Growth and metabolic and hormonal profiles during transpyloric and nasogastric feeding in preterm infants.

The effect of transpyloric and nasogastric feeding on the blood concentration of glucose, lactate, pyruvate, glycerol, hydroxybutyrate, acetoacetate, alanine, insulin, pancreatic and total glucagon was determined in 20 preterm infants. The babies were studied on the last day of transpyloric feeding and the first and fifth days of ensuing nasogastric feeding. In 9 infants hourly measurements of hormones and metabolites were made at 1000, 1100, 1200, 1300 and 1400 hours. The blood concentrations of glucose, alanine, pancreatic and total glucagon were stable, the concentration of the other metabolites and insulin, less so. No significant difference in mean metabolite or hormone concentration was noted by time of day or type of feeding. Measurements made on the fifth day of nasogastric feeding showed no significant differences from those at the time of changeover. The infants were clinically well and growing normally at the time of study, but had low plasma insulin and high plasma glucagon concentrations. We conclude (i) the site of presentation of milk in the gastrointestinal tract has no effect on the circulating concentration of selected metabolites and hormones in the preterm infants, (ii) the preterm infant grows at a normal rate with a plasma insulin/glucagon ratio that in the adult would be expected to favour catabolism.

Essential fatty acid status in premature newborns fed by nasoduodenal technique.

The fatty acid composition of plasma phospholipids and the triene (20:3, n=9) to tetraene (20:4, n=6) ratio in the same lipid fraction have been determined, as indices of the essential fatty acid status, in a group of 22 healthy premature newborns fed by the nasoduodenal technique. Determinations have been carried out on the first and fifth day of life for all 22 newborns, and also on the tenth and 30th day of life for 12 of them. The normal plasma levels of essential fatty acids obtained indicate a satisfactory utilization of dietary essential fatty acids under these unusual nutritional conditions.

Nasoduodenal feeding in high risk newborns.

Recent reports indicate that nasoduodenal feeding is a suitable technique for high risk newborns, in particular those with a low birth weight. In the period between February 1975 and June 1976, 100 high risk newborns, divided into four cohorts on the basis of birth weight in 500 g divisions, were fed by nasoduodenal tube. The effect of nasoduodenal feeding has been measured in several ways, but particularly consideration has been given to weight gain, its caloric cost, the mortality rates observed in the Unit before and after the introduction of this nutritional regime and complications. The use of nasoduodenal feeding abolished the physiological loss of weight which normally occurs during the first week of life and was associated with a subsequent rate of weight gain equal to or better than that observed in infants fed by other routes. The mean caloric intake was greater in smaller infants than in larger ones and it was accompanied by a steady decrease in weight gain per unit of caloric intake with increasing birth weight. In 1974 the overall mortality rate in the Unit was 18% and in 1975 following the introduction of nasoduodenal feeding fell to 11.4%. No serious complications were observed associated with nasoduodenal feeding. The conclusion is that this form of feeding is particularly suitable for infants with a birth weight of 1 500 g or less.

Scrotal involvement in Madelung disease: clinical, ultrasound and MR findings.

Multiple symmetrical lipomatosis, also known as Madelung disease, Launois-Bensaude syndrome, and benign symmetrical lipomatosis, is an uncommon disease that is characterized by the presence of multiple and symmetrical deposits of nonencapsulated adipose tissue in the neck, upper trunk, arms, and legs. It affects mainly men, with a higher incidence in the Mediterranean area, and is typically associated with high alcohol consumption and a high prevalence of peripheral neuropathy that usually appears long after the development of lipomatous masses. We report the case of a patient with involvement of the scrotum. We present clinical, ultrasound, and magnetic resonance appearances of these scrotal lipomatous masses for the first time.

CT evaluation of myometrium invasion in endometrial carcinoma.

Myometrium invasion (M) is one of the principal prognostic factors in the early clinical stages of endometrial carcinoma and can be evaluated presurgically only by CT, although with conflicting results. We compared CT of 65 patients with early clinical stage endometrial carcinomas with the corresponding anatomopathological findings. Myometrial infiltration of the same degree may present different CT images. Therefore, we identified five fundamental CT patterns, each of which corresponded to one of the three degrees of myometrium infiltration (M1, M2, M3). Furthermore, we defined the infiltration index as the ratio of minimum free myometrium to maximum free myometrium. Overall diagnostic accuracy was 76%; however, for clinical purposes CT provided adequate guidelines for therapeutic decisions in 93% of the cases. These criteria proved to be less reliable in elderly women with atrophic myometria, especially when the neoplasia was polypoid in shape.

Independent factors predict supranormal CA 15-3 serum levels in advanced breast cancer patients at first disease relapse.

Data currently available are insufficient to demonstrate a real utility for CA 15-3 in the diagnosis, staging or surveillance of breast cancer patients following primary treatment. The aim of this study was to determine if there was a correlation between supranormal CA 15-3 serum levels and clinical and biological variables in breast cancer patients at first disease relapse. From October 1988 to March 1998, 430 consecutive patients entered the study. Overall CA 15-3 sensitivity was 60.7%. Elevated CA 15-3 levels were found more frequently in patients with liver metastases (74.6%) and in those with pleural effusion (75.7%). CA 15-3 sensitivity was 70.4% in patients with estrogen-receptor-positive (ER+) primary tumors and 45.9% in those with estrogen-receptor-negative (ER-) tumors (p < 0.0001). In patients with a limited extent of disease, marker sensitivity was 57.7% in ER+ tumors and 25.7% in ER- tumors (p < 0.0001). Logistic regression analysis showed ER status, disease extent and pleural effusion as independent variables associated with CA 15-3 positivity. The multivariate Cox analysis showed ER and disease extent as independent variables predicting overall survival, whereas CA 15-3 failed to be statistically significant. CA 15-3 was an independent variable only when the disease extent variable was removed. This study suggests that CA 15-3 in advanced breast cancer patients is a marker of both disease extent and ER status. The direct relationship with ER status indicates that CA 15-3 diagnostic sensitivity in the early detection of disease recurrence could be greater in ER+ patients than in ER- ones. Furthermore, this suggests that patients with elevated CA 15-3 levels could have disease that is more sensitive to hormone manipulation than those with normal CA 15-3 values.

Taurine and cholesterol supplementation in the term infant: responses of growth and metabolism.

The effects of taurine and of cholesterol supplementation to a whey-protein-predominant formula fed ad libitum on the growth and biochemical responses of term infants were studied. The responses of these infants were compared to those of infants fed formula without a supplement and infants that were breastfed. These infants were followed for 12 weeks. No effect of cholesterol was observed on any of the measurements and no effect of taurine was observed on any of the biochemical measures with the exception that taurine concentrations in plasma and urine (reduced in infants fed formula alone) were corrected to concentrations observed in breastfed infants. Taurine supplementation had no effect on the rate of weight gain or on linear growth over the complete course of the study. Thus, taurine supplementation of formulas returns plasma and urine concentrations of this amino acid to those found in human milk-fed infants. However, these data indicate that the supplementation of formulas with taurine has no benefit with respect to growth.

Does concomitant chemoradiotherapy influence cosmetic outcome in conservative treatment of breast cancer?

PURPOSE: To evaluate retrospectively factors influencing the cosmetic outcome after conservative treatment for breast cancer. MATERIAL AND METHODS: From 1988 until 1992, 164 patients were treated with conservative surgery (quadrantectomy) and radiotherapy with 60Co (50 Gy on the whole breast) plus 10 Gy on the surgical bed (300 kV photons) for T1-T2 breast cancers; 46 patients (28%) received concomitant adjuvant chemotherapy (CMF schedule). Cosmesis evaluation was carried out after 24 to 108 months (median, 38 months). A logistic regression analysis was performed to identify independent variables influencing the aesthetic outcome. P values of 0.05 or less were considered significant. RESULTS: Univariate analysis showed that T2 versus T1 (P = 0.0102), lower quadrants site (P = 0.0002) and concomitant adjuvant chemotherapy (P = 0.0009) produced a worse aesthetic outcome. Multivariate analysis confirmed the same factors: tumor size (P = 0.0020), tumor site (P = 0.0150) and concomitant chemotherapy (P = 0.0024). CONCLUSIONS: The significant negative influence on the cosmetic outcome of concomitant adjuvant chemotherapy implies questions about the timing of radiotherapy and chemotherapy in breast cancer conservative treatment.

[A multicenter epidemiological study of burns of the head in childhood in the period of 1986-1990].

A retrospective study was performed over the children admitted to three regional pediatric burn units, over a 5-year period (1986-1990). Out of 695 pediatric patients, 270 (38.8%) were admitted with burns of the head and neck. This whole childhood age group (up to 14 years of age) was examined epidemiologically. The age group presenting the highest incidence was within 0-3 years (71%); scalds were involved in high percentage (68%) and many injuries occurred at home (93%). Epidemiological aspects of the report are discussed with particular consideration of preventive programs.
