Protein 1 (Clara cell protein) serum levels in lung cancer patients receiving chemotherapy.

As many antineoplastic drugs can cause injury to alveolar pneumocytes, Clara cells (nonciliated, nonmucous epithelial cells of the bronchioles) may also be damaged by such drugs. Protein 1 (P1) is an alpha-microprotein secreted by Clara cells. The effect of antineoplastic drugs on Clara cells is examined by the measurement of serum levels of P1 in patients with lung cancer receiving chemotherapy. Serum levels of P1 were studied in 36 patients with lung cancer, before chemotherapy and 5-7, 10-12 and 14-18 days after chemotherapy. One hundred and eight healthy subjects, matched by sex and age, acted as controls. There was no significant difference in P1 serum levels between patients with lung cancer and healthy controls. P1 serum levels decreased significantly 5-7 and 10-12 days after chemotherapy, recovering thereafter. We conclude that P1 serum levels do not differ between lung cancer patients and healthy controls, and that antineoplastic drugs inhibit the synthesis or secretion of P1 by Clara cells in the early period after the administration of medication.

[Intrathoracic transposition of the musculocutaneous flap in treating empyema].

A 62-year-old woman suffered right empyema caused by methicillin-resistant staphylococcus aureus (MRSA) which occurred following a pancreatico-duodenectomy. After open drainage thoracotomy, intrathoracic transposition of the extended musculocutaneous (MC) flap of the latissimus dorsi was performed. The patient is now in good health, without recurrence of either the empyema or the carcinoma, 19 months after the operation. The MC flap, compared to muscle flap, has the advantages that (1) larger empyema cavities can be obliterated, and (2) the deformity of the thoracic wall can be minimized, because of the small range of resected rib segments and well-preserved volume of subcutaneous tissue in the flap long after transposition.

[Congenital cystic adenomatoid malformation of the lung: a case report in a newborn infant with severe respiratory failure and a review of the literature].

The patient was newborn infant with severe respiratory failure. The left upper lobe of her lung was occupied by multiple small cysts and it was getting worse in her condition soon after birth. So left upper lobectomy was underwent urgently four days later. Then she was recovering her strength. Pathological examination of resected specimen revealed congenital cystic adenomatoid malformation (CCAM) of the lung (type II lesion of Stocker's classification). It is reported that about seventy percent of CCAM present between birth and one month of age. Its prognosis is poor without operation. The lesion is showed by Chest-abdominal X-ray and ultrasonography, and recognized to best advantage by computed tomography. Neonatal CCAM must be rapidly diagnosed and decided its indication of lobectomy, then its prognosis may be improved.

[A case of Von Recklinghausen's disease associated with a hemothorax due to a rapidly growing malignant schwannoma].

A 17-year-old man with Von Recklinghausen's disease was admitted to our hospital because of progressive dyspnea on exertion. A chest X-ray film showed a massive left pleural effusion. Thoracentesis revealed dark blood. Hemothorax was diagnosed, and digital subtraction angiography (DSA) was done. DSA revealed a large mass that was supplied with blood via the inferior phrenic artery, but revealed no active bleeding. An intercostal tube was inserted and 1700 ml of dark blood was drained. A chest X-ray film taken after drainage revealed a giant mass showing an extra-pleural sign in the left lung field. A thoracotomy was done to determine the source of bleeding. A giant tumor with hypervascular tissue was seen, and was diagnosed as a malignant schwannoma. No spontaneous hemorrhage from the artery was seen. The source of the hemothorax was believed to be hemorrhage from the tumor vessels. The tumor was treated conservatively, but it continued to grow rapidly and the patient died of respiratory failure. Five other patients have been reported to have hemothorax associated with Von Recklinghausen's disease. The sources of the hemothorax were reported to be hemorrhage from tumor vessels, or spontaneous rupture of the subclavian artery, an intercostal artery, or an intercostal vein.

[Maximal inspiratory and expiratory pressures long after lung resection].

Maximal inspiratory pressure (MIP) and maximal expiratory pressure (MEP) were measured long after lung resection with reference to operative procedure and vital capacity (VC). MIP and MEP were measured before surgery and 3 months after surgery in 80 patients who underwent lung resection. The procedures were pneumonectomy in 4 cases, lobectomy in 42 cases, segmentectomy in 3 cases, and wedge resection in 31 cases. Postoperative MIP, MEP, and VC were expressed and analyzed as percents of their respective preoperative values (MIP(%), MEP(%), VC(%)). Patients who underwent lobectomy had significantly lower MIP(%) and MEP(%) than did those who underwent wedge resection (p < 0.001). Both MIP(%) and MEP(%) correlated significantly with VC(%) (p < 0.001). Pulmonary resection can continue to affect both pulmonary function and respiratory muscle strength even 3 months after surgery.

[A case of metastatic lung tumor from acinic cell tumor of the parotid gland].

A 59-year-old man with metastatic lung tumor from acinic cell tumor of the parotid gland was reported. Five years after resection of the parotid tumor, the abnormal shadow on chest X-ray film was detected in the mass screening. The tumor was located in S10c of the right lung, and resected by partial lung resection. As pathological findings showed the same histology between the parotid gland and the lung, we diagnosed the metastatic lung tumor from acinic cell tumor of the parotid gland. Acinic cell tumor shall be slowly growing and occasionally developing the pulmonary metastasis. So, we have to carefully follow the patient in long-term periods.

[Long-term results of the surgical treatment for thymoma].

Long-term results of the surgical treatment for 27 cases of thymoma were studied. The clinical stages according to Masaoka's classification were: 7 cases of stage I, 9 cases of stage II, 8 cases of stage III, 1 case of stage IVa and 2 cases of stage IVb. The histological classifications were: epithelial cell type in 10 cases, lymphocytic type in 9 cases and mixed type in 8 cases. Complications consisted of 18 cases of myasthenia gravis (MG). The long-term survival at 5 and 10 years were as follows: 90.6%, 74.8% in all cases; 100.0%, 50.0% in the non-invasive type; 88.2%, 80.2% in the invasive type; 72.0%, 72.0% in the epithelial cell type; 100.0%, 75.0% in the lymphocytic type; 100.0%, 80.0% in the mixed type; 88.9%, 44.4% in cases not complicated with MG; 92.9%, 84.4% in cases complicated with MG. Complete resection and adjuvant therapy improved prognosis even in cases of invasive thymoma, but one case died due to local recurrence of thymoma at 10 years after operation, so we emphasize it is important to follow up for the long time period more than 10 years.

A study on the development of a filing system for funduscopic images with a personal computer for Twin AMHTS.

A filing system for ocular funduscopic image data was developed by using a personal computer for the Twin AMHTS. The development of the system was tried as one of the data transfer system including image data between two similar AMHTSs named the Twin AMHTS through the information network system. The filing system is capable of storing 26782 data of ophthalmoscopic pictures with a data compression mode by using a magneto-optical disk (MOD) whose storage capacity of both sides is 616 MB. It takes no long time for retrieval and display of the image data in the filing system. Good quality of compression and decompression obtained and reproducibility of the ocular fundus picture is favorable regardless of normal or abnormal cases. As a result, it is suggested that the developed system has practical utility although it requires more improvement.

Effect of dietary proteins on zinc bioavailability in pregnant rats.

In order to clarify the effects of dietary proteins on zinc bioavailability during pregnancy, two experiments were carried out. In Experiment 1, changes in zinc retention due to pregnancy (difference in retention between pregnant and nonpregnant animals) during early-mid and late pregnancy were examined in rats fed 10 and 20% egg white diets. Total amounts of retained zinc due to pregnancy were about 1000 micrograms or slightly more, equal to the zinc content in the products of conception at term. However, extra zinc retention during late pregnancy ranged between only 20 to 40% of overall retention, suggesting that almost all zinc retained during early-mid pregnancy moved from the mothers to the fetuses near term. Zinc retention in early-mid and late periods of pregnancy was higher in pregnant than nonpregnant rats, due mainly to increases in intake and bioavailability. In Experiment 2, to examine the effects of quality and quantity of dietary proteins, pregnant rats were fed either 10 or 20% egg white (EW), whole egg (WE), casein (C) and soy protein isolate with or without methionine (SM and S, respectively) diets. Total zinc retention during pregnancy was affected by both zinc and nitrogen intakes, though the former effect was greater than the latter. Because rats fed the EW diets retained dietary zinc efficiently, a relationship between zinc retention (Y, microgram/100 g BW/21 d.) and zinc intake (X, microgram/100 g BW/21 d.) was also examined in the non-EW protein groups, resulting in the following regression equation: Y = 0.471X-1790 (n: 51, r = 0.81, p < 0.001). Dietary protein quality affected the food intake resulting in different zinc intake and retention during pregnancy. Zinc from EW diets was more available than from the other four protein diets, because similar plots for rats fed the 10 and 20% EW diets fell above this line. Reasons for efficient bioavailability of zinc in EW were discussed in connection with the forms of zinc in diets and the pancreatic or intestinal responses to ingested EW.

An application of computational fluid mechanics to the air flow in an infant incubator.

An application of the computational fluid mechanical method to the air flow in a two-dimensional model of an infant incubator was described. The air flow in a numerical model was simulated and the Navier-Stokes equations were directly solved using a finite-volume method incorporating a body-fitted coordinate system on a mini-supercomputer. The model was based on a real infant incubator, slightly simplified for the sake of computing speed, and included a model of a baby. The number of computation grids was 101 x 61 = 6161. The calculation was carried out under the condition of unsteady, starting airflow and the results were examined by the means of color graphics animation. There were several very large scale eddies in the incubator free space, and their global structure did not show strong changes once they were established. Although the global structure did not change, small scale eddies were shown to be produced around the air inlet and convected down through the free space of the incubator. From these results, we believe that assuming steady and uniform flow in the incubator may not always be relevant when considering heat loss of a baby in an incubator. The steady and uniform flow has been previously assumed either implicitly or explicitly by most of the authors.

Analysis of the tcPO2 response to blood interruption in infants and adults.

The transcutaneous PO2 (tcPO2) response to blood interruption (BIS test) was measured in 6 healthy adults and 28 infants, including premature infants. The response was similar in both infants and adults with a time lag at the beginning of blood interruption, a subsequent linear decrease, a time lag at the end of blood interruption, and an exponential recovery to the final steady state. The linear decrease of tcPO2 in the BIS test had a correlation coefficient of more than 0.98 in all subjects and indicated the cutaneous oxygen consumption rate independent of the oxygen pressure. There was a significant difference in oxygen consumption according to birth weight in the infants (< 2 kg, 2-3 kg, and > 3 kg). The recovery time constant for infants was 13 seconds which was about 1.4 times faster than that for adults, and it increased significantly along with a higher birth weight. The time lag at blood interruption was longer than that at blood recovery. The time lag for infants was significantly shorter than that for adults, and in infants there was a significant difference between the group with a birth weight < 2 kg and the others (p < 0.005). All of the parameters of the BIS test suggested the physiological immaturity of premature infants.

On a portable memory device for physical activities and informations of maternal perception.

The condition of patients must be known to attending doctors for adequate management of a disease, particularly of high risk pregnancy. For this purpose, we have developed a portable computerized disease condition memory device to record the physical activities with maternal perception of fetal movement and uterine condition in daily life, both at home and during work. This device taken out by the patient is a small battery-driven CMOS 8 bit computer system (size: 107 x 80 x 30 mm, 240 g) and is equipped with push-botton switches on the upper side and a mercury switch inside it. The time of maternal perception of fetal movement and uterine contraction are recorded by the patient pressing the corresponding switch. Meanwhile the mercury switch serves as a acceleration sensor and the physical activities were measured by counting ON-OFF actions of the mercury switch caused by her movements. Consequently, the device has recorded physical activities automatically by wearing this unit all day long. The continuously recordable time is more than two weeks. The evaluation about the sensitivity of physical activity measurement has indicated that the mercury switch sensor was well related to the oxygen consumption rate in rest and mild exercise. Using this device to five pregnant women, the data showed the quantitative difference in physical activities between rest in bed and normal home life, and daily changes could be clearly observed. From these results, the physical activities and the condition of the patient in daily life can be followed by this device.
