[Extended prevention of uterine cervix cancer].

A transverse-oval os uteri or Emmet' tear are the result of a tear at the mouth of the womb during final cervical dilatation. Furthermore, they are the cause of a more or less developed ectopia and combined cervical and vaginal fluor with concomitant colpitis. These alterations also represent the foundation not the cause, of a development of cervical carcinoma. In the sense of prophylactic reasoning the elimination of the symptoms mentioned above should be considered. -- The use of the speculum and care of the suture of the nearly always torn os uteri after each delivery must be the first step to avoid the development of a collum carcinoma. -- In the second place, electrocoagulation or shallow conisation with subsequent coagulation and cryo-surgical treatment of the ectopia has to be carried out. -- Therefore the obstetrician decides whether the patient -- in view of developing a collum carcinoma -- becomes a risk patient. He has to ensure that an almost normal portio uteri returns without ectropion and Emmet'tear and also without cervical and vaginal fluor, which could trouble the patient considerably. -- After every delivery, therefore, the speculum should be used. At the same time a cervix tear haemorrhage, one of the most frequent exsanguinations, can be avoided.

[The "martin-arm" system for abdominal wall fixation for many applications - experience from its use of gynaecology (author's transl)].

The "martin-arm"-System meets the demand for optimal fixation of outer edges of the wound cavity and exact positioning of organs in a three dimensional manner at abdominal operations. The four joints of each arm individually connected to the Op-table make this possible. They are fixed in position by a central joint with a lever which can be tightened. An adequate assortment of exchangeable retractors, specula and spatulas offers the possibility of a clear view of the operation. All instruments are ready for use even without the "martin-Arm". This system is especially suitable for emergency surgery, for small gynaecology wards or during staff shortages e.g. at night. The operation setting, according to requirements, remains in position for the duration of the whole operation. In the case of long operations, fatigue symptoms of the assistant are no longer present with this instrumentation.

Cytochrome P-450 and alkane hydroxylase activity in Candida guilliermondii.

In the investigated Candida guilliermondii strain after growth on n-alkanes as the only carbon and energy source 5--10 nMol cytochrome P-450 per g cells (wet weight) could be detected. Cytochrome P-450 and alkane hydroxylase activity was found in the 100 000 xg pellet. Cofactor studies and inhibition experiments revealed the existence of a NADPH-dependent cytochrome P-450 alkane hydroxylase system.

[Experiences in the fixation of vaginal prolapse using Fletcher's method].

The advantages of Fletcher's operation for vaginal pouch prolapse after previous hysterectomy or total extirpation are reported with a slightly modified method in eight cases. The following results show this surgical procedure to be especially advantageous: A permanent good fixation of vagina without shortening, no recurrence no pain and neither so-called antefixation complaints. A complete rehabilitation of cohabitation and orgasm was effected. Urological complications, such as obstruction of urine or micturition difficulties are eliminated by this surgical intervention.

[Early diagnosis of uterine rupture after previous cesarean section].

A new diagnostic measure for the early detection of still uterus rupture before birth and previous cesarian section is reported. With a portio ready for birth, which permits the finger to pass readily, the possibility exists, after stripping of the membranes, to examine digitally the old caesarian scar in respect of a beginning or already existing scar dehiscence. In 112 investigated patients with this method, once a beginning, six times an incomplete and once a complete uterus rupture prior to another partus was seen. In six cases a stenosis was found in the old section region with retention of the foetal head and threatening uterus rupture.
