[Experimental trial of intraoperative spinal ultrasound].

Dorsal operative stabilization of spinal fractures by internal fixation is an established means of treatment directed at releasing the extrinsic compression of the intraspinal contents caused by the fractured posterior surface of the vertebral body. There are few ways of assessing how successful the repositioning is intraoperatively. Intraoperative ultrasound with a special small transducer is possible, but it is difficult to reach the intraspinal area by this means, because it is surrounded by bones. A laminectomy would destabilize the injured vertebral column even further. By experiments performed on six human vertebral columns we show that it is possible to view the entire length of the fractured posterior surface by ultrasound. The beam has to be projected between the laminae vertebrae following a small laminotomy. Preliminary intraoperative experiences with eight patients confirm our experimental results. The intraoperative ultrasound results have been confirmed by preoperative and postoperative CT. We expect the necessity for laminectomy for examination of the posterior surface of the vertebral body to decline in frequency in the near future.

[Winter sport injuries in childhood (author's transl)].

3374 accidents occurring on the field of sport during the years 1975--1977 accounted for 19% of all accidents dealt with at the Institute of Kinderchirurgie in Graz. 51% of the accidents were caused by the typical winter sports: skiing, tobogganing, ice-skating and ski-jumping with skiing accounting for 75% of the accidents. The fracture localization typical of the different kinds of winter sport is dealt with in detail. The correct size and safety of the equipment were found to be particularly important in the prevention of such accidents in childhood.

[The value of scintigraphy and osteoscintimetry for the early detection of legg-calvé-perthes disease (author's transl)].

Scintigraphy and the stimultaneously effected osteoscintimetry of the pelvis and of both hip joints offers the possibility of a safe early diagnosis of Perthes' disease directly after manifestation of the clinical symptoms, two months before the roentgenologically identifiable, discrete changes of stage I have occurred, or four months before the clear roentgenological visualisation of the disease (stage II). Moreover, scintigraphy enables detection of the time at which restitution or revascularisation of the necrotic bone of the epiphysis of the head of the femur sets in, and to draw further conclusions from the scintigraphic and scintimetric pattern of results, on the prognosis and further course (with time) of Perthes' disease.

[Bone scan of acute hematogenous osteomyelitis in childhood (author's transl)].

A variety of 99m-Tc-phosphate compounds are sensitive and reliable for scintigraphic diagnosis of bone diseases. Therefore bone scan has become a safe method for evaluating patients with suspected acute hematogenous osteomyelitis. So we were able to perform early and adequate treatment without evidence of bony changes in the radiograph. Quantitative regional measurements with a relative uptake ratio was found to be a clinically useful parameter in the follow-up of osteomyelitis.

[Septic arthritis and osteomyelitis as a complication of neonatal intensive care (author's transl)].

In a prematurely born infant of a diabetic mother a purulent arthritis with osteomyelitis of the elbow occurred 18 days after an enterobacter aerogenes-septicemia was proved. Enterobacter (aerobacter) aerogenes is a nosocomial gramnegative germ, that is more and more regarded responsible for infection of the newborn in intensive care units. In accordance with the literature recording a high rate functional loss after septic arthritis also our patient suffered from a mobility deficiency in his right elbow a year after the onset of the disease.

Anti-Bacterial Agents

[Osteosynthesis and simultaneous anchorage of a plastic hip cup in an unstable acetabulum (author' transl)].

Attempts at total hip replacement in the presence of unstable acetabula--which may be caused by a recent fracture or pseudarthrosis--are doomed to failure if conventional implantation technics are used. Such attempts lead inevitably to a rapid loosening of the cup. This article describes a newly developed implant which simultaneously re-establishes acetabular stability while solidly anchoring the cup. Use of this implant contributes to early patient mobilization and undisturbed healing of fractures and pseudarthroses.

[Cervical myelography after lumbar application of metrizamide (author's transl)].

Cervical myelography by means of lumbar application of metrizamide was performed on 110 patients. The exploration technique, the results, the quality of the myelograms, and the side effects observed are discussed. The most frequent complaint was headache. There were also cases of vertigo, vomitus, pain in the back and legs, and one case of tachycardia. Complications of a more serious nature, in particular epileptic seizures, did not occur.

[The social and medical situation of people suffering from multiple sclerosis--considerations on the basis of an epidemiologic field study (author's transl)].

Within the framework of an epidemiologic field study conducted in South Niedersachsen a standardised questionnaire was used to obtain data on the social and medical situation of persons with multiple sclerosis. Of the 195 people with multiple sclerosis questioned in that area in 1976 answers were received from 178 (91%). The mean age was 46 years, the average history of disease 15 years. Family status and social position corresponded to social average. A social downfall as a result of the disease was not found to have taken place. Approximately 50% who had been employed prior to the disease (N = 147) had given up their jobs at the time of the survey. The monthly net income was found to be average. Of the studied persons 42% were living in their own houses or flats. A lower percentage, compared to the general public, was living in lodgings (58% instead of 65,7%). The majority (56%) was living in flats suitable for the disabled. 13% of the surveyed persons lived alone, 5% in homes. Social care was found to be very poor in comparison with medical treatment. The introduction of compulsory notification for this disease was advocated by 83% of the people questioned.

Serological response of multiple sclerosis patients and controls to 6/94-parainfluenza virus.

The serological responses of 195 multiple sclerosis (MS) patients and 251 controls were tested against 6/94-parainfluenza virus, which was previously isolated from brain tissue of two patients with MS. The hemagglutination-inhibition titers of greater than or equal to 1: 128 were found more frequently in MS patients (21.5%) than in controls (14.0%). However, the geometric mean titers did not differ between these two groups. The present study concludes that a causal relationship of 6/94-virus to MS, based on a specific immune response, is improbable, although it does not exclude the possibility of a pathogenetic significance of the agent in the cases from which the autopsy material was derived.

[The force flow in hip joint-endoprostheses (author's transl)].

The mechanical force flow distribution produced by the hip joint endoprosthesis in the remaining, living femur and its deviation from that of the natural bone have a considerable influence on the quality of the bone-endoprosthesis compound and its duration of life. The loads are calculated for the natural bone as the reference system as well as the compound with conventional endoprostheses and a new type, the Tension-band-Endoprosthesis (R-G). For the calculation the finite element method is used by application of three-dimensional tetrahedron elements. It becomes apparent that in the case of the R-G-Endoprosthesis the induced stresses within the medial range of the remaining bone are far less unphysiological than in the case of the conventional endoprosthesis.
