[Results of re-thoracotomy for the management of early postoperative complications after bronchoplastic operations].

We reviewed cases of re-thoracotomy performed for early complications after bronchoplastic procedures. One hundred and sixteen bronchoplasties were performed in our department over 20 years. The diseases for which bronchoplasty was undertaken were lung cancer in 102 patients (87.9%), tuberculous stenosis of the bronchus in eight, esophageal cancer in three, and trauma in three. The most frequent postoperative complication was difficulty of expectoration and atelectasis, which generally improved with conservative treatment. Re-thoracotomy was performed for early postoperative intrathoracic complications on 11 patients. The reasons for re-thoracotomy were bronchial anastomotic dehiscence in five cases, obstruction of bronchial anastomosis in two, atelectasis in two, and occlusion of anastomosis of pulmonary arterial angioplasty in two cases. All except two underwent re-thoracotomy within two weeks of the first operations. The operative procedures performed were completion pneumonectomy in six cases, re-bronchoplasty in three, suture of anastomotic dehiscence in two, patch closure of pericardial defect with aspiration of secretions in the atelectatic lobe in one, and partial decortication with suture closure of the alveolar fistula in one. Pedicled omental wrapping was applied to two patients with re-bronchoplasty and one with completion pneumonectomy. Post-operative complications after re-thoracotomy were anastomotic insufficiency in two cases, bronchopleural fistula in two, and pneumonia in one. Two patients underwent a third thoracotomy. There was no anastomotic dehiscence or bronchopleural fistula in the patients with pedicled omental wrapping. One patient died due to bronchopleural fistula within 30 days of re-thoracotomy. Six patients died of recurrence or pneumonia from 39 days to one year after re-thoracotomy.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical application of the omentum in chest surgery].

The benefit of the omentum in chest surgery was experimentally evaluated on the basis of findings in relation to recanalization of the bronchial artery in bronchoplasty with and without omental wrapping. The arterial blood supply at the anastomotic sites was observed in the early stage following bronchoplasty using the omental wrapping. Clinical application of the omentum in chest surgery was performed by wrapping anastomotic site at tracheobronchoplasty in 3 cases, bronchial stump coverage after lobectomy or pneumonectomy in 2 cases, repair of bronchial fistula following pneumonectomy in 1 case and repair of bronchopleurocutaneous fistula in pulmonary tuberculosis in 1 case. Six of these clinical cases could have favorable postoperative results. Based on these experimental and clinical results, we consider that the omentum is very useful tissue for wound healing in chest surgery.

[Pharmacokinetics of cisplatin and vindesine in a patient with chronic renal failure undergoing hemodialysis].

Plasma total platinum and vindesine levels were monitored in a patient with chronic renal failure undergoing hemodialysis. Plasma levels of cisplatin were determined as platinum by atomic absorption spectrophotometry, and plasma vindesine levels were measured by radioimmunoassay. Fifty mg. of cisplatin in combination with 3 mg. of vindesine were infused for 60 minutes before dialysis. During dialysis, plasma total platinum levels decreased rapidly in a biphasic fashion and 1.76 at 30 hours after infusion. Plasma vindesine levels declined with higher concentration. In conclusion, it is considered that cisplatin and vindesine can be given to anuric patients undergoing hemodialysis. However, it is suggested that smaller doses should be given to prevent side effects.

Antineoplastic Combined Chemotherapy Protocols↗

[Surgical experience with tracheoplasty].

We have had experience with 31 tracheoplastic procedures. Underlying diseases consisted of primary tumor, cancer invasion from carcinoma of the thyroid gland and the esophagus, traumatic rupture and post-tracheostomy and tuberculous stenoses. During operation, respiratory support has been satisfactorily established by using the intubation method via the operative field and/or high frequency ventilation instrument. Furthermore, development of absorbable suture material like Dexon and Vicryl benefited by minimizing granulation stenosis at anastomosis due to less foreign body reaction. The prognosis for carcinomatous involvement from surrounding organs was much worse according to advanced cancer stages. However, surgical outcome of this procedure was satisfactory in terms of relieving respiratory distress.

Congenital saccular aneurysm of the superior vena cava.

A 69-year-old woman was admitted to our hospital for surgical treatment of a mediastinal mass. Midsternotomy was performed which revealed a saccular aneurysm of the superior vena cava, 6 cm in diameter, in the right superior mediastinum. Aneurysmectomy was done by dividing a 15 X 10 mm venous connection with the superior vena cava and the patient's postoperative course was uneventful.

[Changes in cell cycle and RNA content in cultured cancer cells treated with cis-diamminedichloroplatinum (II)].

Recent reports have shown that CDDP interacts with RNA and protein as well as DNA. We studied the alteration of cell cycle, cellular RNA content and the effect of nucleic acid metabolism on cultured cancer cells after treatment with CDDP by flow cytometry and 3H incorporation assay. The alteration of cell cycle was found to be accumulation of cells in after delay S phase in cytostatic concentrations, CDDP inhibited 3H-TdR uptake markedly at this time and 3H-UR uptake earlier. Increase in RNA content accompanied accumulation of cells in G2M phase. This increase was not a specific phenomenon caused by CDDP, because increase in RNA content was also induced by other inhibitors of DNA synthesis. It is more likely that the direct alteration of cell cycle and cellular RNA content due to action of DNA-combined CDDP rather than that of RNA-combined CDDP.

Influence of suture on bronchial anastomosis in growing puppies.

A comparison study of synthetic nonabsorbable suture (nylon or Prolene) with absorbable suture (Dexon-S or Vicryl) for bronchoplasty was performed in growing puppies. The experiments (n = 15) were followed up for 185 to 381 days (average 323.8 days). Bronchoscopic and bronchographic studies were done at intervals. No stenosis was observed in the group receiving absorbable suture (n = 8), and the anastomoses grew proportionately with the proximal and distal bronchi. In the group receiving nonabsorbable suture (n = 7), variable stenoses developed; two moderate and two severe stenoses were observed during the early healing stage by bronchoscopic examination. In the sequential bronchograms, an increase of stenosis was noted in two dogs. The results suggest that absorbable suture is superior to nonabsorbable suture in pediatric bronchoplasty.

[The concomitant effects of SM-4300 with some antibiotics on severe bacterial and/or fungal infections in surgery].

SM-4300, a newly developed human immunoglobulin preparation, has been studied for the safety and effectiveness/efficacy in the 16 patients with severe bacterial and/or fungal infections which are resistant to antibiotic therapy. Clinical effect of 16 cases were excellent 1 case, good 4, fair 4, poor 4 and unknown 3. The efficacy rate was 38.5% and the efficacy rate including fair was 69.2%. No remarkable adverse reaction due to administration of SM-4300 were observed.

[Limited operation for lung cancer].

The availability of a limited operation for the treatment of primary lung cancer was discussed on 67 cases who underwent either wedge resection or segmentectomy. The reasons for the application of limited operation were mainly cardiopulmonary failures such as hypertension, abnormal finding on the ECG and obstructive ventilatory failure including the elderly over 70 years of age. Favorable prognosis of a limited operation was obtained in T1N0 and T2N0 cancer when compared to those who underwent lobectomy. Improved prognosis following a limited operation was more attributable to N-factor rather than T. Furthermore, early postoperative course and performance status after a limited operation were more favorable rather than lobectomy. In particular, on the first day of a limited operation, hemodynamic changes in an increased afterload of the right ventricle and reduced left ventricular work tend to minimize in patients with a limited operation. An experimental study also elucidated the advantage of this operative procedure in terms of cardiac function after one or bilobectomy on coronary diseased dogs whose descending coronary artery was made to be a 50% stenosis. In conclusion, it was emphasized that a limited operation should be indicated for patients with T1N0 and T2N0 cancers of primary lung cancer and limited cardiopulmonary function.
