Esophagectomy for metastatic carcinoma of the esophagus from lung cancer.

A patient with metastatic carcinoma of the esophagus from lung cancer is reported. The patient was a 54-year-old woman who underwent a left lower lobectomy for lung cancer 5 years previously. The authors performed a thoracic esophagectomy, dissection of mediastinal lymph nodes, and reconstruction of the esophagus; the surgery was followed by chemotherapy. Because the histologic pattern of the esophageal tumor was similar to that of lung cancer and mucosal involvement was not seen, the esophageal tumor was interpreted to be a metastasis from lung cancer. The patient is well without recurrence of disease 23 months after operation. This is the first report of a successful esophagectomy for metastatic carcinoma of the esophagus from lung cancer.

The RNA content of alveolar macrophages in canine lung allografts.

To evaluate the utility of RNA synthesis by alveolar macrophages (AM) in lung allografts as a marker of acute rejection, we performed canine left lung allotransplantation and measured the AM RNA content during immunosuppressive therapy with azathioprine (AZ), cyclosporine A (CsA), or CsA combined with AZ and prednisolone (Pr). We determined the RNA content by determining the RNA index (RI) (peak channel number of AM/peak RNA channel number of peripheral blood lymphocytes) by flow cytometry. The dogs were classified into one of five treatment groups: Group 1 (n = 18), control (no surgery) dogs; Group 2 (n = 11), allotransplanted dogs without pre- or postoperative immunosuppressive therapy; Group 3 (n = 14), received AZ at dose of 5 mg/kg/day orally; Group 4 (n = 17), received CsA at dose of 20 mg/kg/day orally; Group 5 (n = 15), CsA combined with AZ (5 mg/kg/day) and Pr (0.5 mg/kg/day). The RI of normal lungs was 2.14 +/- 0.20 (mean +/- SD) (n = 18). The RI of rejected lungs was significantly higher than the RI of normal lungs in Group 2 (2.51 +/- 0.21, n = 8, P < 0.05) or Group 3 (2.76 +/- 0.36, n = 7, P < 0.05), but did not differ significantly from that in Group 4 (2.19 +/- 0.20, n = 7) or Group 5 (2.05 +/- 0.45, n = 8). The RI of rejected lungs was higher than the RI of nonrejected lungs in Group 2 (vs 2.18 +/- 0.12, n = 3, P < 0.05) or in Group 3 (vs 2.18 +/- 0.22, n = 7, P < 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

Surgical experience with 5 n3(+) early gastric carcinoma cases.

Of 370 patients who underwent a resection for early gastric carcinoma, 317 (85.6%) were N(-), 28 n1(+), 20 n2(+) and five (1.4%) n3 respectively. In this study, n3(+) early gastric cancers were clinicopathologically evaluated and sites of the metastatic nodes were also defined. In addition, a comparative study in DNA ploidy patterns between primary and metastatic node cell was performed according to Hedley's method using a FACS-IV scan of paraffin-embedded tissues. All n3(+) early gastric cancers showed an aneuploidy pattern. R2 or R3 operations are recommended for early gastric carcinomas with aneuploidy pattern and most of the metastatic nodes arise from the identical stem cell line of cancer cells.

Combined transection of the trachea and esophagus following cervical blunt trauma.

The successful management of a patient with combined transection of the cervical trachea and esophagus following blunt trauma is reported. This type of injury was rare in the past, but is gradually increasing at the present time because of the increased use of motorcycles. Immediate primary closure of the transected trachea and esophagus offers the best chance for a good result.

[Effect of FK-506 on bronchial healing of canine lung allografts].

The effects of the immunosuppressive agents on the healing of the bronchial anastomosis following canine lung allo-transplantation were evaluated. Auto- or, allo-transplantation was performed in 44 dogs. The dogs were classified into following three groups: Group A (18 dogs) underwent autograft, Group B (13 dogs) underwent allograft following immunosuppressive therapy with cyclosporin A (20 mg/kg/day) orally and Group C (13 dogs) underwent allograft with FK-506 (0.1 mg/kg/day) injection intramuscularly and omentopexy on the bronchial anastomosis. The dogs were sacrificed at 1, 2, 3 or 4 weeks after surgery. The grafted lung and anastomosed bronchi were removed, and microscopic examination was performed. We estimated bronchial healing according to the inflammatory response score (IRS), which was defined as comprehensive assessment by histologic findings in each of the inflammatory response: inflammatory cell type, a degree of the proliferation of collagen fibers, edema, bleeding, degeneration and ulcer. There was no significant difference between the incidences of rejection and the inflammatory cell types. The degree of the proliferation of collagen fibers in Group C was moderate at 2 weeks and remarkable at 3 or 4 weeks. IRS at one week after operation was 9.1 +/- 1.5 in Group A, 8 in Group B and 8 in Group C. IRS at two weeks after surgery was 4.8 +/- 1.3 in Group A, 5.1 +/- 1.2 in Group B, 4.3 +/- 1.0 in Group C. IRS at three or four weeks after transplantation was 3.0 in Group A, 3.2 +/- 1.9 in Group B and 2.0 +/- 1.2 in Group C. There was no significant difference between each group and each week.(ABSTRACT TRUNCATED AT 250 WORDS)

[Lung preserving operation for bronchogenic carcinoma--segmentectomy and bronchoplastic lobectomy].

The results of segmentectomy and bronchoplastic lobectomy for bronchogenic carcinoma were reported. Until December 1992, 920 patients underwent lung resection for primary lung cancer at our institution. Among them, 70 patients (7.6%) had segmentectomy and 149 (16.2%) had bronchoplastic lobectomy. In cases with segmentectomy, adenocarcinoma (57.1%) and Stage I disease (77.1%) were dominant, and in cases with bronchoplasty, m squamous cell carcinoma (69.8%) and Stage IIIA (45.0%) were frequent. There were no operative deaths and one hospital death (1.4%) in patients with segmentectomy, and 8 operative deaths (5.4%) and 9 hospital deaths (6.0%) in those with bronchoplasty. Five-year survival rates of the patients with segmentectomy were 49.6% in total and 64.6% in those with Stage I disease. Five-year survival rates of those with bronchoplasty were 33.9% in total, 51.1% in those with Stage I, 29.1% in those with Stage IIIA and 39.0% in those with squamous cell carcinoma. Respiratory function was well preserved in patients with segmentectomy and bronchoplastic lobectomy. Lung-preserving operations such as segmentectomy and bronchoplastic lobectomy are meaningful as a curative procedure for primary lung cancer for selected patients.

[Comparison of typical and atypical bronchoplasty for bronchogenic carcinoma].

One-hundred and twenty-eight patients who underwent bronchoplasty for bronchogenic carcinoma from 1969 to 1990 were retrospectively analyzed. Ninety-six patients had upper sleeve lobectomy or upper lobectomy with wedge bronchoplasty (typical procedure) and 32 underwent bronchoplasty with lobectomy other than upper lobes (atypical bronchoplasty). The two groups were statistically similar in preoperative characteristics including sex, age, cell types and stages. In typical group, right upper sleeve lobectomy was performed in 48 patients, left upper sleeve lobectomy in 27, right upper lobectomy with wedge bronchoplasty in 11, left upper lobectomy with wedge bronchoplasty in 7 and sleeve bronchoplasty alone in 2. In atypical group, right middle and lower lobectomy with sleeve bronchoplasty in 10 patients, left lower sleeve lobectomy in 5, right lower lobectomy with wedge bronchoplasty in 4 and others in 13. Postoperative mortality was 5.2% in the typical group and 9.4% in the atypical group. Postoperative respiratory complications occurred in 16 patients (50%) in the patients with the atypical group and in 33 (34.7%) in those with typical group. Three years and 5 years survival rates in the typical group were 46 and 40% and those in the atypical group, 27 and 18%. However, there was no statistical difference in survival between the two groups. In conclusion, atypical bronchoplasty is useful for preserving the lung parenchyma, but, careful perioperative management and surgical technique of bronchoplasty are mandatory.

Upper airway obstruction from a benign goiter.

Acute upper tracheal obstruction from a huge benign goiter developed in an 80-year-old woman. Computed tomography showed severe stenosis of the cervical trachea by extrinsic compression. A left hemithyroidectomy was performed and the tracheal lumen returned to normal size. However, the tracheal wall was malacic due to the prolonged compression by the goiter and a silastic T-tube insertion was necessary for a two month period. This type of tracheal obstruction is rare. The importance of evaluating the upper trachea in patients with prolonged benign goiter is stressed and the diagnosis as well as the management of this type of complication are discussed.

Twenty-four-hour canine lung preservation using UW solution.

The left lungs of 14 mongrel dogs were isolated, preserved, and then reperfused for 120 min. Three groups of lungs were investigated: group 1, nonpreserved lungs (control n = 5); group 2, lungs were flushed with UW solution and cold-stored (4-6 degrees C) in the same flush solution for 24 hr (n = 4); and group 3, lungs flushed and cold-stored with modified Euro-Collins' solution for 24 hr (n = 5). Airway pressure (AWP), static lung compliance (Cst), and pulmonary vascular resistance (PVR) 120 min after reperfusion were significantly higher in group 3 compared with the lungs in group 1 and group 2. AWP was 18.7 +/- 3.9 in group 1, 21.1 +/- 3.8 in group 2, and 33.8 +/- 9.2 ml/cmH2O (mean +/- SD) in group 3 (P less than 0.05). Cst was 14.0 +/- 3.5, 10.8 +/- 1.5, and 6.2 +/- 1.2 ml/cmH2O, respectively (P less than 0.01). Pulmonary vascular resistance was 125 +/- 16, 120 +/- 42, and 410 +/- 108 mmHg/L/min (P less than 0.05). We conclude that UW solution is useful for hypothermic canine lung preservation.

Immunohistochemical expression of blood group substances and related carbohydrate antigens in breast carcinoma.

In forty-one carcinomas and sixteen benign lesions (fibroadenoma and mastopathy) of the human breast, immunohistochemical expression of sialylated and non-sialylated forms of both Lea and Lex, and the A, B, and H type 2 blood group substances were studied by using an indirect immunoperoxidase staining. In normal ductal epithelium and benign lesion of breast, Lewis-related antigens were mostly expressed. Breast carcinomas showed these antigens with the following frequencies: Lea, 31.7% (13/41); sialyl Lea, 56.1% (23/41); Lex, 46.3% (19/41); sialyl Lex, 68.3% (28/41); A/B/H type 2, 38.1% (16/41). Sialylated forms of Lea and Lex were observed more frequently than their respective non-sialylated forms in breast carcinomas. In both one normal epithelium and four carcinomas of breast with Le(a-b-) phenotype, the expressions of type 2 antigens were observed, while type 1 antigens were not consistently expressed. Although compatible expression was observed in all specimens of both normal epithelium and benign lesion of breast, twenty-four cases with the deletion of A and/or B antigens, six cases with H type 2 accumulation and one case with incompatible expression were demonstrated in breast carcinoma. Thirty-one breast carcinomas which showed the deletion of A/B/H type 2 expressed the Lewis-related antigens more frequently than nine cases which showed compatible expression. These results suggested that the activation of terminal fucosyltransferase and sialyltransferase as well as inactivation of some glycosyltransferases had occurred in cancer cell membrane, and sialyl Lex, defined by a new monoclonal antibody CSLEX1, may be useful as a tumor-associated antigen independent of Lewis blood group type in breast cancer.

[An analysis of 70 patients with bilobectomy for bronchogenic carcinoma].

From 1955 to April 1989, 70 patients underwent bilobectomy for the treatment of primary lung carcinoma. Thirteen patients (18.6%) underwent right upper and middle lobectomy (UML), while 57 patients (81.4%) underwent right middle and lower lobectomy (MML). Indications for bilobectomy were cancer invasion into intermediate bronchus (34%), tumor extending to neighbouring lobe across a fissure (29%), interlobar lymph nodes metastasis with or without invasion to intermediate bronchus (24%), vascular invasion (5%), and others (7%). Squamous cell carcinoma was present in 31 patients, adenocarcinoma in 28, large cell carcinoma in 5, small cell carcinoma in 4 and others in 2. About 60 percent of the patients had Stage III or IV diseases. Postoperative complications occurred in 27 patients (38.6%) and 3 died within 30 days after operation (operative mortality rate. 4.3%). Pneumonia, empyema, atelectasis and arrhythmia were prominent postoperative complications. There were no statistically different postoperative complication rates between those with UML and those of MLL. Five-year survival rate of the patients with bilobectomy for lung cancer was 25.7%, which was between those with single lobectomy (36.9%) and those with pneumonectomy (5.6%). However, there was no statistical difference in 5-year survival rate among operative procedures in each stage.

[Lung function following sleeve bronchoplastic lobectomy for bronchogenic carcinoma].

Twenty-five patients who underwent upper sleeve lobectomy for lung cancer were studied by spirometric examination to evaluate postoperative pulmonary function, including Vital Capacity (VC), %Vital Capacity (%VC), Forced Expiratory Volume in one second (FEV1.0), and Forced Expiratory Volume in one second as a percentage of the forced vital capacity (FEV1.0%). The results of the patients with bronchoplasty were compared with those of patients with lobectomy alone and of patients with pneumonectomy. Lung function was periodically examined postoperatively following bronchoplasty. VC, %VC and FEV1.0 were decreased postoperatively in the patients with upper sleeve lobectomy, while FEV1.0% was increased. There were no differences in postoperative %VC or FEV1.0% between the patients with bronchoplasty and those with upper lobectomy alone. However, the postoperative %VC of the patients with pneumonectomy was significantly decreased compared with patients who underwent postoperative %VC of the patients with pneumonectomy was significantly decreased compared with patients who underwent bronchoplasty or lobectomy alone. %VC in the patients with bronchoplasty was decreased at 3 months after operation, but it to gradually returned to the preoperative value by 13 to 24 months after operation.

[Proliferative activity assessed on the basis of DNA ploidy patterns in primary lung cancer].

Flow cytometric cellular DNA-RNA content analysis by acridine orange staining was conducted on surgical fresh specimens of primary lung carcinomas from 66 patients (31 squamous cell carcinomas, 34 adenocarcinomas and 1 large cell carcinoma). The frequency of aneuploid tumors was 84.6% among the tumors. RNA content (RNA Index) in the DNA aneuploid tumor much more significantly (p less than 0.05) increased than the DNA diploid tumor. Tumor doubling time in the DNA aneuploid tumor was significantly (p less than 0.05) shorter than in the DNA diploid tumor. In the patients with lung cancers that recurred within 1 years, recurrence of the DNA aneuploid tumor was higher than the DNA diploid tumor. It is evident from the above results that proliferative activity in the DNA aneuploid tumor increases much more than in the DNA diploid tumor. This in turn may induce early recurrence in patients with lung cancer.

[Observation of bleeding sites by fiberoptic bronchoscopy in a case of vasculo-Behçet's disease with pulmonary involvement].

A 45-year-old man complained of fever and bloody sputum. Chest X-ray films showed a round opacity in the left hilum. He developed massive hemoptysis (500 ml) and emergency embolization of the bronchial artery was performed. Pulmonary artery aneurysm and thrombosis were detected angiographically, and fiberoptic bronchoscopy revealed a reddish irregular eminence of the left main bronchus and lingulate++ ++ bronchus. These sites could have been the bleeding sites. Because of recurrent hemoptysis, left pneumonectomy was performed 26 days after the massive hemoptysis. Pathological findings showed panarteritis, luminal thromboembolism of the pulmonary artery and markedly dilated vessels of the bronchial wall. He developed genital ulceration after the operation and these findings suggested incomplete vasculo-Behçet's disease with rare pulmonary involvement. The disease is now well-controlled by corticosteroid therapy 27 months after the operation.

[Results of the surgical treatment of patients with advanced small lung cancer].

From 1970 to 1988, 603 patients with non-small cell lung cancer were operated in our department. Among them, patients with small lung cancer (diameter less than 3 cm) with advanced disease were analyzed. There were 19 patients with mediastinal node metastasis, 2 with contralateral or supra-clavicular node metastasis, 3 with distant organ metastasis and one with pleural dissemination. In T1N2 group, adenocarcinoma was predominant in 17 patients and only 2 with squamous cell carcinoma. Sixteen patients underwent curative lobectomy with mediastinal node dissection. In T1N3 group, lobectomy and mediastinal and cervical node dissection were carried out. In T 1M1 group, 3 patients had lobectomy and removal of metastatic lesions, separately or simultaneously. One patient with pleural dissemination had segmentectomy and extirpation of disseminated foci. There were no operative deaths or hospital deaths in this series. Survival of the patients with T1N2 with curative resection was 93.8% in one year, 38.2% in 3 years and 22.9% in 5 years after operation. Among them, patients with intraoperative diagnosis of negative mediastinal node had relatively well survival. However, patients with N3, D+, or M1 had poor prognosis.

[Results of pulmonary angioplastic operations for cases of bronchogenic carcinoma].

Pulmonary artery reconstruction for bronchogenic carcinoma was performed in 36 patients (5.8%) in our department between 1955 and 1988 for the purpose of preservation of pulmonary function. Twenty-six patients had squamous cell carcinoma (72.2%) and 24 had Stage IIIA. The operative procedures were sleeve resection of the main pulmonary artery (P.A.) and end-to-end anastomosis in 20 cases, wedge resection of P.A. and transverse suture in 14 and window type resection of P.A. and patch closure in 2. In 28 patients bronchoplastic procedures were combined. Three patients (8.3%) died within 30 days after operation. Postoperative complications were relatively high in the patients with combined operations. The 5-year survival rates were 45.7%. Postoperative pulmonary functions were evaluated with spirometric examinations and pulmonary scintigrams. There were no differences in postoperative pulmonary functions between pulmonary angioplastic procedures and simple lobectomies.(ABSTRACT TRUNCATED AT 250 WORDS)

[Malignant fibrous histiocytoma in the chest cavity, report of four resected cases].

Case 1: 35-year-old man. Chest X-P showed a mass shadow, which was 6 X 5 cm in size, well defined margin and homogeneous. He underwent enucleation but died 6 months after operation with local recurrence. Case 2: 55-year-old man. A peduncle tumor, which was 5 X 4 cm in size, arising from the left parietal pleura was resected. 2 years after the operation, local recurrence occurred at left parietal pleura and the lesion was resected. 9 years after the first operation, he is still alive free from the disease. Case 3: 62-year-old man. Multiple masses which invaded to the chest wall involving rib, lung and pericardium were situated in the site of left parietal pleura. He underwent partial resection of chest wall, parietal pleura, pericardium and lung, but he died 8 months after the operation with local recurrence. Case 4: 49-year-old man, a mass which completely obstructed the left main pulmonary artery situated at the site of left hilar region. He underwent left pneumonectomy. 2 year after the operation, he is still alive free from the disease. Malignant fibrous histiocytoma in the chest cavity is very rare in frequency. We have experienced recently 4 cases, and discussed with literature.

[Analysis of cellular DNA-RNA content using flow cytometry between primary and metastatic foci in lung carcinoma].

By flow cytometry, cellular DNA-RNA content was analyzed with Acridine Orange staining for primary tumor and the metastatic foci in 12 cases of primary lung carcinoma. Five (42%) of the primary and metastatic foci showed heterogeneous cellular DNA content, with high cellular DNA content of metastatic foci in all cases. Cellular RNA content was significantly higher in metastatic foci than in primary foci, indicating possible higher proliferative activity of the former foci. Three different metastatic foci in one patient showed essentially the same cellular DNA content. From the above it is evident that primary and metastatic foci of lung carcinoma show considerable heterogeneity not only in cellular DNA content but RNA content as well, thus indicating proliferative activity of the metastatic focus. It is consequently advisable to use a combination of various supplementary medications differing in cytocidal effect. Cases with higher cellular DNA content may likely have higher metastatic potential.
