Prophylactic fluconazole promotes reepithelialization in full-face carbon dioxide laser skin resurfacing.

BACKGROUND AND OBJECTIVE: Laser skin resurfacing is used to treat photodamaged skin, rhytids, and acne scarring. Many patients are placed on antibiotics and antivirals pre- and postoperatively. The purpose of this study was to determine whether prophylactic fluconazole increased the rate of reepithelialization in patients undergoing full-face CO(2) laser skin resurfacing. STUDY DESIGN/MATERIALS AND METHODS: Ninety-one patients underwent full-face CO(2) laser skin resurfacing with the Coherent Ultrapulse 5000C. At least two passes of 300 mJ, density 5, were used except periocularly. Study group I consisted of 48 consecutive patients who received either cephalexin or ciprofloxacin for 7 days postoperatively. Study group II consisted of 43 patients who received 300 mg of fluconazole on postoperative days 3-8, in addition to ciprofloxacin. Both groups received acyclovir 400 mg t.i.d. pre- and postoperatively. RESULTS: Time to complete reepithelialization was compared between the groups by t-test. Group II reepithelialized significantly faster than group I (7.65 +/- 1.20 days vs. 10.27 +/- 2.94 days; P < 0.0001). Ninety-five percent of patients receiving fluconazole (group II) healed completely by day 9 versus only 53% of patients in group I. CONCLUSION: Fluconazole administered postoperatively between days 3 and 8 significantly promotes reepithelialization in patients undergoing full-face CO(2) laser skin resurfacing.

Extensive lower eyelid pigment spread after blepharopigmentation.

PURPOSE: To report on a complication of blepharopigmentation. METHODS: Case report. RESULTS: An 81-year-old woman underwent cosmetic tattooing of all four eyelids. The pigment immediately spread from the right lower eyelid lash line to the nasojugal fold. The dispersion was so extensive in surface area that nonsurgical techniques for ameliorating the dispersion would not be effective. The unintentionally tattooed skin was excised and reconstructed with a lateral canthal suspension and full thickness skin grafts. Pathologic examination of the excised tissue revealed pigmentation of all layers of the skin and the surface of the orbicularis muscle. CONCLUSION: This case illustrates that extensive pigment spread following blepharopigmentation may require extensive reconstruction to correct the problem.

Paralytic ectropion: a complication of malar implant surgery.

The use of the malar implant to augment the malar-zygomatic eminence is rapidly becoming a popular aesthetic procedure; however, this surgery can lead to paralysis or paresis of the facial nerve. Paralytic ectropion may result from orbicularis oculi dysfunction. We report two cases of paralytic ectropion as a result of malar implant placement. Conservative management for mild orbicularis oculi dysfunction consisted of topical lubricants and observation, whereas persistent ectropion required surgical repair. Paralytic ectropion and secondary exposure keratopathy are possible complications of malar implant surgery.

Lipogranuloma: a preventable complication of dacryocystorhinostomy.

This report describes the unusual occurrence of lipogranuloma at the incision site of dacryocys-torhinostomy. Four patients had soft cystic masses after external dacryocystorhinostomy for acquired nasolacrimal duct obstruction. This was attributed to the antibiotic ointment vehicle that coated the petroleum jelly-impregnated gauze placed intranasally to prevent epistaxis. Postoperatively, all four patients had an encapsulated cystic mass within 3 weeks. Three of the masses were excised under local anesthesia and one was aspirated. None of the masses recurred.

Double rhomboid silicone rod frontalis suspension.

The frontalis suspension operation has proven effective for patients with significant ptosis and poor levator function. In patients with poor eye protective mechanisms, silicone rods may be used since the material has enough tensile strength to raise the eyelids yet it is sufficiently elastic to allow closure via action of the orbicularis muscle. This article reports the technique and results of a modified operation that uses two silicone rods per upper eyelid and eliminates fixation sutures to tarsus inferiorly and frontalis muscle superiorly. The results obtained are discussed. This technique provides a better upper eyelid contour and is easily adjustable postoperatively.

Transient internal ophthalmoplegia during blepharoplasty. A report of three cases.

The present paper presents three cases of transient internal ophthalmoplegia during blepharoplasty. All cases were characterized by a dilated pupil that was unreactive to light or an accommodative stimulus. There were no cases of visual loss or permanent pupillary abnormalities. We attribute the temporary pupillary mydriasis and accommodative insufficiency to anesthesia of the short ciliary nerves or ciliary ganglion from local anesthetic diffusing into the orbit at the time of injection. To the best of our knowledge this is a complication of eyelid surgery that has not been previously described.

Cicatricial ectropion in lamellar ichthyosis.

A 35-year-old woman presented with bilateral lower lid ectropion associated with lamellar ichthyosis. She was successfully treated with postauricular skin grafts and horizontal lid shortenings. This report reviews the literature on the surgical management of this condition and suggests several modifications to prevent recurrence.

Subperiosteal volume augmentation of the anophthalmic socket with RTV silastic.

Patients who have worn an ocular prosthesis for many years often develop laxity of the lower eyelid and deepening of the superior sulcus caused by downward displacement of the orbital contents. Herein we describe our technique and results with the lateral canthal sling and subperiosteal room temperature vulcanizing (RTV) silastic volume augmentation procedure performed through a single incision in the lateral canthus.

Fibrous capsule from extruded implant used as graft material in cul-de-sac reconstruction.

A 74-year-old woman underwent radiation therapy for ocular malignant melanoma. The eye was subsequently enucleated. Three years later, implant migration and conjunctival contracture necessitated a socket reconstruction. The fibrous capsule that formed around the migrated implant proved to be an easily harvested and effective tissue for expanding the contracted socket.

Cyanoacrylate adhesive: use in frozen section examination of eyelid tumor margins.

Frozen sections are used to determine the extent of eyelid tumors. The contraction of skin that occurs between the time of eyelid skin excision and frozen section preparation may result in the false impression that the tumor is incompletely excised. This requires the unnecessary removal of additional normal tissue. We have demonstrated in animal and human studies that the surface application of cyanoacrylate adhesive to skin significantly reduces the degree of skin contraction. Furthermore, the cyanoacrylate coating provides added rigidity that prevents the tissue margins from rolling inward. This facilitates the production of full-thickness, vertically oriented sections that are perpendicular to the skin surface. The cyanoacrylate coating prevents folding of the sections at the time of microtome cutting. The adhesive does not interfere with the staining, interpretation of the histopathology, or subsequent reconstruction.

Scleropachynsis maculopathy.

To our knowledge, this is the first clinicopathologic report of a previously unrecognized maculopathy in which there is thickening and mucopolysaccharide deposition in the sclera subjacent to the macula. A healthy 50-year-old man had bilateral mottling of the retinal pigment epithelium (RPE) in the macular region. The left eye was enucleated because of a choroidal melanoma. Histopathologic examination disclosed a thickened sclera with abnormal collagen fibrils with diameters up to 5,800 A and compressed choroid. The overlying RPE had areas of hypopigmentation, hyperpigmentation, and hyperplasia and contained intracytoplasmic lipofuscin.

Optic disc changes with intracranial subarachnoid cysts.

We have recently encountered two patients who presented with ophthalmoscopically unusual optic disc changes, and who were found to have intracranial subarachnoid cysts. The discs appeared chronically swollen, had microtelangiectatic vessels on the surface and leaked fluorescein. This report documents unusual swollen discs in patients with no clinical evidence of increased intracranial pressure yet who are found to have intracranial subarachoid cysts by computed tomography.

Iodine disinfection of hydrophilic contact lenses.

A simple and effective technique for disinfecting hydrophilic contact lenses uses a saturated solution of molecular iodine in water. This technique has disinfected lenses that were incubated in actively growing cultures of bacteria and fungi in concentrations of 10(7) to 10(8) organisms per ml. Ocular tolerance studies showed that lenses disinfected by this method did not damage rabbit cornea, as demonstrated by slit lamp and histologic observations. This method has a built-in safety feature because iodine-soaked lenses are yellow, while lenses rinsed free of iodine are clear.

Optic nerve clipping for hemostasis during enucleation.

An instrument is described for sectioning the optic nerve during enucleation that has several advantages over presently used techniques. the instrument applies a nonreactive, tantalum clip around the optic nerve, and a built-in knife blade transects the nerve anterior to the clip. In addition to providing rapid and effective hemostasis of the vessels within and surrounding the optic nerve, the device cuts only tissue that is encompassed by the clip and not extraneous structures.

Control of tumor growth in animals by infusion of an angiogenesis inhibitor.

Angiogenesis and tumor growth were inhibited in two different animal models by regional infusion of a partially purified cartilage extract. In rabbits bearing corneal implants of V2 carcinoma and receiving the inhibitor, vascular growth rates were < 3% of those in control animals receiving either Ringer's solution or bovine trypsin inhibitor (Trasylol). Subconjunctival B16 melanoma implants in mice receiving the inhibitor weight < 2.5% of implants in mice receiving Ringer's solution, Trasylol, or albumin. Histologic study of major organs and standard blood tests revealed no toxic effects in any of the animals. The inhibitor did not retard the growth of either tumor cell type in tissue culture at concentrations as high as 1 mg/ml. These results suggest that the cartilage factor does not interfere with the growth of the tumor cell population directly but that it prevents tumor growth by inhibiting angiogenesis.

Stromal vascularization prevents corneal ulceration.

Experiments were performed with a model of focal, thermal-induced ulceration to test the clinical impression that vascularization prevents ulceration of the corneal stroma. Slow-release polymers containing a vasoproliferase agent (tumor angiogenesis factor) were placed in corneal pockets 2 mm central to the limbus of albino rabbits. These polymers elicited blood vessel ingrowth up to the implant. Control eyes received empty polymers which caused minimal to no vessel growth. Polymers were removed, and each cornea received a focal, thermal burn placed just central to the polymer site. All control corneas ulcerated: most (79%) developed deep stromal or perforating ulcers. Only 25% of prevascularized corneas developed stromal ulcers, and none was deep or perforating. After thermal burns, vessels in both groups grew at the same linear rate toward the burned area. There was a direct relationship between the distance separating the nearest blood vessel and the burned area at the time of burning and the maximum depth of stromal ulceration. Thus prevention of or less severe stromal ulceration is correlated with the earlier presence of vessels in the burned area.
