[Combination of malignant tumors of the genitourinary system in the same patient: testicular seminoma, renal adenocarcinoma and adenocarcinoma of the prostate].

A rare case of neoplastic association is reported in a 74-year-old patient who successively presented with right testicular seminoma, left renal clear cell adenocarcinoma, and adenocarcinoma of the prostate. This uncommon association of primary tumors in the same patient and the course of the disease are described and the cases previously reported in the literature are briefly reviewed.

[Risk of abnormal absorption of lead in glass decoration workers].

Following the observation of an abnormal lead absorption (PbB 72 micrograms/100 ml) in a young female glass decorator on a routine laboratory test, blood lead levels were measured in glass decorators of the entire area of the Local Health Unit of Treviso (Northern Italy). Abnormal lead absorption was found in a large number of glass workers, the source of exposure being the high content of inorganic lead in the "low-melting" paints that were used (brush and spray painting). Most of the exposed workers were females of fertile age, for whom even a moderate exposure to inorganic lead may constitute a risk during pregnancy. Attempts to reduce lead exposure levels by means of a health education programme and environmental improvements at the workplace were unsuccessful. In conclusion, a reduction in lead absorption in female glass decoration workers can only be achieved by using "low-melting" paints with a lower lead content.

[Feeding the newborn infant with respiratory insufficiency].

In feeding the newborn with respiratory distress it must be considered the increased metabolic and water requirements. To feed this newborn we can choose among total parenteral nutrition, total enteral nutrition and a combination of the two methods. Data concerning 98 newborns with respiratory distress, admitted into Neonatal Intensive Care Unit of the Provincial Maternity Hospital of Milan, are described.

[Substitutive therapy with surfactants in premature infants with respiratory distress syndrome].

The main pathogenetic moment of the respiratory distress syndrome (RDS) is recognized to be the lack of alveolar surfactant. Many attempts of substitutive therapy with several kinds of surfactants have been tried (artificial, synthetic, from amniotic fluid, exosurf). After the considerable results obtained in animals, the use of substitutive surfactant proved to be useful also in human newborns with RDS. The authors report the personal data relative to the TA-surfactant (artificial surfactant made of phospholipids + DPPC + protein substances extracted from bovine lung). They analyse first results and, on this ground, they put forward some assumptions in order to obtain a more rational and efficient use.

[Correlation of the DNCB skin test and CEA assay in the prognosis of malignant neoplasms].

130 neoplastic patients were subjected to DNCB skin tests and radioimmunological CEA measurements during antiblastic and/or radiation treatment. The prognostic reliability of the two techniques was examined as was the connection between DNCB response and CEA plasma levels. Results obtained confirm the link between development of the neoplasia and immunological factors in the host organism in 73% of the cases. The connection between CEA measurement and DNCB test was noted in only 56%. Finally CEA measurements of haematic levels alone corresponded to the clinical development of the condition in 71% of the cases examined.

Prognostic factors in metastatic breast cancer patients obtaining objective response or disease stabilization after first-line chemotherapy with epirubicin. Evidence for a positive effect of maintenance hormonal therapy on overall survival.

Randomized trials suggest that the outcome of metastatic breast cancer (BC) patients is not affected by the currently available therapies. Although response rates per se may be associated with survival prolongation, patients experiencing objective response may be those patients fated to have the longest natural disease history. The separation of responders from progressing patients after first-line chemotherapy could allow the selection of a more homogeneous subgroup in which further treatment strategies might achieve a better control of the disease. This study investigated the influence of some patient characteristics, disease characteristics, and previous treatments on the outcome of non progressing patients after first-line chemotherapy with epirubicin administration. We also evaluated the effect of the maintenance endocrine therapy in improving response rate and overall survival (OS). From May 91 to May 93, 207 patients were enrolled in a randomized trial aiming to compare the activity of epirubicin (120 mg/sqm) +/- lonidamine (600 mg/daily). Among the 169 patients attaining complete (CR), partial response (PR) or disease stabilization (SD), 65 were not randomly submitted to maintenance endocrine therapy (MET). Liver involvement, previous adjuvant chemotherapy and previous hormonal therapy (administered in adjuvant setting or for advanced disease) were found to negatively influence OS both in univariate and multivariate analysis. Differences in OS stratifying patients according to DFI, estrogen receptor status and PS did not attain statistical significance. Patients receiving MET survived significantly longer than those submitted to observation and this difference maintained the statistical significance also within patient subsets homogeneous for specific prognostic features. In conclusion, most prognostic factors for advanced BC have been confirmed in our series of patients obtaining CR, PR or SD to full dose epirubicin. The positive prognostic impact of MET is impressive and deserves confirmation in randomized studies.

[Environmental exposure and blood levels of benzene in gas station attendants. Comparison with the general population].

Environmental benzene levels were measured in 26 petrol stations using both active and passive stationary and personal samplers. Simultaneously, benzene levels were measured in the petrol station operators on blood samples collected at the end of the work shift and the following morning before starting work. The petrol stations belonged to various different oil companies and were studied both during the winter (9 stations) and in the summer (17 stations). The environmental levels measured with active samplers in the 26 stations were on average 256 ng/l, were significantly lower (98 ng/l) in winter and higher (326 ng/l) in summer. The blood levels of benzene in 77 workers at the end of the work shift were on average 548 ng/l, were significantly lower (306 ng/l) in winter and higher (651 ng/l) in summer. The following morning, blood levels of benzene were lower than those found at the end of the work shift, on average 249 ng/l in winter and 427 ng/l in summer. Smokers had higher benzene levels than non-smokers, both in winter at the end of the work shift (617/170 ng/l) and the following morning (506/137 ng/l), and in summer at the end of the shift (742/517 ng/l) and the following morning (535/233 ng/l). A comparison with a sample of 243 "normal" subjects of the general population showed that their mean blood level of benzene of 165 ng/l was significantly lower than the level found in petrol station workers the morning after the work shift (364 ng/l).

[Asthma and rhinoconjunctivitis caused by rape flour: description of a clinical case].

A 48 year old man, employed in a grain and animal feed store for 9 years, was referred to our clinic complaining of nasal blockage, rhinorrhea, sneezing, ocular burning, coughing and wheezing occurring over the last 12 months. The man's main task was to manually load and unload the unpackaged grain and feed. Symptoms occurred only when he worked directly with oilseed rape flour and not when he worked with other types of grains. Eye and nasal symptoms appeared during the work shift, while respiratory symptoms were worse at night than during the day after exposure to rape for more than 2 consecutive days. Physical examination was normal, as were the results of the pulmonary function studies. The methacholine inhalation test, performed to measure the level of non-specific airways responsiveness, showed normal bronchial reactivity. Results of allergy skin prick tests were negative for common inhalant and food allergens, but slightly positive for the oilseed rape flour extract. Registration of the peak expiratory flow (PEF) showed slight decreases in PEF values after occupational exposure. We conclude that this case is suggestive of asthma and rhino-conjunctivitis, induced by oilseed rape flour, probably due to an allergic mechanism.
