Investigations in the problem of diarrhoea in the Melut district, South Sudan (1981-1982).

Studies of diseases associated with diarrhoea in the Melut district of South Sudan in the years 1981-1982 showed amoebic dysentery and Rota-virus to be predominate, whereby according to our previous results Yersinia, Campylobacter, Shigella and Salmonella infections also play a role. In addition other parasitological bacterial and viral-infections are presumed.

Serological testing of human blood samples for infectious diseases in the Abeokuta and the Minna Hospitals/Nigeria.

In serological investigations undertaken in two hospitals in Nigeria a total of 188 blood samples were examined and the following positive reactions for various diseases found: malaria 100%, leishmaniasis 9.5%, biharziasis 2.1%, yersinia 16.4%, Legionella pn. 9%, gonorrhea 6%, syphilis 6.9%, measles 65.4%, rubella 84%, cytomegalic 78.2%, herpes simplex 67%, varicella 30.8%, Resp. sync. virus 34.6%, influenza A 57.4%, influenza B 73.9%, para-influenza 1, 2, 3, 20.7%, 16.5%, 52.6%, adenovirus 25%, Mycoplasma pneumoniae 33.5%.

Serological investigations in Nigeria for anthropozoonoses in human sera: brucellosis, echinococcosis, toxoplasmosis, chlamydial diseases, listeriosis, rickettsiosis (Coxiella burneti and Rickettsia conori).

176 blood sera taken from patients in the hospital of Minna and Abeokuta (Nigeria) were examined for anthropozoonoses. The following positive reactions could be found: Brucella abortus 9%, Brucella melitensis 11.7%, Echinococcosis 0.53%, Toxoplasmosis 79.2% (CF-Test 6.9%), Chlamydial diseases 45%, Listeriosis 28.7% (Typ 1H) and 19.7% (Typ 4bH), Rickettsiosis--Rickettsia conori 18.6% and Coxiella burneti 63.3%.

Serological investigations for the cause of diarrhoea among patients and personnel in the Minna and Abeokuta Hospitals/Nigeria.

188 blood samples were investigated for antibodies against bacteria causing diarrhoea in two hospitals in Minna and Abeokuta in Nigeria. Antibodies were found against Entamoeba histolytica, Yersinia enterocolitica, Campylobacter jejuni, Salmonella typhi, Salmonella paratyphi C, Shigella dysenteriae, Shigella sonnei, Shigella boydii, Shigella flexneri and Rotavirus.

[Gas permeable contact lenses in the management of problem cases and in prolonged wearing time].

The continuous improvement of contact lenses (copolymers with high gas permeability) makes better care possible in problem cases. They can also be used for continuous wear. The present authors report upon 104 cases in which a contact lens made of P3, was tested. This is a hard, flexible contact lens with a DK value of 33, which is produced without additional treatment of the surface.

[PMN elastase determination in tear fluid in corneal ulcer].

The PMN-Elastase concentration in tear fluid was determined in 87 persons without eye disease and compared with the PMN-elastase values of 28 patients with ulcus corneae. A significant increase in the PMN-elastase concentration was found in patients with ulcus corneae (means = 1196 micrograms/l) in comparison with the control group (means = 73 micrograms/l). The lapse control shows that the PMN-elastase values in the tear fluid of patients with ulcus corneae are elevated for a number of weeks, even though there are clinical signs of healing of the ulcer. The determination of PMN-elastase in tear fluid represents a new parameter indicating the real end of the inflammatory processes in the eye.

[Limbus-based flap versus fornix-based flap in goniotrepanation].

In a prospective, randomized study 61 eyes in which goniotrepanation had been performed using fornix-based or limbus-based conjunctival flaps were followed up for 4 to 18 months. No differences were found in the results of filtration, nor in the form and frequency of filtering blebs or the frequency of complications. In view of the easier technique and better visualization, the present authors prefer the fornix-based flap.

High frequency hearing following otitis media with effusion in childhood.

78 children had been treated for secretory otitis media at the age of 4.4, on the average. At the age of 18.6, their hearing was checked by means of conventional and high frequency (HF) audiometry. In the conventional range, the average hearing level was nearly normal and no air-bone gap could be found. In HF-range, the average hearing level decreased clearly but the HF-hearing level was significantly affected only in cases in which an air-bone gap exceeding 40 dB and more had existed during the active secretory otitis.

Influence of quinidine on the intestinal secretion of digoxin and digitoxin in guinea pigs.

The secretion of digoxin and digitoxin into in situ perfused jejunal and colonic segments of normal or quinidine treated guinea pigs was studied. Quinidine was administered intravenously by constant rate infusion resulting in a quinidine plasma concentration of about 6 micrograms/ml. After 2 h digoxin or digitoxin was injected i.v. (10 micrograms/kg). The quinidine treatment enhanced the plasma concentration of [3H]digoxin to about 140% as compared to controls, whereas the [3H]digitoxin concentration was not influenced by the quinidine infusion. Both, digoxin and digitoxin were secreted against a concentration gradient into the intestinal lumen. During the experimental period of 180 min controls secreted 0.24% of the administered digoxin dose per cm of jejunal and 0.13% per cm of colonic segment. Quinidine treatment resulted in a decrease of the jejunal digoxin secretion to about 80% of the control values. In both, jejunum and colon the concentration ratio between lumen and plasma (L/P) was diminished by quinidine to 50% as compared with the controls. The amount of [3H]digitoxin secreted into the intestinal segments was decreased by quinidine from 0.19% of the dose/cm to 0.13% in the jejunal and from 0.17% to 0.12% in the colonic segments, respectively. The decrease of the L/P ratio for [3H]digitoxin was more pronounced in the colon (58%) than in the jejunum (77% of the control values). As compared with controls the content of [3H]digoxin in the jejunal as well as colonic tissue was decreased by quinidine to 60% or 73%, respectively. On the other hand quinidine increased the tissue content of [3H]digitoxin in jejunum (+56%) and colon (+88%). In conclusion quinidine inhibits the intestinal secretion of both, digoxin and digitoxin, possibly by different mechanisms.

[Polyvinyl iodine in ophthalmic surgery].

The importance of minimizing the number of bacteria on the conjunctive before intraocular surgery has been undisputed for many years now. Bacteriological considerations led to antiseptic measures being preferred to antibiotics. Until recently there was no antiseptic agent available which was tolerated by both the cornea and the conjunctiva. The good results obtained in the treatment of bacterial and viral keratoconjunctivitis with PVP iodine led to this study of the preoperative preparation of 41 eyes prior to cataract extraction.
