[Effect of heading in soccer on the head--a quantifying EEG study of soccer players].

We observed a patient with epileptic seizures as a consequence of an occipital contre coup injury, due to heading in football. We decided to investigate the EEGs of ten healthy football players by computerized EEG analysis during their "header training". One player's EEG showed focal slowing, there were no clinical symptoms. The EEG of the other players showed only signs of relaxation after training. These results lead us to the following conclusions: - Cerebral damage due to heading in football cannot be assumed in general. - In predisposed persons, however, the microtrauma caused by heading can lead to clinical manifestation. - Subclinical cerebral disfunction due to heading is possible. However, long term studies should be performed to prove that there might be a risk of permanent cerebral damage in football players, as has already been established in boxing.

[Neurology and practical emergency medicine].

From neurological point of view some important acute situations are discussed. These are: seizures, radicular pain attacks, headache-attacks, stroke, impaired consciousness of uncertain origin. The two main criteria of acuity, i.e.: dramatic and dangerous, will combine in many cases, but dissociate sometimes. This has to be considered especially in those cases where acute danger exists although a dramatic picture is missing, for example in subarachnoid hemorrhage with poor symptomatology and in pareses or cross section syndromes without pain. The primary contacted physician is able to contribute a great deal to further diagnosis and treatment through exact primary observation which is important in acute syndromes because of their tendency to change rapidly and to present under different features when the patient arrives in hospital. This concerns above all: differentiation of seizures, differentiation of stroke and differentiation of impaired consciousness. Besides the primary dramatic situation and acute therapy it is important not to neglect the necessity of further diagnostic particularly in seizure patients who recover quickly after the acute situation but nevertheless need to be brought to sufficient diagnostic and treatment.

Acute Disease↗

[Guide-lines to the rehabilitation of patients with neurological disorders, especially stroke (author's transl)].

Rehabilitation requires the interplay of three essential factors, namely medicine, organization and humanity. The concept of entirety and permanency implies that rehabilitation must commence on the very first day of acute therapy and continue, with the aid of relatives, after discharge from hospital. Depression very frequently accompanies a stroke and requires expert handling both with psychopharmaceutical drugs and psychotherapeutical methods (including nursing staff and family) to overcome this hurdle to rehabilitation. Physiotherapy is of paramount importance, with particular emphasis on active movement. Therapeutic management with medicines must continue along the lines commenced in the acute stage. The therapy of stroke requires a considerable involvement of manpower, which must not be denied to this step-child of rehabilitation. Apart from medical considerations, it is salutary to remember that every fifteenth person is likely to be handicapped after a stroke at some time in his life.

Cerebrovascular Disorders↗

[Abnormal cytological findings in cerebrospinal fluid with normal cell count (author's transl)].

A qualitative cytological study demonstrated that normal cell count in the cerebrospinal fluid does not exclude abnormal cytological findings. Siderophages were found as a late indicator of haemorrhage into the CSF. In some cases of tumours of the CNS sporadic atypical cells were found without other alterations in CSF findings. Granulocytes or macrophages were found in vascular diseases, in response to trauma, in tumours and after inflammation of the meninges as an indicator of persistent minimal inflammatory activity. Two cases of Heredopathia atactica polyneuritiformis (Refsum's syndrome) showed characteristic vacuolar macrophages in the CSF (described for the first time), in spite of a normal cell count. The findings point to the diagnostic values of qualitative cytological analysis even in cerebrospinal fluid specimens with a normal cell count. The importance of adapting the cytological method for routine laboratory procedure is stressed.

Di-, mono- and nonphytanyl triglycerides in the serum: a sensitive parameter of the phytanic acid accumulation in Refsum's disease.

The relative proportions of the diphytanyl, monophytanyl and nonphytanyl triglycerides ("triglyceride pattern"), the phytanic acid content of the triglycerides and the phytanic acid levels in the serum of 3 patients with Refsum's disease (heredopathia atactica polyneuritiformis, phytanic acid storage disease) were estimated by thin-layer chromatography, densitometry and gas chromatography, respectively. The individual triglyceride patterns were clearly dependent on the phytanic acid content of the triglycerides: the more phytanic acid in the triglycerides, the higher the percentage of the diphytanyl and the lower the percentage of the nonphytanyl triglycerides. The monophytanyl triglycerides were also related to the phytanic acid content of the triglycerides, although in a complex manner. The triglyceride pattern can be taken as a parameter of the phytanic acid accumulation in Refsum's disease to be at least as sensitive as the serum phytanic acid level in the biochemical evaluation of the efficiency of the dietary treatment.

[Clinical and biochemical follow up of Refsum's disease (author's transl)].

Within a family refered here, two sisters are presenting heredopathia atactica polyneuritiformis (Morbus Refsum) with the complete clinical, electromyographical and serological findings (increased phytanic acid level, accumulation of mono- and diphytanyl, triglycerides). In addition, some symptoms of Refsum's disease are clinically apparent in the eight year old daughter of one of the patients, but significant principal characteristics are absent (dissociation of the spinal fluid, retinopathia pigmentosa, increased phytanic acid level). The phytanic acid level in the serum of ten clinically normal blood relations (three of which are also obligatory heterozygotes) is normal. In both patients with manifest illness we found not only the usual spinal fluid changes with highly increased protein levels (gammaglobulin, IgA- and IgG-fraction) but also a remarkably large number of macrophages with vacuoles. Within almost two years of keeping to a strict diet with low phytanic acid and phytol content, both patients showed a distinct clinical improvement in parallel with a decrease of the phytanic acid level. Even a temporary increase of the phytanic acid level in one of the patients did not cause clinical relapse. Regarding the biochemical control of the course of the disease, the phytanic acid containing triglycerides proved to be a highly sensitive parameter.

[Autogenic training today (author's transl)].

Autogenic training (AT) creates an autonomic shift within the Hypnoid; in neurophysiological terms conceivable by a lower level of vigilance, synchronisation of EEG and modulation of afferent impulses. The said status differs clearly from wakefullness, drowsiness or sleep. AT however means more than just achievement of a hypnoidal state by ways of the classical six standard exercises, but to our opinion its use within a complex psychotherapy. We very much combine it with analytically orientated group therapy. Accordingly we oppose mass presciption of the standard exercises outside the medical sphere. Application of AT outside of psychotherapy within "psychohygienic indication" should carefully be considered only with strongly motivated persons, as otherwise the lack of motivation is likely to lead to failures.

[Improvement of zoster therapy by adamantine].

In the course of 3 years we observed a considerable improvement of herpes zoster in 44 patients being treated with amantadine. The periods of pain and efflorescence were shortened to 1/3 of the values usually experienced and painful post-zoster complications did not occur. The therapeutic effect depends on a) beginning treatment with high doses as early as possible, b) combination of local and systemic administration of adamantine, c) continuation of treatment for several weeks with gradually reducing doses. Harmless side-effects which are easily controlled are dryness of mouth, slight fall in blood pressure and insignificant general stimulation. In old people half the standard dose should be given in the beginning and particular attention paid to symptoms of restlessness on account of a possible delirium. Severe disorders of renal function are a contraindication.

[Epilepsies due to cerebrovascular disorders].

We stress the importance of differential diagnosis between late onset epilepsies and incidental fits in higher age. This is not merely a nomenclatory play but of great therapeutical consequence. Epileptic and non-epileptic attacks with onsets in higher age, either of generalized or of lateralized phenomenology, form an important precursor of impending stroke. "Vaskuläre Präkursivepilepsie", so called by us, within this group of premonitory signs is of special importance and may open way to adequat prophylactic therapy, medically or surgically (mainly in the sence of carotid surgery). Late onset epilepsy in itself can be treated with relative low dosis of antepileptics plus cardiotonic and vasoactive medication. Hence, the status epilepticus as accompanying symptom of a stroke is difficult to treat and very often forms a signum mali omniis leading to death by exhaustion of the predamaged brain.
