[Ultrastructural findings in the cornea in xeroderma pigmentosum].

Xeroderma pigmentosum is a very rare disease caused by an inherited defect in the DNA repair system. The cells are less able to repair defects caused by UV radiation, which results in early aging of the skin and the development of malignant skin tumors. Robins assumes that the eye is involved in 60% of the cases. In our patient we found a dense opacity of both corneas in addition to the typical lentiginous alteration of the skin. Therefore, a corneal transplantation was carried out on both eyes. The corneal buttons obtained by keratoplasty were examined by light, and transmission electron microscopy. We found massive alterations in the tissues of all corneal layers.

[Endothelium microscopy studies following keratoplasty].

The quality of the endothelium in 15 patients with transplants after penetrating keratoplasty was analyzed. Donor age was not taken into account (average age of donors 65.8 years). At the time of the investigation the mean age of the grafts was comparatively high (oldest donor 92 years). There were only a few morphological changes in the endothelium and visual acuity was satisfactory. Even with low cell count (625/mm2) the graft had normal clarity. A number of pathologic changes in corneal endothelial cells after keratoplasty are described.

[Choice of the lens implant model].

Report on 530 lens implantation procedures performed in the years from 1981 to 1985. Different complications with different lens models are mentioned, in particular cystoid macular edema, subluxation or luxation of the artificial lens, vitreous complication, secondary cataract, and retinal detachment. A comparison was made with a group of aphakic eyes in which no lens implant was used. In a preliminary overall assessment in the group of eyes with lens implants there were no complications more severe than those in the group of aphakic eyes without artificial lenses.

[Intraocular infections in perforating eye injuries (author's transl)].

Our of 688 eyes with perforating injuries in 16 cases (2.33%) which were operated on in the years 1973 to 1979, a serious intraocular infection occurred. In 8.94% of the cases in which there were foreign bodies, and in 0.88% of the remaining cases, without intraocular foreign bodies, infection also occurred, although antibiotics were administered prophylactically. With intensive combined antibiotic therapy, and by performing vitrectomy as early as possible it is sometimes possible to save the bulb, and in some cases useful residual function can be achieved. The success of early results of vitrectomy in endophthalmitis recorded in the literature to date could not be obtained as late results in our cases.

[Light and electron microscopy studies of actinomycosis of the lacrimal duct].

For the first time we present electron-microscopic results of an actinomycosis of the lacrimal canaliculus. The interior of the actinomycotic conglomerate showed no evidence of a cellular defence reaction but in the loosly woven outer network of hyphae we observed a massive granulocytic reaction. Also, after phagocytosis the structure of the actinomycotic micro-organisms within the granulocytes is not significantly damaged. Within the tissue of the lacrimal canaliculus adjacent to the actinomycotic conglomerate we observed an increased number of plasmacells but no pathogenic organisms.

[Bacterial contamination of intraocular metallic foreign bodies].

Bacterial flora were cultured from 20 metallic intraocular foreign bodies which were removed by magnetic extraction. Bacterial flora were domonstrated in 35%. Report on the microorganisms isolated, the size of the foreign bodies and the time between the penetration and the magnetic extraction operation in relation to the contamination.

[Retinal detachment surgery: Incidence and causes of silicone sponge implant rejection].

In 51 patients with retinal detachments silicone sponge implants have been applicated. In 9 of these cases (17,6%) the implants had to be removed 3 weeks to 16 months postoperatively because of inflammatory reactions. The influence of these complications on the healing of the retinal detachment is considered as relatively unimportant. In light- and electron-microscopic examinations we could establish that the reactions are mainly caused by infections (which primarily spread within the caves of the sponge implants). Chronic infections caused by so-called apathogenic bacteria are more frequent than acute infections by so-called pathogenic micro-organisms. A foreign body reaction could be proved only in one case, in which the sponge was loosened and displaced. Defence reactions against the foreign body material probably develop only if mechanical irritation of the tissue is caused by the sponge material.

[Malignant melanoma of the choroid in melanosis bulbi (author's transl)].

Report of a case with melanosis bulbi observed in a relatively young patient of 33 years, in which a malignant melanoma of the choroid developed. As reported in earlier examinations the incidence of malignant melanoma in cases of melanosis bulbi is much higher than in normal eyes. The authors recommend strongly routine biomicroscopic examinations in cases of melanosis bulbi every 1--2 years to recognize the development of a melanoma in an early stage.

[Experience in therapy and prophylaxis of epidemic keratoconjunctivitis (author's transl)].

101 patients with epidemic keratoconjunctivitis were treated with different eye drops: cortisone, antibiotics and P.V.P.-Iodine. The treatment of 19 patients with P.V.P.-Iodine showed that inflammatory symptoms disappeared rapidly; corneal complications however such as superficial keratitis could not be prevented. After the outbreak of epidemic keratoconjunctivitis, severe hygienic measures had been taken at the eye-clinic. On account of the hygienic prophylactic measures further infections could be prevented at the clinic almost completely.

[Light- and electron-microscopic examinations of deep keratomycosis (aspergillosis) (author's transl)].

Light-microscopic and ultrastructural examinations of a corneal button with deep keratomycosis revealed that keratocytes are partly injured and partly of normal structure. In the cellular reaction, macrophages especially, are found that encircle hyphae with pseudopodia or cover the apical end of the fungi. In many sections, the fungal walls are very resistant to phygocytosis, but, on the contrary, the cytoplasm is damaged earlier and more severely. The cellular reaction in the anterior chamber is massive and intensive with predominance of macrophages.

[Penetration of gentamycin into the aqueous humor of non-inflamed and inflamed human eyes (author's transl)].

The penetration of gentamycin into aqueous humor after parenteral application (i. m., i. v., s. c.) of non-inflamed and inflamed human eyes was investigated. The dose of gentamycin was 80 mg i.m. and i. v. respectively. A doses of 40 mg gentamycin was used by s. c. injection. With Agar-diffusions test it could be demonstrated that the level of gentamycin was higher in inflamed than in non-inflamed eyes.

[Primary reconstruction of injured canaliculi (author's transl)].

Experiences of 163 patients with injuries of the canaliculi which were treated by different methods between 1960--1975 are presented. Of the 125 cases who were available for follow-up examinations, in 83.4% the injured canaliculus was patent. The advantages of retrograd introduction of the supporting silicon material is discussed. In more than 90% patency will be reached if microsurgical readaptation of the injured canaliculi by 3 direct sutures is done.
