[Relative discharge of tears by the upper and lower lacrimal ducts].

Forty eyes were studied in order to establish the relative function of the upper and lower lacrimal ducts. Following artificial occlusion first of the upper and then of the lower lacrimal ducts. Schirmer's No. 1 test was performed. The wetting of the test strips was used as an indirect measure of tear discharge. Higher quantities were discharged from the upper ducts of 17 eyes and from the lower ducts of 15. In 8 eyes the discharge from the upper and lower ducts was equal. The findings in this study thus disprove the commonly held belief that the upper lacrimal duct is plays a subordinate role in tear discharge.

[Unrecognized intrauterine toxoplasmosis despite screening].

A female small for date infant (BW 1500 g) was delivered after uncomplicated pregnancy in the 36th week of gestation. On routine screening for toxoplasmosis a negative SFT had been obtained in the 13th week of gestation. The second examination in the 32nd week was positive (SFT 1:16384). The mother was then put on specific chemotherapy (sulfametoxydiazine and pyrimethamine). The infant, however, had severe- and characteristic lesions (cerebral calcifications, chorioretinitis) not responding to therapy. Morphology of the lesions and serology led to the conclusion that the infant was infected between the 17th and the 24th week of gestation and that the disease remained undetected until the 32nd week, when treatment came already too late. This indicates that in case of a negative test further serological examinations should be carried out at closer intervals to establish the diagnosis in due time.

[Development of retinal vein thrombosis after argon laser photocoagulation (author's transl)].

In this paper 21 cases of branch vein thrombosis and 15 cases of central retinal vein thrombosis treated by argon laser photocoagulation are compared with an untreated control group. The visual prognosis after photocoagulation was no better in the treated group than in the control group. After central retinal vein occlusion, three cases of hemorrhagic glaucoma (20%) were observed in the control group but there was no rubeosis iridis or hemorrhagic glaucoma in the group treated by argon laser therapy.

[Partial canaliculus transplantation (author's transl)].

In some cases of palpebral tumors, causing epiphora by canalicular obstruction, surgical reconstruction of the lacrimal system is necessary . The defect of the nasal part of the lower lid is closed by transplantation of the nasal part of the upper lid of the other eye, including a part of the canaliculus. Three patients suffering from basaliomas in the region of the lower canaliculus were treated by this method. Good cosmetic and functional results were achieved.

[Rehabilitation after serious perforating eye injuries (author's transl)].

In a recent study of perforating eye injuries the authors found that in 21% there was no binocular function. To prevent complications arising from this, early fitting of hard contact lenses seems necessary. The problems of fitting contact lenses to posttraumatically aphakic eyes are discussed, with special regard to aniseikonia.

[Clinical experience in treatment of glaucoma with timolol (author's transl)].

Forty patients with primary and secondary glaucoma were treated with the beta-blocking agent Timolol for six months. We used a 0.25% or 0.5% solution twice a day. In several cases sufficient pressure regulation was achieved by means of Timolol monotherapy. If pilocarpine or acetazolamide therapy was not sufficient we used a combination therapy with Timolol.

[Observations of the course of branch and central venous occlusions (author's transl)].

Electro-ophthalmological responses determined with EOG, photopic and scotopic ERG, flash VEP's and pattern reversal VEP's, in combination with fluorescein angiography and computerized perimetry, were evaluated in seven patients suffering from branch vein occlusions and in eight patients suffering from occlusions of the central retinal vein. The findings were as follows: In cases of branch vein occlusion and central retinal venous occlusion with good prognosis there was a normal or slightly subnormal response in all electro-ophthalmological examinations. In the cases of central retinal venous occlusion with poor prognosis, likely to lead to destruction of the nervous structures of the retina, the response was considerably weaker as regards photopic and scotopic b-waves, and especially VECP's.

[An improved method for detection of diatoms (author's transl)].

An improved method for detection of diatoms in case of drowning is reported. The sample is digested with nitric acid. In order to avoid destruction of diatoms as well as losses by centrifugation etc. the time for wet digestion was reduced and the digest solution is filtered through a membrane filter. Fatty material is removed by alternate washing the filter with 2-Propanol and Petroleum ether. Following the wet digestion of the filter, aliqutos of this digest solution are filtered and the filter is examined microscopically. As the recommended procedure is less time consuming and yields almost complete recovery of diatoms the method has great probative value in case of death by drowning.

Prediction of drug distribution in vivo on the basis of in vitro binding data.

For 11 drugs it was investigated whether tissue distribution in vivo can be predicted by use of binding data obtained in vitro. The selection of drugs represented a broad spectrum of physicochemical and pharmacokinetic properties thought to be important for distribution of drugs in vivo. The extent of binding to plasma and to tissue-homogenates of rabbits was determined in vitro. The drug concentrations in plasma, liver, lungs, kidneys, and skeletal muscle of rabbits were determined in vivo after i.v. administration of the drug. The tissue-plasma partition ratios measured in vivo were compared with the theoretical tissue-plasma partition ratios calculated from the in vitro binding data. For all drugs investigated the muscle-plasma partition ratio could be reasonably well predicted by the in vitro binding data. In liver, lungs, and kidneys good agreement was found between measured and predicted tissue-plasma ratio for anionic drugs; marked differences, however, were observed between measured and predicted tissue-plasma ratios of lipophilic cationic drugs. A significant correlation was found between binding of drugs to muscle tissue in vitro and the volume of distribution of the unbound drug (Vf), opening the possibility to approximate Vf from in vitro binding studies with rabbit muscle tissue.
