[Long-term observations on the etiology and antibiotic resistance in urinary infections (1971-1987)].

The activity "in vitro" of six antibiotics (co-trimoxazole, cephaloridine, nitrofurantoin, carbenicillin, nalidixic acid, gentamicin) was tested against four Gram negative: E. coli, Pseudomonas sp., Klebsiella sp. and Enterobacter sp. isolated from urine samples from 1971 to 1987, at the Istituto di Igiene of Trieste. The temporal analysis of antibiograms revealed an decrease of the incidence of resistance in the year 1979, 1980 and 1981. In the following years the percentage of resistance showed an increasing of incidence.

Improving patient flow in ambulatory care through computerized evaluation techniques.

The Authors describe the application of Patient Flow Analysis (P.F.A.), an efficiency evaluation technique in a Gynaecologic Clinic in a large Provincial Hospital. This technique evaluates both patient flow and staff utilization in ambulatory settings. Staff efficiency (percentage of work time spent with patients) was low, (43% for clinicians and 36% for aides, and patients waiting time was excessive (average 2 hours 44 minutes) for only 35 minutes of service time, and could have been substantially reduced by a more personalized appointment pattern and punctual starting of clinics by the clinicians. The usefulness of Patient Flow Analysis is clearly demonstrated.

[Nickel and chromium. Analysis of 3 detergents using atomic absorption spectrophotometry].

It is well known that Nickel and Chromium are present, at different concentrations, in detergents used for washing-up, hand-washing and cleaning surfaces. In the present study we have analysed three trade marks of detergents (one for each different use) to evaluate, by means of atomic absorption spectroscopy, whether the concentrations previously detected are specific to a particular trade mark or are connected with the metals concentration variability typical of all detergents as commercial products. According to our results, Nickel and Chromium are always present, at different concentrations, in all the samples tested; detergents used for hand-washing contain the highest Nickel percentage; the concentrations observed show discrete variations also within the same trade mark.

[Availability, consumption of alcoholic beverages and alcohol-related mortality in Italy].

On the basis of the W.H.O. guide lines the relevant parameters for the study of alcoholism in Italy have been identified. The production and utilization of alcoholic substances is very high, then appears to have reached a plateau about ten years ago. Likewise, consumption has not increased since a few years. Hospital admissions for symptoms related to alcoholism are no more available after the closure of all mental hospitals in 1978. Alcohol-related deaths seem to be stable, as well as cause specific death rates for cirrhosis .

[A monitoring system for occupational cancer].

INTRODUCTION: The Italian Occupation and Safety Act (d.lgs 626/94) provided for the establishment of a nationwide occupational cancer registry, under the National Institute for Occupational Health and Safety (ISPESL), with the aim of detecting cancer cases of occupational origin and estimating the influence of occupation in cancer causation. METHODS: Information on cancer cases, drawn from six Italian population-based cancer registries (CRs of Friuli Venezia Giulia Region, Genoa Town and Genoa Province, Macerata Town, Umbria Region, Varese Town, Veneto Region), and on a random sample of population controls selected in each CRs area were linked with data on subjects employed in private enterprises that have been available in electronic form since 1974 at the National Institute for Social Security (INPS). In this way, both for cases and controls, the occupational histories of past employment were collected. A population-based case-control study covering the period 1990-1998 was carried out with the aim of estimating occupational cancer risk in the private sector by site and economic category in each area. Since one of the major drawbacks of this approach is the difficulty in distinguishing true occupational hazards from incidental findings derived from multiple comparisons, an extensive research of occupational literature was carried out, independently of the study results, to compare our results with existing knowledge on occupational risks. RESULTS: Pooled analysis of the most recent incidence data based on 36,379 cases and 29,572 controls was performed; 34 "statistically significant" associations were found for 11 economic categories. Using our literature review, 10 associations were supported by more than 5 publishedpapers, 14 by a number of papers between 1 and 5, and 10 associations had not been previously reported. CONCLUSIONS: This system appears suitable for assessing existing occupational cancer risks and can eventually lead to detecting occupational hazards in many areas of Italy. The system can also provide a list of cases suitable for in-depth search for past occupational exposures.
