Risk factors for healing of duodenal ulcer under antacid treatment: do ulcer patients need individual treatment?

In order to identify the risk factors affecting the healing of duodenal ulcer, a clinical trial with effective dose of antacid was carried out in 53 patients. Duration of ulcer history, number of relapses, duration of the last and present relapse, number, duration and severity of pain attacks in the present ulcer relapse, pain radiation to back, vomiting, appetite, smoking habit, intake of analgesics and previous haemorrhage were registered. Number of ulcers, ulcer depth, bublar narrowing, erosions, duodenitis at initial endoscopy and healing of ulcer were assessed by one endoscopist. Basic and peak acid output were measured. The extent of duodenitis on the site opposite the ulcer was determined by histological examination. Sixty per cent of the duodenal ulcers were healed after three weeks. By univariate analysis, the following factors affect the healing; pain radiation to back and pain duration during treatment (p less than 0.001), multiple or deep ulcers, narrowing of duodenal bulb (p less than 0.01), number of pain attacks and poor appetite (p less than 0.05). By the stepwise logistic regression model, the following factors were selected as predictors for healing of duodenal ulcer with 76% correct classification: pain radiation to back (p = 0.002), deep ulcer (p = 0.013), multiple ulcers (p = 0.028). Number of cigarettes/day (p less than 0.007) and male sex (p = 0.036). By this model, the prediction of healing could be accurately assessed in 78% in a new sample. Individual treatment should be carried out on the basis of these factors.

N-alkanes induce the synthesis of cytochrome P-450 mRNA in Candida maltosa.

In the yeast Candida maltosa the level of cytochrome P-450 was 100-300-fold higher in alkane-grown cells than in glucose-grown ones (detected by a radioimmunoassay). It was shown immunochemically (1) that this was not the result of an assembly of preexisting apoenzyme with the prosthetic heme group. By cell-free translation of total poly(A)RNA in a wheat germ system and subsequent immunoprecipitation it was shown that the amount of mRNA coding for cytochrome P-450 paralleled its concentration in the cell.

A comparative immunological investigation of the alkane hydroxylating cytochrome P-450 from the yeast Candida maltosa.

The immunological relations of the cytochrome P-450 from the n-alkane utilizing yeast Candida maltosa to cytochrome P-450 forms of other organisms - yeasts, bacteria and mammalia - were investigated using a solid-phase double-antibody radioimmunoassay. Only the microsomal fraction of other n-alkane utilizing yeasts shows a distinct cross-reaction with an antiserum against cytochrome P-450 from Candida maltosa. Neither the tested bacterial nor the mammalian cytochromes P-450 cross-react with the antiserum.

[Ultrastructure of endocardial lesions in the carcinoid syndrome (author's transl)].

In a 67-year-old female patient with metastasising carcinoid of the small intestine there were right-sided endocardial plaques characteristic of the carcinoid syndrome. Ultrastructurally, the lesions showed proliferation of smooth muscle cells and myofibroblasts, which were surrounded by basement-membrane-like material and occasionally showed intercellular connections of the macula adhaerens type. The ground substance consisted of acid mucopolysaccharides, normal collagen fibrils, and microfibrils. The endocardial plaques in the carcinoid syndrome differ morphologically from other endocardial lesions by the lack of amorphous elastin. The formation of plaques and its relation to the endocrine activity of the carcinoid tumors is discussed, and pertinent literature is reviewed.

[How to support breast-feeding after delivery. results of an inquiry at 641 maternity hospitals in west germany (author's transl)].

Human milk is unique and impossible to imitate; psychologically, breast-feeding is an essential part of the natural mother-baby-relationship. Therefore available possibilities of encouraging breast-feeding should be fully used. To define these possibilities, in the autumn of 1977 a questionnaire was sent to all West German maternity wards in order to find out about incidence of breast-feeding as well as the external nursing conditions during the neonatal period. The results of this inquiry showed interesting correlations: Higher breast-feeding rates were reported by hospitals which encouraged all mothers very much to breast-feed, which allowed the mothers to nurse their neonates shortly after birth and which later gave the mothers th opportunity to nurse long and often. The best breast-feeding results were obtained by those hospitals that allowed rooming-in and self demand feeding. From these results - in accordance with several other studies - it was concluded, that these external factors seem to have a positive effect on successful breast-feeding.

The alkane-hydroxylating enzyme system of the yeast Candida guilliermondii.

The growth of the investigated Candida guilliermondii strain on n-alkanes induces an alkane-hydroxylating enzyme system, which consists of a cytochrome P-450 and a NADPH-dependent reductase. The cytochrome P-450 was purified to 4 nmoles per mg protein. Long-chain alkanes, preferably hexadecane to octadecane, are hydroxylated to the corresponding primary alcohol by this enzyme system. The substrate induces a type I spectrum, other compounds checked type II spectra.
