[Senior sports in science and general practice].

Aging does not just mean diminishing of abilities. It means gain of new qualities too. Modern sports medicine as well as medicine in general has developed its own specialty: sports medicine for seniors, establishing age specific possibilities in sports, and diminishing its possible negative outcomes to a minimum. The goal is not just to achieve physical results in seniors. What counts even more are psychohygienic possibilities in the sense of contentment, integration into family society, contacts to youth. Happily enough youngsters too articulate same opinions about sports for seniors. Thus sports of the aged is by no means just a matter for a single outsider who wants to demonstrate physical top performances. It is of important social value. It does not just regard sport associations but all of us: doctors, politicians, opinion leaders, present or future seniors.

[Peak performance athletes speak with physicians about common problems--a panel discussion].

The sports-physician sees the necessity of a holistic view resulting out of the wishes of top ranking athletes in contrast to a quick temporary recovery of unsatisfying duration. On the other hand one must consider the short active period of the athlete and adapt less time consuming diagnosis and therapy. The sports-physician has to be aware of the stressfield resulting out of a polarity of "immediate" versus "perfect" therapy, and national and economic interests. This means an everlasting inner conflict for the sports-physician. Holistic view means also to consider the personal and the professional development. This is important in children-sports especially to prevent the loss of a playful and funny dimension and to offer different kinds of sports. Without the ambition of the parents there will be no children top ranking sports. Exaggerated parents ambitions however want cautious medical interference. There is still a big lack in permanent psychohygienic care for top athletes reaching from professional training, social contacts to mental training and/or psychotherapeutical dimensions. A permanent, interested, integrative cooperation between sports-medicine, science of sports, psychology of sports, organisation and last but not least athletes themselves leads not only to better results but also to better human value of every day life before, during and post active period.

[Autogenic training (AT)--respiratory feedback (RFB); similarities and limits in general practice oriented differential indications].

Autogenic Training (AT) is not merely a "relaxation exercise". We quote five dimensions of AT that should by clearly differentiated: 1) concentration, 2) relaxation, 3) vegetative switch toward the hypnoid state, 4) dynamising reprise exercise. 5) holistic inclusion in a complex psychotherapeutical concept. We consider the self induced hypnoid state plus dynamising reprise exercise as the main components for the beneficial effect of AT. The hynoid state has its neurophysiological definition as a third human state of consciousness. The reprise exercise is more than making it abolish, but has its own therapeutical values: dynamisation. Respiratory feedback (RBF - a special kind of biofeedback) has the factors relaxation and vegetative switch towards hypnoid in common with autogenic training. Where these components are specially requested, RFB proves valuable in therapy. In other indications however AT is of much better use. Namely: where the educational value of mutation, self-concentration and the therapeutic context is of predominant importance. AT as well as RFB may be considered "misused" in having them replace the therapeutical and understanding conversation between doctor and patients, in a mechanistic way. As a slogan it may be said: AT goes deeper, RFB goes quicker.

[Stroke. Status and general practice of conservative treatment and rehabilitation].

Adequate help for the apoplectic patient according to recent standards can be given only by simultaneous and equivalent orientation towards both acute therapy and rehabilitation, which should merge smoothly into each other. This can be achieved only by specialised hospitalisation. The idea of totally domestic treatment, as advocated in times past, has to be abandoned for good. In the foreground of acute therapy are treatment of edema, cardiac output and blood pressure, rheological factors: blood viscosity, thrombocyte aggregation, erythrocyte elasticity. In the foreground of rehabilitation efforts we place the principle of "totality and permanence" regardless of prognostic selectivity. The principle of permanence refers to rehabilitative efforts from the very beginning to long-term rehabilitation for months and years with the help of our special rehabilitation service and systematic training of relatives. At least, it has to be stressed that the rehabilitation patient is not a medical waste product and not a symbol of medical surrender either. It would be unfair and senseless to help him through the acute phase and leave him alone during his rehabilitation period. Permanent attention for the apoplectic patient is essential within a system of social medicine. Much is lacking in this respect, as far as academic institutions, advanced training, and information is concerned. We hope that we herewith demonstrate that not only the acute therapy, but also the rehabilitation of the apoplectic patient is a field of scientific fascination, still being in consequent further development. It is intended to give an additional stimulus for the optimal after-care of the apoplectic.

Cerebrovascular Disorders↗

[Long-term neurorehabilitation].

Our slogan of rehabilitation to be "holistic and permanent", in terms of its holistic aspect, expresses inclusion of the patient's total environment--relatives, politics, administration, legislation. The aspect of permanence may involve periods of many years, and is not amenable to standardization; meaningful limitation is subject to the individual needs, based on critical scientific follow-along of rehabilitation. The emergence of "rehabilitation hospitalism" may best be prevented by proper attention to the patient's emotional situation and social environment, with special regard also to experiences of frustration, to regressive tendencies, "associated depression". Well-versed rehabilitative management should therefore include the dimensions of pleasure and diversity. We have been able to realize some of the above in the "Vorarlberg Rehabilitation Model", in terms, on the one hand, of the scientific identity of rehabilitation, and of individualized, optimum patient-orientedness on the other. An important step forward has moreover been the inauguration of a "Ludwig Boltzmann Institute for Neuro-Rehabilitation and -Prevention" at the Landes-Nervenkrankenhaus Valduna, of Rankweil, Austria.

[A hierarchy of findings within the scope of cerebrovascular diagnosis today].

Table 5 gives an overview as to the timing and indication for diagnostic methods and procedures in cerebro-vascular diseases. Anamnesis and clinical aspects have still leading position. After that, computer-tomography stands first in the diagnostic line, followed by angiography, indicated in the acute phase only in progressive stroke. More often it is used during rehabilitation period for indication towards extra-intracranial anastomosis. Sonography gives informations specially in the extracranial stenoses by duplex sonography. The EEG is still necessary in the early stage and for follow-ups. Cerebro spinal fluid analysis, intern medicine (mainly cardiology) examinations, give extra information and should not be skipped. Psychological exams and intracranial bloodflow-studies are used during rehabilitation. Digital angiography, with the possibility of postprocessing, is in comparison to conventional angiography, less invasive. Nuclear-magnetic-resonance with its fast development is taking now place in the clinical field. Cerebro-vascular patients should be treated in specialized centers where beside the equipment, an experienced team is available, which has required its knowledge on a high number of cases and where the cooperation between the different fields of conservative (intern and neurology) radiology and surgery is guaranteed. This paper however has tried to show the up-to-date-guidelines to the other medical specialties that handle cerebrovascular patients.

[Old age in discussion].

Many questions have arisen in connection with the problems of advanced age. They cannot be solved by means of our overcome medical knowledge alone, but require constant open-mindedness. It could be demonstrated how important the dialogue is between legislation, administration, medicine, sociology, and psychology. Only this will enable us to keep pace with the increased significance of the complex problems of advanced age. Accordingly, dialogues like the present one should be continued, as we sincerely hope.

[Old age as a special and integrated field, from the neurologic viewpoint].

Geriatrics and rehabilitation show a wide overlapping. On the one hand, the approach has to be of a specialized nature, while, on the other hand, both spheres can be dealt with in an interdisciplinary context only without strict boundaries between medical and non-medical fields. Our phrase "via neuro-rehabilitation towards a social neurology" complies with the increase in the geriatric clientele and emphasizes the social necessity of long-term care for the patients inabled by geriatric disorder. Important instruments in this respect are the training of relatives and the follow-up rehabilitation service, the latter having been established in Vorarlberg as the first of its kind within the German-speaking countries. The specific geriatric aspect of medication have to be known and applied. Special reference is given to the goal-directed use of antidepressants in the amelioration of what we got to call "concomitant depression", one of the main causative factors in the so-called "rehabilitation hospitalism". Goal-Directed psychotherapy in advanced age has been systematically administered and evaluated in our institution. It can be regarded as an essential psychohygienic measure. Worth mentioning are furthermore several other psychohygienic provisions, such as rational and systematic preparation for retirement with appropriate legal measures in order to prevent an artificial restriction of reasonable activity in advanced age.

[Presentation of a survey scale for neuro-rehabilitation--with a general discussion of current rehabilitation scales].

Presented is a newly developed rehabilitation survey scale for the field of neuro-rehabilitation, in which levels of functioning relative to ten specific and four global functions are documented at set intervalls by means of a five-grade rating scale. Additionally, numerous symptoms or associated phenomena are recorded ungraded. Used in the clinical setting, this scale affords a quick overview of the rehabilitation profile, with relatively little time and effort being required. Scientific evaluation may uncover information on the impact the disorders of specific functions have on global functions, which is relevant primarily in the social context. Also, the scale may be applied in the framework of larger-scale comparative studies. Dealt with in an extensive discussion, our newly presented scale may be accorded a number of advantages over currently used scales, which warrant its introduction. It is the outcome of many years of developmental work and pertaining reliability verifications.

[Silent abuse].

The rate of "silent abuse" is probably underestimated by the current esteem of 1:200 in the general population. Because of the criterion of "silent inconspicuousness" most of the patients concerned elude statistical records. So much the more, however, they need medical attention. It is a highly urgent problem, involving individual as well as social medicine, and carrying the risk of massive damage to health, shortening of life expectancy and loss of ability to enjoy creativity and pleasure. The presenting symptoms are mainly headache, sleep disturbances and vegetative manifestations. The (over-)medication in use consists mainly of analgesics, tranquillizers and narcotics. The etiological background is made up (usually in close interdependency) of depression, external circumstances and neurotic development as well as a distinct type of personality (to be understood as the result of interaction between genetic and psychodynamic factors). Especially, there appears a personality structure according to the present ideal picture of achievement and order. Therefore, even from the medical point of view, hardly ever to be regarded as deviant or in need of therapy. In some cases out of this constellation arises a further enhancement of the mechanisms of abuse. Among rational objective measures we can propose: Increased information of the public and further special education of medical people, especially referring to rational therapy of depression and pain without the use of analgesics, furthermore, rational psychotherapeutic guidance; attempts at more specified and follow-up care of disaccustomed abusers; intensified public relation work in cooperation between medical doctors and politicians.(ABSTRACT TRUNCATED AT 250 WORDS)

Headache and (concomitant) depression.

'Concomitant depression' (Begleitdepression) is a term proposed by us to signify presence of somatic illness plus depressiveness at the time in one individual. Headache patients form a main part of patients bearing concomitant depression. Depressiveness can be a sequela of headache, a causative factor of headache, as well as a modifying one. The more severe a headache syndrome is the more it tends to be associated with depressiveness. Cephalea patients are more depression-afflicted than migraine patients, women more than men, higher age group more than lower ones. We have denominated a special 'depressive headache syndrome of higher age groups' for its great frequency and clinical importance. Therapy has to take into account all the causative and accompanying factors at the time; thus to include antidepressant therapy (in a complex way, using drugs beside psychotherapy) together with all indicated further somatotropic headache therapy. As slogan: No either/or-therapy but a comprehensive therapy.

["Typical" and "less typical" sports injuries in the field of neurology].

In times of increasing "sparetime pathology" sequels of sports are getting a new meaning in medicine. Surgeons are mainly involved in this issue but it also contacts neurologists. We differentiate between direct sequels of sports and concomitant incidents of sports. Besides the typical head injuries we saw a number of atypical traumatic damages such as a hit by a golf ball, fall on the buttocks and the header in football. Discprotrusions are mainly understood as concomitant incidents at sports because in most cases one has to consider preexisting disc lesions. Though there are typical motions in sports which rather lead to discprotrusions such as strong torsions of the body in football, serving in tennis, heavy lifting in wrestling or an unexpected fall into a hollow in skiing. In cerebrovascular events, sports is usually only one additive etiological component. Nevertheless the lack of adequate acclimatisation and the brisk beginning of stressing sports in high mountain areas (which is enabled through modern technic) can contribute a great deal to decompensation of cerebrovascular disorders. In this respect doping can have a serious influence since protecting physiological mechanisms are eliminated. Diving accidents are able to give rise to spinal, cerebral and cerebellar gasembolisation. Warning early symptoms of great value are TIA, vertigo and fits. Sport practice can figure as an unspecific test situation in order to provoke these events. Diagnostic investigation of the cardiovascular system and the CNS has to be performed in those cases. Particular regard should be paid to expert situations which tend to distort the clinical pictures.(ABSTRACT TRUNCATED AT 250 WORDS)
