Two patients with the benign type of follicular mucinosis (FM) are presented. Their clinical features and course were characteristic for this subgroup of FM. Light and electron microscopy, direct immunofluorescence, and immunoperoxidase cell marker studies were undertaken to characterize the nature of the disease process. Light microscopy confirmed the follicular outer root sheath and sebaceous gland epithelial degenerative changes. The infiltrating inflammatory cells were morphologically benign. Electron microscopy detailed the cellular associations in the areas of degenerative change. Disattached keratinocytes were closely apposed to significant numbers of macrophages and Langerhans cells. Direct immunofluorescence studies demonstrated primarily complement (C3) and fibrinogen/fibrin in areas of reticular degeneration. Immunoperoxidase studies revealed large numbers of T cells and macrophages and a striking increase in the number of Langerhans cells in the affected follicular epithelium. The findings suggest that cell-mediated immune mechanisms may play a role in the pathogenesis of this disorder.
We report on a patient with primary cutaneous malignant melanoma, with numerous intransit metastases involving the left lower extremity. The clinical presentation was striking in that the intransit metastatic lesions of the skin had morphologic characteristics of Kaposi's sarcoma. Her disease was complicated by marked lower limb edema. Cutaneous ulceration did not develop despite the extensive tumor burden. Treatment with regional limb perfusion resulted in marked reduction of the edema and shrinkage of the tumor mass. In addition to presenting this case report and response to therapy, we would like to suggest the term "chronic melanoma of the extremity" (CME) as a descriptive term for this unusual variant of malignant melanoma.
A boy with agminated spindle cell nevi is described. Present within the area of involvement were congenital nevocellular and composite spindle/nevocellular nevi. Other unusual features included a dynamic pattern of growth and regression of the lesions, with the presence of halo nevi and background café au lait pigmentation. This unique presentation of multiple nevi is discussed in the context of previous reports of agminated spindle cell nevi.
Paget's disease of the breast is a malignant lesion consisting of Paget cells in the epidermis and an underlying ductal carcinoma of the breast tissue. It is estimated that perhaps 3% of female breast carcinoma cases are Paget's disease. If one estimates male breast carcinoma to be approximately 1% of the rate in the female population, then Paget's disease in men is clearly a rare clinical event. Although Paget's disease has an epidermal component, little attention has been given to this entity in the dermatologic literature. We present a case report of this disease in an elderly male patient and comment on the clinical features which should alert the physician to the presence of the malignancy.

A two-month visit to the Chaim Sheba Medical Center, Tel-Hashomer, Israel, provided an opportunity to investigate some aspects of that country's health care system. Observations on the Israeli health care system and on the populations served, with highlights on cultural, demographic, political, and historic factors, are essential to an understanding of health care in Israel. A detailed description of the organization and administration of services at the Chaim Sheba Medical Center may aid in this understanding. It is essential to our understanding of this health care system to realize that Israel is not a purely Western nation. To gain insight into the system, we must consider the nation's political and economic situation. Such an awareness is crucial to the study of any foreign health care system.
Protochlorophyll (Pchl) and protochlorophyllide (Pchlide) are at comparable levels in 2-day-old (young) etiolated bean leaves (Phaseolus vulgaris L. var. Red Kidney). During subsequent development in the dark, both pigments increase, but the rate of Pchlide increase is greater than that of Pchl, leading to the commonly observed predominance of Pchlide beyond 7 days (old leaves). Both protopigments are phototransformable to their respective chlorophyll(ide) photoproducts throughout dark development. The rate of protopigment regeneration in young leaves after illumination is rapid and displays no lag, whereas this process in old leaves begins slowly and achieves only about one-fifth the rate of younger leaves. The rate of chlorophyllide esterification is also faster in the younger tissue. Since the proplastid-related properties of young bean leaves are quite similar to those of Euglena, young leaves and Euglena may represent an evolutionarily primitive case compared with older bean leaves which contain etioplasts. Since Euglena and young beans green perfectly well when exposed to light, the extensive modifications associated with prolonged dark growth do not seem to be obligatory for plastid development. The properties of older beans are viewed as being the consequence of prolonged etiolation which may provide a faster rate of plastid development and appearance of photosynthesis as the plant nears the limits of its stored reserves.
