries and osteonecrosis through daily local fluoridation in radiotherapy patients].

The healthy dentition of 432 patients treated by radiotherapy for ENT and salivary gland lesions was preserved and subjected to a programme of strict hygiene and daily local fluoride application using a gel containing 1% of sodium fluoride. Effectiveness in terms of the prevention of caries was satisfactory. Caries developed in only 7% of the patients. None of the patients in whom there was a clear indication and who followed treatment carefully to an extent greater than 70% suffered decay. Thorough initial assessment, and very careful selection of cases should make it possible in the future to avoid the majority of sources of failure observed. The prevalence of osteo-necrosis secondary to dental preservation was 1.2%, identical to the prevalence of trophic complications seen in patients who underwent total extraction before radiotherapy. The technique of fluoride application is simple, with no risk of toxicity, but must be continued indefinitely. Preservation of the dentition must be undertaken only with the close cooperation of the responsible medical and odontological teams, the latter being responsible for the initial assessment, care and surveillance. Preservation of a healthy dentition together with the possibility of early fitting of a prosthesis if necessary contribute to the rapid functional rehabilitation of patients irradiated for malignant disease in the ENT area.

[Dacryography in the repair of the lacrymal pathways (author's transl)].

Dacryography is the only means of exploring the permeability of the lacrymal ducts and to conclude as the whether watering of the eyes is organic or functional. Pre-operatively, it is indispensable in order to orientate the surgical intervention, it is definitely indicated if there is a relapse of watering of the eyes. The appearance of new surgical techniques, such as the use of venous or arterial grafts, has given rise to a new interest in this technique, which up to now had been considered as an exceptional investigation.
