Spontaneous thrombosis of the renal vessels. Rare entities to be considered in differential diagnosis of patients presenting with lumbar flank pain and hematuria.

We report 2 cases of spontaneous thrombotic occlusion of the main renal vessels presenting with acute lumbar flank pain and hematuria suspect of nephrolithiasis. Clinical and laboratory signs of blood hypercoagulability, generalized arterial embolism, nephrotic syndrome or glomerulonephritis were absent. Excretory urography, nephrosonography and retrograde ureteropyelography showed no evidence of upper urinary tract calculi or other causes of obstruction. Renal angiography and cavography demonstrated an acute renal vein thrombosis in 1 patient and a thrombotic occlusion of all but one of the segmental renal arteries in the other patient. These 2 cases demonstrate that thrombotic occlusion of the renal artery or renal vein has to be considered in patients who are presenting with lumbar flank pain and hematuria, in whom the excretory urogram shows severe malfunction of one of the kidneys, and stone disease can be excluded. Renal angiography and cavography as well as CT scan should be carried out in these patients. When the disease is diagnosed at an early stage, an intra-arterial thrombolysis can be attempted.

Prostate-specific antigen and prostate acid phosphatase in the detection of early prostate cancer and the prediction of regional lymph node metastases.

Serum values of prostate-specific antigen (PSA) and prostatic acid phosphatase (PAP) were determined in 180 patients prior to pelvic lymphadenectomy and radical prostatectomy and in 40 patients prior to pelvic lymphadenectomy alone. In all tumor stages, PSA was superior to PAP in detecting cancer of the prostate. By PSA determination using a cutoff level of 4 ng/ml (Tandem assay), 28.8% of the patients with prostate cancer, stage pT2pN0M0, and 17.8% of the cases with a stage pT3pN0M0 tumor could not be detected. All these tumors had been noticed, however, by digital rectal examination. This indicates that PSA determination cannot replace digital rectal examination as a screening method for prostate cancer. In this study, it was possible neither by PSA nor by PAP to define a practicable cutoff level for patients with and without lymph node metastases. A clear differentiation between the stages pT2pN0M0 and pT3pN0M0 was not possible by either PSA or PAP alone.

Value of nuclear DNA ploidy patterns in patients with prostate cancer after radical prostatectomy.

Flow cytometric analysis of DNA ploidy was performed on prostatic adenocarcinoma specimens from 80 patients. In all these patients a radical retropubic prostatectomy had been performed. The nuclei for DNA ploidy determination were extracted from paraffin-embedded material of whole sections of the prostate from patients treated by radical prostatectomy between 1980 and 1985. DNA ploidy was a strong prognostic indicator independent of tumor grade and tumor stage. DNA ploidy offered additional information on both tumor stage and tumor grade. In stage C disease the likelihood of progression-free survival was 89.5% in diploid tumors and 27.8% in aneuploid tumors after 9 years. In tetraploid tumors all patients progressed after 9 years. The computed survival rates in stage C disease showed that patients with diploid tumors did significantly better than those with aneuploid or tetraploid tumor patterns. These data indicate therefore that DNA ploidy patterns determined by flow cytometric analysis provide important additional prognostic information on prostatic adenocarcinoma treated by radical prostatectomy.

Influence of cyclic interferon gamma on lymphocytes and their subpopulations in patients with renal carcinoma.

Natural killer (NK) cell stimulation can be repeatedly elicited by cyclic administration of recombinant human interferon (rHu IFN) gamma in patients with metastatic renal cell cancer. In all cycles of treatment, the elevation of the cytolytic activity of the NK cells was statistically significant. The shift of the T4/T8 ratio (T-helper/T-suppressor lymphocytes) a short time after the commencement of IFN application might be a manifestation of the immunostimulation induced.

Extracorporeal shock wave lithotripsy in patients with cardiac pacemakers.

Between May 1985 and July 1988, 11 patients with cardiac pacemakers were treated for renal or ureteral calculi by extracorporeal shock wave lithotripsy. During treatment and at follow-up no dysfunction of the pacemakers was noted. There were no cardiac complications during extracorporeal shock wave lithotripsy. All 11 patients were free of stones within 3 months after therapy. From these data it can be concluded that in patients with cardiac pacemakers extracorporeal shock wave lithotripsy of renal or ureteral stones can be performed safely when certain precautions are taken and special attention is paid to the electrophysiology of the pacemaker.

Diagnostic value of transrectal ultrasound in tumor staging and in the detection of incidental prostatic cancer.

To evaluate the accuracy of transrectal ultrasonography in staging and detection of prostatic cancer, a prospective study was initiated. In 466 patients transrectal ultrasonography was performed before transurethral resection, suprapubic adenomectomy, or radical prostatectomy. The findings were compared with the histopathologic results. In 160 of the 466 patients a carcinoma of the prostate was proven by histology. In 21 patients the cancer was an incidental finding after transurethral resection. In 133 of the 160 patients (83.1%) the tumor was detected by ultrasound prior to surgery. The analysis of the results revealed, however, great differences in the accuracy in the different tumor stages. In those patients where the tumor was confined to the prostate, the carcinoma was identified only in 37.5% of the cases. Especially prostatic cancer stage pT1a-b (incidental) was detected only in 3 out of 21 patients. Thus, it can be concluded that transrectal ultrasonography is not more sensitive than the rectal palpation for detection of prostatic cancer stage T1a-b.

Cyclic interferon gamma treatment of patients with metastatic renal carcinoma.

The treatment of metastatic renal carcinoma is still unsatisfactory because of the lack of effective systemic therapy. In 14 patients with metastatic renal carcinoma an attempt was made to influence the course of the disease by administration of recombinant human interferon (rHu-IFN) gamma; 9 patients were evaluated after treatment. There was partial remission in 3 patients, stable disease in 2 and tumour progression in 4. The cyclic application of rHu-IFN at a dose of 0.25 mg/day for 8 days, with treatment-free intervals of 3 to 4 weeks, was tolerated well. Side effects consisted mainly of fever and leucocytopenia. From this small series it seems that cyclic IFN gamma treatment might be helpful in some cases of metastatic renal carcinoma.

Evaluation of the T stage of carcinoma of the bladder by transurethral ultrasonography.

In order to evaluate the accuracy of transurethral ultrasonography in predicting the T stage of bladder tumors a prospective study was initiated. Transurethral ultrasonography was performed in 308 consecutive patients prior to cystoscopy, transurethral resection, biopsy or cystectomy. The findings of ultrasonography were compared with the histopathologic results. In 218 out of the 308 patients a carcinoma of the bladder was diagnosed histologically, whereas in 90 patients a tumor could not be detected on histological examination. In 78.2% of those patients, in whom a carcinoma of the bladder was present, it was possible to predict the correct T stage. It was not possible, however, to distinguish tumors of stage TA from those of stage T1. Further analysis of the results revealed an overstaging of the tumor in 12.0%, whereas in 8.0% of the cases the tumor was understaged or had not been recognized at all by ultrasonography.

Influence of interferon alfa-2c on the kinetics of spontaneous cell-mediated cytotoxicity in urothelial carcinoma in vivo and in vitro.

It was demonstrated that patients with superficial bladder cancer displayed a raised spontaneous cell-mediated cytotoxicity (SCMC) compared to patients with advanced bladder cancer and healthy control subjects. By use of recombinant interferon alfa-2c, the activity of the spontaneous cell-mediated cytotoxicity at the level of the individual cell could be increased both in vitro and in vivo. In vitro, this was the case in patients with superficial bladder cancer as well as in patients with advanced bladder carcinoma, and in healthy control subjects. The kinetics of cytolysis were not affected by recombinant human interferon (rHu IFN) alfa-2c. After in-vivo application of rHu IFN, there was an elevation of the target binding cells (TBC) and the number of active natural killer (NK) cells within 24 h, but this was only detected for a brief period of time.

Antiproliferative effects of interferons against human bladder carcinoma cell lines in vitro.

In order to evaluate the antiproliferative effects of recombinant human interferon-gamma 2c (rHu IFN-gamma 2c), recombinant human interferon-gamma (rHu IFN-gamma), natural interferon-beta (IFN-beta), and their combination with cytotoxic agents, 17 different human bladder carcinoma cell lines were tested in vitro. The antiproliferative effects were compared in evaluating the tumor cell inhibiting potency of the different interferon (IFN) classes. It could be demonstrated that interferons have inhibiting effects on the bladder cancer cell multiplication rate, yet there are significant differences in the susceptibility of different IFN preparations on different cell lines. The cell lines BT1, RT4, EJ, 468P, 253J, SD, TCCSUP, and SW1738 can be defined as sensitive, T24, 647V, VM-CUB2, and J82 as semisensitive, HT1376, 5637, VM-CUB1, 639V and SW1710 as resistant upon treatment with IFN. The combination of rHu IFN-alpha 2c, IFN-beta, and rHu IFN-gamma seems to be more effective than treatment with rHu IFN-alpha 2c alone. The cytotoxic effect of doxorubicin on bladder cancer cells can be intensified by combining it with IFN.

Transrectal aspiration biopsy and punch biopsy in the diagnosis of prostate carcinoma--a comparative study and literature review.

It can be concluded from the data presented here, that perineal punch biopsy is able to supply more reliable results than fine needle aspiration biopsy in the diagnosis of prostate cancer. The advantage of aspiration biopsy, however, is its low incidence of complications. Since the application of both methods, namely transrectal aspiration biopsy and perineal punch biopsy, offers an additional factor of reliability, it appears worth-while to utilize both procedures in the diagnosis of carcinoma of the prostate.

Comparative value of ultrasonography, computerized tomography, angiography and excretory urography in the staging of renal cell carcinoma.

From 1982 to 1985, 225 patients with renal cell carcinoma were treated by nephrectomy. To evaluate the diagnostic significance of ultrasonography in predicting tumor stage the results of ultrasonography, computerized tomography, renal angiography and excretory urography were compared to the histopathological findings. Since local tumor extension has a considerable impact on the operation strategy evaluation of the T classification was of particular interest. All 4 diagnostic procedures were performed in 73 of the 225 patients. The T stage was determined correctly by ultrasonography in 77.8 per cent of the patients, while the tumor was not identifiable in only 0.6 per cent. Computerized tomography was almost as reliable as ultrasonography (the T stage was predicted correctly in 72.3 per cent of the examinations). In contrast, the tumor was staged correctly by angiography in only 57.2 per cent of the patients and by excretory urography in only 59.2 per cent. From these results ultrasonography appears to be an effective, noninvasive, inexpensive and safe procedure to evaluate the T stage of renal tumors.

Diagnosis and treatment of renal angiomyolipoma (based on 15 cases).

Fifteen cases of renal angiomyolipoma not associated with tuberous sclerosis are presented. The clinical and radiological features, the pathology and the management of these rare but well-documented tumors are discussed. They pose problems in diagnosis, and controversy still exists regarding their management. Additional techniques such as ultrasonography and computerized tomography may help in the diagnosis. When conservative surgery is not possible, or when a solid tumor cannot be confidently excluded, nephrectomy should be performed.

Are antireflux procedures indicated in patients with small pyelonephritic kidneys?

Antireflux surgery was successfully performed in 16 adults and children with bilateral or unilateral vesicoureteral reflux associated with a small pyelonephritic kidney. Although in 14 cases of the preoperative 131I-Hippuran clearance of the affected unit was less than 100 ml/min/1.25 m2, in 10 cases renal function was stabilized or slightly improved postoperatively. Only 1 child showed a severe deterioration of renal function. All 16 patients were cured of their acute pyelonephritis. Surgical correction of the reflux had no beneficial effect on renal size or renal scars. Urinary tract infection was eliminated in 12 of the 16 cases. It can be concluded that a small pyelonephritic kidney associated with a vesicoureteral reflux should not be removed even if its isotopic clearance is less than 100 ml/min.

Can recombinant human alpha-2 interferon prevent recurrence of high-grade superficial bladder tumors?

Eight patients with superficial bladder cancer were treated by systemic interferon (IFN) alpha-2 application to investigate whether the rate of local tumor recurrence could be diminished. All patients had recurrent high-grade urothelial cancer. Recombinant human IFN alpha-2 was administered by daily intramuscular injections for a period of 6 weeks. The natural-killer cell activity and the antibody-dependent cellular cytotoxicity was augmented for a short time only. However, this was of no clinical benefit. Tumor recurrence was observed in seven out of the eight patients within 6 months following therapy. IFN serum levels, which were measured during interferon treatment, did not inhibit tumor cell proliferation in vitro.

Immunological findings in patients with superficial bladder cancer during human alpha-2-interferon treatment.

6 patients with recurrent high grade superficial bladder cancer were treated with human recombinant alpha-2-Interferon (alpha 2-IFN). After initial transurethral electroresection of macroscopically visible tumor, human recombinant alpha 2-IFN was administered by daily intramuscular injections for a period of 6 weeks. The influence of IFN on the patients' immune reactivity was assessed by determination of the natural killer (NK) cell activity. Within the first week, in all the patients an increase of the NK cell activity was noted. This, however, was only a short-term effect. An increase of the IFN dosage did not lead to further NK cell stimulation. Determination of the IFN serum levels during IFN treatment revealed maximum levels of 260 IU/ml. They were measured 2 weeks after therapy begin. During treatment no further increase of IFN serum levels were found. The augmentation of the NK cell activity does not seem to depend on IFN serum levels. The amount of IFN found in patients' sera did not effect tumor cell growth in vitro.
