[Surgical indications for scintigraphically cold thyroid nodules in relation to clinical, scintigraphic, sonographic and cytologic findings].

The frequency of indication for surgical resection of goitre was retrospectively analysed for 2100 thyroid examinations. During 1974/75 (group I) only scintigraphy was available in addition to clinical examination; in 1979/80 (group II) cytological criteria were added, and in 1983/84 (group III) ultrasound findings were available as well. There were 700 patients in each group. The proportion of scintigraphically negative nodules was largely the same in the three groups (I and II: 21%; III: 22%). Indication for operation was less common after the introduction of ultrasound (I: 56%; II: 55%; III: 44% of patients with scintigraphically cold nodules). In particular, there was a decrease in the proportion of those nodules not definitely considered benign which would have required histological diagnosis (I: 32%; II: 18%; III: 8%). The results underline the special place of small-needle biopsy and, especially, ultrasound examination of the thyroid, in addition to clinical and scintigraphic findings, in providing the indication for and planning of operative nodular goitre resection.

[Specimen carriers for blood sugar determinations].

The use of boric acid-impregnated filter paper has proven to be a useful and reliable means of absorbing capillary blood samples for determining glucose concentration. In this method a circular piece of filter paper (6.36 mm radius) infiltrated with the blood sample is punched out with a commercially available punch, glucose is eluted from the filter paper with a haemolyzing solution and subsequently glucose concentration determined by whatever method is available. Using the hexokinase method, this gives satisfactory precision and unimpaired accuracy. It compared well with the frequently used sampling with capillary tubes, correlating with an r value of 0.9932. The sample, obtained as described, is insensitive to light, temperature, humidity and time interval until measurement. Glucose concentration remains stable under various conditions for at least four weeks.

[Classification of fractures of the proximal end of the humerus following prognostic and therapeutic aspects].

The classification of the proximal humeral fractures according to the four-segment concept of Neer has proven to be very useful. It not only considers the fracture itself but also lesions of the soft tissue. Adapting Neer's concept we propose a modified classification of the proximal humeral fractures which we believe is clearer and more practicable. This classification is illustrated by means of drawings and typical roentgenograms. The description of the different fracture-types includes the mean characteristics such as fragment-displacement, status of the rotator cuff and blood circulation to the head segment.

Cytological findings in medullary carcinoma of the thyroid.

In six cases of medullary carcinoma of the thyroid rather characteristic findings were seen in cytological specimens obtained by aspiration or imprint. The roundish-polygonal or spindle-shaped cells were predominantly disseminated, but occasionally arranged in small groups. The nuclei were frequently found in an excentric position; binuclear cells were sometimes encountered. In addition, five of the six cases showed intranuclear cytoplasmic inclusions which were also investigated by electron microscopy in specimens of two cases. Such inclusions in aspiration biopsies of the thyroid have so far been regarded, as characteristic for papillary carcinoma. In smears as well as in histological sections amyloid and calcitonin could be demonstrated by congo red staining and by immunocytochemistry, respectively. The cytomorphology of the tumor cells, their pattern of arrangement, and the peculiar nuclear inclusions make up a combination of findings that may justify the diagnosis of medullary carcinoma in aspiration biopsies. In smears the diagnosis can already be confirmed when (immuno-) cytochemical methods are applied to smear preparations.

[Differential diagnosis of thyroid nodules. Sonography as a complement to scintigraphy and puncture cytology].

The place of ultrasound in the diagnosis of thyroid nodules was assessed in 384 patients by comparing its findings with those obtained on palpation, scintigraphy, fine-needle cytology and (in 45 cases) histology. Further diagnosis of scintigraphically "cold" nodules is the main indication for ultrasound examination in goitre-endemic areas. In 13 of 69 cases of a cold nodule with complex or echo-poor echo image proved it to be malignant, while in 74 with cold nodules but with a normal or increased echo pattern there was no malignant tumour. Thus, if the echo pattern is "complex" or "echo-poor" urgency of surgical intervention is strengthened, while with echo-normal or echo-rich nodules or cysts indications for operation are relative. In future, a combination of scintigraphy, ultrasound and cytology (ultrasound-guided small-needle biopsy) will give the highest rate of diagnostic accuracy in the differentiation of malignant from benign thyroid lesions.

Time courses of antibody levels in Mastomys natalensis after infections with Litomosoides carinii, Dipetalonema viteae, Brugia malayi or B. pahangi, Determined by ELISA.

Using a Litomosoides carinii adult antigen, time courses of antibody levels were followed by an ELISA in L. carinii, Dipetalonema viteae, Brugia malayi and B. pahangi infected Mastomys natalensis. Using various groups of infected animals, periods up to 400 days after infection were covered. In L. carinii infected Mastomys, antibodies were first detected 11 days p.i. and levels increased rapidly until day 40. Temporarily reduced levels about the beginning of patency were followed by increasing values until about 100 days p.i. Then the antibody content of the sera remained more or less constant until about 250 days p.i. although maximum levels were found at day 170. Thereafter, the antibody concentration in the sera declined slowly but high levels were still observed 390 days p.i. The antibody content was usually higher in animals with high microfilariae densities than in those with low microfilariae counts but relations could not be proven statistically. In D. viteae infected Mastomys, maximum antibody values were reached within the beginning of patency. Levels were not altered markedly until about 110 days p.i. Thereafter they decreased slightly but then remained constant until the end of the investigation period 350 days p.i. B. malayi infected animals showed a rapid increase of the antibody content in the sera; a maximum was reached by 20 days after the infection. Thereafter, somewhat constant levels were found for 4--5 months. After 300 days p.i. the antibody levels declined progressively, accompanied with increasing parasitaemia densities; after 380 days the levels reached about 2/3 of the maximum. However, despite this, no relation was found between the levels of parasitaemia and antibody in individual animals. In B. pahangi infections the main prepatent antibody increase occurred during week 5 p.i., when maximum values were observed. The beginning of patency and the early patency were accompanied with slightly declining antibody levels. From 150 days p.i. until the end of the investigation 400 days p.i., the antibody content of the sera was fairly constant.

[Fractures of the head of the tibia - an indication for CT?].

Experimental and clinical investigations of fractures of the head of the tibia by computed tomography are described. CT provides much information which is of significance for treatment. In particular, it demonstrates the size of fragments, of defects and areas of compression more clearly than does conventional radiography.

[Diagnosis and therapy of fibular ligament injuries in the ankle joint in children].

Turn fibular ligaments of the ankle go often undetected when not accompanied by osseous lesions. The significance of injuries to the major supporting ligaments of the ankle is underestimated in children. Next to the clinical findings stress radiographs of the ankle joint in two projections are mandatory. Measurements of anterior and lateral displacement under stress and displacement index need to be compared with the healthy ankle. Diagnosis of a fibular ligament rupture is based on clinical findings and on differences between the measurement results (injured compared to healthy) by way of three different techniques. Rupture of a fibular ligament is an absolute indication for surgery--even in children.

[A new combined local transferable flap technic for closure of pretibial soft tissue defects].

The cross-leg flap is a well established and recommendable procedure although it has disadvantages, like fixation in uncomfortable position, three or four weeks of bed rest and injury to the donor leg. With diseased or missing donor leg the procedure is impossible. This also is the case if the patient refuses the operation because of the uncomfortable fixation. As alternative procedure for these cases we developed a combined local flap procedure. It consists of a subfascial mobilized dorsolateral rotated flap and a subfascial mobilized dorsomedial doublebased flap; both are adapted without tension over the defect. The procedure proved to be efficient. Posttraumatic soft tissue defects with exposed osteosynthetic material were closed, cases of long standing osteitis with skin ulcerations were treated successfully after en-bloc-resection of the infected skin and bone region. The advantages of this procedure: early mobilization of the patients, shortening of treatment, non-affection of the opposite leg, non-irritated, resistant, well-cushioned and innervated soft tissue coverage as well as cure of the osseous infection.
