[Comparative study of the therapeutic effect of antiangina agents].

The therapeutic effect of obsidan (25 to 250 mg/24 h), cordanum (75 to 300 mg/24 h), nitropenton (30 mg/24 h), combination of obsidan with nitropenton, prenylamin (180 mg/24 h), stenopril (180 mg/24 h), cordaron (600 mg/24h) and corvaton (4 mg/24 h) was studied according to clinical asessment in 127 patients with stenocardia and in 44 of them by loading tests (veloergometry or treadmill). According to the clinical assessment -- very good (abating of stenocardia paroxysms) and good effect (less frequent paroxysms nitroglycerin necessary every one-two days) was established in 82 per cent of those treated with obsidan, in 78 per cent of the treated with cordanum, in 86 per cent of those treated with the combination obsidan with nitropenton, in 44 per cent of the treated with nitropenton, in 42 per cent of the treated with prenylamin, in 46 per cent pf the treated with stenopril, in 7 out of 9 patients treated with cordaron as well as in 7 out of 9 patients treated with corvaton. According to the data from the loading tests--very good (increase of capacity, not limited by pain and ST depression) and good effect (considerable increase of capacity but limited by pain and ST depression) was found in 77 per cent of the patients administered obsidan, in 67 per cent of those administered cardanum and in 33 per cent of the patients administered tenopril and prenylamin. Beta--blockers significantly excel the therapeutic effect of stenopril, prenylanin and nitropenton, both according to clinical assessment (P < 0,02 to p < 0,001) and to the data from the loading tests (P < 0,02). Beta-blockers dosage was individually determined but 80 per cent of the patients with a favourable effect by obsidan or cordanum had received 60-120 mg/24 h and 150 mg/24 h respectively. The individual effect of the patients is presented so that even the small number of cases observed with cordaron and corvaton treatment shows it reasonable to include new preparations in antianginose treatment.

[Complex spatial evaluation of the electromotive force of the heart in persons with healthy hearts].

A complex spatial analysis of the electromotive cardiac force was performed to 150 subjects with intact hearts, aged from 20 to 55. The extend, direction and rate of the change in the direction and extend of 10, 20, 30, 40, 50, -30, -20, -10 and maximum spatial vectors were analysed. The results were processed according to the method of variation analysis, presented in diagrams and tables. The results obtained, which do not contradict those found in literature, show that the methods applied (graphic and automatic) could successfully be applied in practice and the data obtained--serve as standards for the work with the same apparatuses (Hellige EG-22 with an appliance, constructed and produced in Bulgaria.

[Clinical use of electrical atrial stimulation].

Electric auricle stimulation (EAS) was realized in a total of 77 patients. In 60 of the patients EAS was performed as a hypoxic test (step-wise increase of heart rate, with ECG registration and in many of the cases -- registration of certain indices of general and intracardial hemodynamics) with a view to precise diagnosis of ischemic heart disease and assessment of myocardial contraction function. In 91 per cent of the patients with stenosed coronary pathology on coronarogram, the symptoms of coronary insufficiency were positive (pain attack, ECG changes via ischemic type and/or deterioration of the myocardial functional curve) with the aid of EAS. Various correlations were established between the manifestations of coronary insufficiency and the deterioration of the functional curve of left ventricle. Furthermore, in the majority of the cases, one way results, among the data of veloergometry, coronarography and EAS, were recorded. In some cases EAS provided additional data for coronary insufficiency with negative veloergometric test (7 patients) and negative coronarography (5 patients). EAS could be applied in the diagnostics of ischemic heart disease, and in a hemodynamic aspect, via EAS a myocardial loading is realized with a minimum change of cardiac index and aortic pressure. In 16 patients EAS was applied with a view to the investigation of the functional state of sinal node and heart conduction system. In 13 patients, data about the syndrome of morbid sinal node were found (pathologic time of sinal node restoration) or for a latent lesion of heart conduction system. In the other three patients the suspicion of syndrome of morbid sinal node was not confirmed. The data obtained contributed to the determination of the therapeutic approach of those patients. In 5 patients a high frequency auricle electrostimulation was applied successfully in the interruption of paroxysms of supraventricular tachycardia. EAS was concluded, on the base of the data obtained, to be appropraite for application and precise diagnosis of ischemic heart disease, as well as in the studies on sinal node state and heart conduction system. In some rhythm disturbances, resistant to medicamentosus treatment, the electric auricle stimulation could prove a valuable therapeutic method.

[Planimetric analysis of ventricular depolarization on Frank's corrected orthogonal electrocardiogram in healthy hearts and in patients with ventricular loading].

Nineteen planimetric indices of 110 cardiac healthy subjects, 141 patient with left ventricle loading and 136 patients with right ventricle loading are analyzed. On the base of the variation analysis and determination of statistically significant differences, it was established that in right-ventricular loading the following indices deviate from the norm: ASX, AQZ, AQRSX, AQRSz, SAQRSx, SAQRSy, SAQRSz, SAS, SAQRSg, whereas in left-ventricular loading -- ARx, ARz, AQRSx, AQRSz, SAQRx, SAQRSy, SAQRz, SAR, SAQRSg. At a second stage, the sensitivity of the separate indices from the groups with left ventricular and right-ventricular loading was amalyzed, as well as the separate subgroups (pulmonary stenosis, aortic stenosis, mitral stenosis, interauricular defect, arterial hypertension, mitral or aortic insufficiency. The results were compared with those of axial indices, obtained from another investigation of the authors. The planimetric analysis was established to be more complex than the axial and the index SAQRSg to be with the best sensitivity in the cases with hemodynamically lightly loaded musculature.

[Significance of sinus rhythm for ventricular function].

The changes in hemodynamics in conditions of dosed loading prior to and post cardioversion were followed up in 21 patients with valvular defects or atherosclerotic myocardiosclerosis and absolute arrhythmia with auricular fibrillation. In conditions of auricular fibrillation all the patients showed data about a deteriorated ventricular function (increased arterial-venous difference--tachycardia), both at rest and at dosed loading. One week after the sinus rhythm restoration, a considerable improvement of ventricular function was observed, at rest and loading (normal arterial-venous difference, higner minute and beat volume), suggesting the favourable effect of the restored sinus activity. The improved ventricular function is due to the restored auricular contraction, together with the restoration of rhythmic cardiac activity.

[Electrophysiological methods of assessing myocardial hypoxia].

The potentialitis of the standard electrocardiogram, precordial cartogram and spatial analysis are confronted in 38 patients with extensive anterior or anteroseptal myocardial infarction, acute phase, in order to determine the extent and propagation of the myocardial hypoxia. The determination is performed on the base of the correlation dependence as regards the creatinine-phosphokinase level in blood. The best degree of correlation dependence has the spatial analysis of the changes in the segment ST - Vmax ST sp. r=0.69, as well as the changes in the segment ST reflected in the precordial electrocardiography - nST-0.1 mv. r=0.67. Close is the degree of correlation dependence in the standard electrocardiogram - STv r=0.59. The difference in the correlation coefficients are not statistically significant, allowing their application in the routine work of the three methods.

[Comparison of precordial electrocartography with pathologicoanatomic data on 20 patients with recent myocardial infarct].

In 20 patients with recent myocardial infarction, that died till the 40th day after the onset of the disease, a comparison is made of the data from the pathologoanatomical findings with the indices from the precordial electromapping. The changes in the ST segment of the precordial electromapping correlate, to a significant extent, to the weight of the infarction zone in patients with interior site of the myocardial infarction (sigmaST r = 0.89, p less than or equal to 0.01; nST r = 0.62, p less than or equal to 0.01). The indices from the precordial electromapping characterizing the changes in depolarization, are with a considerable less degree of correlation dependence to the weight of the enfarcted zone in the same patients (nQS r = 0.62, p less than or equal to 0.62, p less than or equal to 0.05; sigmaAR r/0.45, p less than or equal to 0.05). In patients with posterior and/or inferior site of the infarction the correlation dependence to the indices from the precordial electromapping is less manifested.

[Precordial electrocartography (mapping) in cardiac infarct].

After a detailed literature survey the authors share the results from the precordial electromapping with the registration of 30 points upon the chest in 34 patients with ischemic heart disease (30 with transmural infarction and 4 with intermediary syndrome). The following indices were followed up: sigmaST (the sum of the elevation of ST segment measured at 60 msec of the S deflection recording) and the index nST (the sum of the points with an elevation greater than 0.1 mV/1 mm). Both indices well correlate to the level of blood creatine phosphokinase, determined 24 hours after the examination. Due to the close degree of correlation dependence, the two indices are interchangable for the practice. The precordial electromapping reflects the degree of extent of the "myocardial hypoxia" as well as the degree of the disturbed biological activity of the tissues, affected by the myocardial infarction.

[Diagnostic value of Frank's corrected orthogonal electrocardiogram].

The changes in the corrected orthogonal electrocardiograms of 411 patients were analyzed: 134 with right ventricle loading, 137 with left ventricle loading and 140 with myocardial infarction (chronic phase). On the base of sensitivity and specificity criteria the indices SXmV, SXmsec and QZmV for the patients with right ventricle loading and RXmV and RZmV for the patients with left ventricle loading proved to be with the highest diagnostic value. Sensitivity is within 19% to 61%. Sensitivity is within 85% and 95% in patients with myocardial infarction (chronic phase), for the different indices depending on the infarction zone. The comparative analyses of the indices of the corrected orthagonal Frank electrocardiogram and the standard electrocardiogram (12 leads) give a certain preference to the indices of the standard electrocardiography.

[Spatial analysis of the electrophysiological changes in ventricular loading].

Space analysis methods of the changes in heart electromotive force are described (EMF). The size of the space vector in millivolts, azimuth and elevation are obtained by formula substitution in the mathematical analysis. The same values, in graphis analysis, are obtained by the "Circle for space vectors determination", proposed by the authors. Axial and space analysis of the changes in the electrogenesis of right ventricle was performed in 27 patients with pulmonary stenosis, confirmed by catheterization, on the base of the corrective orthogonal electrocardiogram according to Frank. The axial indices, indicating the size of the forces directed to the right (Sxmv) and forward (Qzmv) are compared as well as the size, elevation and azimuth of the maximal space vector, directed to the right (Vmax.sp-x) and forward (Vmax.sp-z). The index from the axial analysis (sensitivity 62%, r = 0.65) shows a priority for the forces, directed to the right. The priority is on the side of the indices from space analysis (sensitivity 33%, r = 0.78) in case of forces directed forward. There is no statistically significant difference in the diagnostic value of the two methods.
