[The structure of the spongy bone in lumbar vertebrae and the neck of the femur. A comparative analysis of the age-dependent remodelling process (author's transl)].

In 114 autopsy cases of both sexes, aged between 31 and 97 years and without bone disease, the influence of mechanical forces on the age-dependent remodelling of the spongy bone was determined in the 3rd and 5th lumbar vertebrae and the neck of the femur. For this purpose contrasty X-ray images of 100 mu thick polished bone sections were analysed using the LEITZ texture analysis system. The volumetric density and the surface density are highest in the neck of the femur, lowest in the 3rd lumbar vertebra, and almost as low in the 5th lumbar vertebra. The volumetric density decreases with increasing age by about one-third in all three bones. Correspondingly, the surface density also decreases in the lumbar vertebrae by one-third, but only by 18% in the neck of the femur. The specific surface reveals no age differences in the lumbar vertebrae, but increases by 19% in the neck of the femur. The constancy of the specific surface in the lumbar vertebrae can be explained by the fact that compensatory hypertrophy of the remaining trabeculae takes place. This compensatory growth of bone does not occur in the neck of the femur. The age-dependent decrease in spongy bone manifests itself in a residual spongiosa structure that depends on compressive forces in the case of the lumbar vertebrae and bending forces in the neck of the femur.

[Changes in form and structure of vertebrae as a manifestation of mechanical loading (author's transl)].

In 240 autopsy cases of both sexes, aged between 19 and 88 years and with no evidence of clinically manifest bone disease, the structure of the spongy bone and the external form of the 3rd and 5th lumbar vertebrae were investigated quantitatively and morphometrically on the basis of high-contrast X-ray images of 100 mu-thick polished bone sections, using the Leitz texture analysis system. The structure of the spongy bone was analysed on the basis of the parameters volumetric density, surface density and specific surface in horizontal "fifths", the form being determined by measuring the greatest and smallest heights and widths. In the individual "fifths", the structural parameters manifest a "stepped" distribution, the highest values being in the outer, the lowest values in the middle "fifths". With increasing age, this distribution pattern flattens to differing degrees so that, in old age, only the outer"fifths"differ from the middle and inner"fifths". The form parameters of the vertebrae reveal an age-independent constant heigh and an age-dependent increase in maximum width. The vertebral bodies are subjected mainly to compressive forces and represent a compound construction of spongy bone and fluid-filled medullary cells. The inhomogeneity of the vertebral structure is the constructional equivalent of the intravertebrally regionally differing mechanical forces. Also the structure of the residual spongy bone resulting from the age-dependent remodelling process, is oriented to these mechanical forces. Despite the considerable intravertebral remodelling of structure, the form of the vertebral bodies remains largely constant. The increase in maximum width is based on an increase in the tensile forces acting upon the outer edges of the vertebral bodies consequent upon degenerative intervertebral disc changes.

[Treatment of osteogenesis imperfecta with (+)-catechin (author's transl)].

4 girls aged 4--12 years with various forms of osteogenesis imperfecta were treated with (+)-catechin for several months. Bone punch biopsies from the iliac crest were investigated by electron microscopy before and during treatment. The frequency of fractures decreased clinically. There were no radiographic changes in the bones and there were no side effects. Electron microsopy showed a dilated coarse endoplasmatic reticulum with infrequent ribosomes, thin collagen fibrils and decreased predominantly disseminated mineralisation before treatment. Under the influence of treatment electron microscopical aspects of the bone improved. The cisternae of the endoplasmatic reticulum were arranged parallel with densely packed ribosomes, collagen fibrils were wider and in closely packed bundles and mineralisation was clearly improved. The electron microscopic findings are evidence for a possibly beneficial influence of (+)-catechin in some cases of osteogenesis imperfecta.

[Endoscopic needle biopsy in larynx and pharynx (author's transl)].

The needle-biopsy taken in the pharynx and larynx is a simple and reliable procedure for the histological diagnosis of submucosal tumours. It can be performed either in local anaesthesia using the zoom-endoscope by v. Stuckrad or during microlaryngoscopy under general anaesthesie. This method however does neither replace the single biopsy of exophtic tumours nor the microlaryngoscopic excision of suspicious, precancerous lesions.

[Comparative studies of various suture techniques in transthoracic esophagogastric anastomoses in dogs (author's transl)].

In 27 dogs esophagogastric anastomoses were performed in a single-layered end-on, a three-layered end-on and a double-layered inverting technique. The anastomoses were reinvestigated on the 2., 4., 7., 15. and 30. postoperative day. Suture line leakages were not observed. The inverted suture resulted in considerable anastomotic stenosis, ulceration of the mucosa, disordered and initially incomplete revascularisation, and widespread granulomatous inflammatory reaction. In contrast, the end-on technique led to almost no stenosis, ordered revascularisation, and narrow and delicate scar tissue formation while maintaining a normally layered structure. The single-layered methode was superior to the three-layered one. The best results were obtained by the extramucous technique.

[Colonic anastomoses. Mechanical and morphological findings].

In healthy mongrel dogs 45 colonic anastomoses were performed in one--and two layered inverting and single layered end-on technique. They were compared 4, 8, and 18 days after surgery macroscopically, histologically and by tension testing. All criteria showed the single layered end-on technique to be superior. The principle of Lembert, aiming for contact of large areas of serosa, should be replaced by a technique resulting in exact reconstruction of intestinal wall without mucosal damage.

[Is the prophylactic hemithyreoidectomy indicated in radical neck dissection? (author's transl)].

To find an explanation for tumor-recurrences around the tracheostoma, in 59 cases of squamous cell cancer of larynx, hypopharynx, oropharynx and tongue the ipsilateral hemithyreoidectomy was included into the tumorexstirpation and radical neck dissection. This histological work up of the thyroid showed no evidence of metastases within the gland. However in cases of extended metastasis or difficult surgical approach to the prae- and paratracheal areas the hemithyreoidectomy is advised.

[Histomorphology and resorption of longtime-implanted cartilage chips conserved in merthiolat in rhinoplasty (author's transl)].

A histological study is presented on 18 cartilage grafts, conserved in Merthiolat TM, which had been implanted for reconstruction of the caudal border of the nose for time periods of 1 month up to 11 years. These cartilage graft preparations were compared to non graft cartilage from patients died in an age of 37, 51 and 53 years respectively. Our findings show that cartilage grafts are eroded from the edge by resorption granulations of the host. Besides resorption one case showed purulent chondritis and another one osteoneogenesis. The extent of the erosion and the resorption rate seem to depend strongly on the original quality of the cartilage graft. This is possibly due to degenerative focuses which could be found in all of the natural cartilage preparations. After our opinion these focuses regarded like "loci minoris resistentiae". Therefore cartilage material to be used for rhinoplasty ought to be checked for such degenerative focuses.

[Dura transplantation. Multi-sequential transplants of solvent dehydrated dura mater. Animal experiment studies on the question of sensitization].

After the excision of 7 X 5 mm abdominal muscle sections in an experiment using rats, a total of 4 xenogenic, solvent dehydrated dura mater of the same size were implanted at 4 week intervals each. The transplant areas were continuously examined under a light and electron microscope for a period ranging from 7 days up to 3 months after the last transplant. Spontaneous layers of absorbent granulation tissue, rich in cells and blood vessels, surrounded the grafts in the beginning. Subsequently the grafts were gradually decomposed from the periphery to the center through macrophages like biological foreign matter and replaced with endogenous, poorly vascularized, collagenous connective tissue. The tissue reaction to the multi-sequential, xenogenic grafts was consistently the same as after single transplants. Immuno-competent cells do not increase. The solvent preserved dura mater is suited for multisequential transplants due to lacking sensitization, i.e. immunological rejection reaction.

[Animal experimental studies on the healing around ceramic implantation in bone lesions in the maxillary sinus region].

Clinical and histological tests were made in five minipigs to study the hitherto unknown tissue behaviour of the receiver organism to unilaterally uncovered perforated ceramic implants in the bilaterally osteotomized facial maxillary sinus wall. The implants were covered with connective tissue and coated with respiratory mucosa on the uncovered side. After 11 weeks, they had all healed without irritation, and were solidly anchored in the surrounding tissue by the connective tissue which had grown into the perforations.
