[Hemoperfusion therapy in severe suicidal digoxin poisoning].

Severe digoxin intoxications have a mortality rate of about 20%. Clinical course of intoxication, electrocardiographic changes as well as the behaviour of the plasma-digoxin concentration could closely be controlled on 4 patients after suicidal ingestion of potentially lethal dilanacin -new doses. In 3 patients a haemoperfusion treatment lasting 4 to 6 hours was performed. Clinical symptoms and existing disturbances of cardiac rhythm were clearly positively influenced under perfusion. The earlier the treatment was begun the more favourably appear the effects on the course of intoxication. After an at first benign picture of intoxication in the non-perfused female patient a comparatively distinct prolonged clinical and electrocardiographic symptomatology developed. Though the digoxin extraction established by computation fairly slightly imposes, this eliminated free digoxin fraction seems, nevertheless, to possess a considerable pharmacological and clinical importance. The adsorber material used showed well reproducible high clearance performances. During the haemoperfusion treatment in no case clinical complications were observed.

[Discontinuous plasma exchange with the conventional hemodialysis technic and blood cell centrifugation].

A conventional haemodialysis system, to which the patient is connected as for acute dialysis, forms the basis of the plasma substitution method. At each session around 2 litres of plasma were exchanges within 3 hours. Freshly frozen donor plasma was mainly used as the substitution solution. A total of 20 substitutions were carried out in 5 patients in a period of 7 months. The indications were: 3 x major mismatch in the ABO system before bone marrow transplantation, myasthenic crisis, violent attacks connected with lupus erythematosus with rapid development of renal insufficiency. The therapeutic goal of quickly eliminating immunopathogenetic proteins was achieved in all cases. It was possible to reduce the level of isoagglutinins, haemolysins, autoantibodies (myasthenia gravis) and immunity complexes (LE) effectively. The clinical course confirmed the excellent effectivity. There were no serious complications despite the serious primary diseases. Volume imbalances, changes of body temperature, precipitations of cryoglobulins, electrolyte disturbances or citrate intoxications were always well controllable thanks to the combination of plasma substitution and dialysis.

Tetronic acid derivatives from Aspergillus panamensis. Production, isolation, characterization and biological activity.

When Aspergillus panamensis, CBS 120.45, was grown on a medium with high carbon and low nitrogen content, it was found to produce six antibiotically active metabolites. All compounds were identified as tetronic acid derivatives; three of them were found to be identical with gregatins A, B, and D, previously reported as metabolites of Cephalosporium gregatum. The other three compounds are new. All six compounds show antibacterial activity towards Gram-negative and Gram-positive bacteria. Five of the antibiotics exhibit inhibitory effects on the macromolecular syntheses in cells of the ascitic form of EHRLICH carcinoma (ECA) of mice.

Investigations of the pigments from Cytophaga johnsonae Cy jl. New flexirubin-type pigments.

Besides carotenoids a complex of flexirubin-type pigments was isolated from the gliding bacterium Cytophaga johnsonae Cy jl and separated into 6 components, which partly containe chlorine. In spite of the fact that these components still consist of pigment mixtures, the gross structures of 18 new flexirubin-type pigments could be deduced by spectroscopic and chemical investigations. The results open insights into biosynthesis and structural variety of the flexirubins, the novel non-isoprenoid pigments recently found in Flexibacter elegans.

Metabolic products of microorganisms 167. Cyclopaldic acid from Aspergillus duricaulis. 1. Production, isolation and bioloical properties.

In the course of a screening for new metabolites from fungi we isolated a substance with antimicrobial activity from cultures of Aspergillus duricaulis (CBS 481.65) (Tü 679). It was antagonized by putrescine, spermidine, spermine, arginine, citrulline, lysine, ornithine, in higher concentration by aspraagine and glutamine too. The effect of ethylenediaminetetraacetate on the susceptibility of Streptomyces viridochromogenes (Tü 57) and Bacillus subtilis ATCC 6051 to this antibiotic has been studied. The substance was characterized and identified as cyclopaldic acid.

[Anemia in chronic hemodialysis].

On the basis of general haematological, clinicochemical, cytomorphological, cytochemical and nuclearmedical investigations the anaemia of chronic haemodialysis can be considered as a renal anaemia modified by a permanent dialysis. It is less characterized by lack of erythropoietin, but more by a strong lack of iron, moderate haemolysis and slight deficiency of folic acid. After gradual diagnostics the substitution treatment with iron is absolutely necessary, medication of folic acid should be attempted. A permanent therapy with 80-120 mg of iron weekly administered parenterally can be successfully performed and 10-15 mg of folic acid daily, which will markedly lower the rate of transfusion.

[Mitomycin and a new phenoxazone pigment from Streptomyces michiganensis. Metabolic products of microorganisms (author's transl)].

Streptomyces michiganensis strain Tü 1074, was isolated from a Tunesian soil sample and produces in liquid medium an antibiotic active pigment complex. Besides mitomycin A the separation of this complex yielded a nonactive phenoxazone, which hitherto has not been described in the literature. In contrary to all known phenoxazones from micro-organisms the new compound lacks a 2-amino-function. The production of this phenoxazone could be enhanced by optimizing the condtions of fermentation.
