[The solvent-dried dura mater. A new dura transplant in the animal experiment].

After the excision of 7 X 5 mm abdominal muscle sections in an experiment using rats, rehydrated, solvent dried dura grafts of the same size were implanted. After three hours the tissue reaction of the transplant areas were continuously examined under a light and electron microscope for 14 months. A gradual graft decomposition advancing in a coating form from the periphery to the center and a simultaneous replacement with collagenous connective tissue of the body resulted due to an absorbent granulation tissue rich in cells and blood vessels after the initial infiltration of neutrophil granular leukocytes caused by the operation. A fibrous-rich connective tissue plate, in part forming two layers and thereby serving as a reminder of the former dura graft is decomposed in the abdominal muscles of the rats without infection or rejection and replaced with endogenous connective tissue. The duration of this decomposition and transformation process is contingent on the size of the graft and the tissue reactivity in the surrounding transplant area.

[Meatoplasty with allogenic preserved cartilage. Histomorphological model in animal experiments (author's transl)].

The reconstruction of the posterior wall of the external acoustic meatus destroyed by surgery or a cholesteatoma is necessary for the complete restoration of function. For this purpose, allogen cartilage preserved in thiomersalate has been transplanted on the anterior wall of the air-containing mastoid bone in 160 clinical cases. The histomorphological reaction of the recipient to unilaterally uncovered transplant material has previously been unknown. In 10 guinea pigs, 15 fenestrated bullae (hypotympanum equivalents) were closed with cartilage transplants fixed in place with tissue adhesive. After a period varying between 7 1/2 and 11 months, the walls of the bullae with the transplant were then examined histomorphologically. Despite the variations in the period in situ, the avital cartilage transplant, which remained largely constant in volume and shape, was always to be found. It was tolerated without reaction and was "enveloped"--also on the primarily uncovered surface--by a capsule of bone and connective tissue. In no case was rejection observed. Marked foreign-body reaction was, however, found in the vicinity of the tissue adhesive, which was absorbed only slowly. The clinically well-known tissue-compatible and complication free behaviour of the allogenic cartilage transplant preserved in thiomersalate, is thus explained by the histomorphological findings. In contrast, the fact that marked foreign-body reaction was demonstrable even after almost one year, suggests that tissue adhesive should not be used.

[Endoscopic-histological differential diagnosis of rare benign endolaryngeal proliferations (author's transl)].

In the period from 1961 to August, 1975, 2,222 microlaryngoscopic examinations were carried out at the ENT Department of the University. A correlation was seen between the increase in the number of examination in the last few years and the increase in the number of malignant tumours of the larynx, while malignant growths in the hypopharynx were found to be only slightly increased. An anlysis of 1,027 microlaryngoscopies carried out under anaesthesia over the last 2 2/3 years showed almost 50% cancers and precancers. In 5% of the case at most more rare laryngeal findings were also seen. Of these, the granular cell tumour, plasmocytoma, oncocytic cystadenoma and amyloid-tumour of the larynx, but also tuberculous laryngitis, are represented. The modern endoscopic examinations and the fundamental bioptic histomorphological examination of proliferations allow an exact differentiation of these changes in the tissues of the endolarynx. In this connection, a valuable technical aid is the v. Stuckard low-power magnifying laryngoscope, that has been in use for the past six months.

[Comparative clinical, histomorphological and immunological investigations into etiological aspects of polyposis nasi et sinuum (author's transl)].

43 patients with polyps affecting the nose and the sinuses were examined for immunological deficiencies on the basis of clinical, histomorphological and immunological findings. Constant histological findings were a chronic non-specific inflammation of varying intensity; immune-histologically a varyingly dense collection of Ig-producing cells with intracellular and extracellular immunoglobulins was observed. A determination of the immunoglobulins in the serum allowed no conclusion to be drawn as to the local condition of the mucosa. Hypertensitivity reactions played no role as a causal pathogenetic factor in the development of polyposis. In a third of the cases, however, polypi led to a secondary bronchial asthma. After the surgical removal of the polypi from the nose and paranasal sinuses, not only an improvement in nasal breathing and in the aeration of the sinuses was regularly achieved, but obstructive disorders of ventilation also disappeared.

[Effect of carbohydrate infusions on the formation of calcium oxalate deposits in the kidney: postmortem morphological and biochemical findings in humans and in animal experiments].

Histological and biochemical studies were carried out in a total of 300 patients who had died in the recovery room, and in rabbits, to investigate the frequency of deposits of calcium oxalte crystals in the kidneys, the influence of infusion therapy and the pathological significance of such deposits on the kidney tissue and on renal function. - Quite independent of any infusions, however, deposits of calcium oxalate crystals were found in the presence of kidney-specific diseases, in particular uraemia and anuric conditions. Xylitol infusions of 0.4 g/kg body weight or, in individual cases, of not more than 500 g total in 7 days, had no infllence on the appearance of calcium oxalate deposits. The blockage of the tubular system by the calcium oxalate deposits leads to a temporary reversible increase in serum urea and serum creatinine. With time, and uninfluenced by infusions, the deposits disappear out of the kidney again without having caused any organic renal damage. In the presence of a temporary excess of serum oxalate, the kidneys temporarily act like a cloaca.

[On the individualized, combined therapy of Ewing's sarcoma (author's transl)].

With eleven patients, for whom in the period 1967-74 a diagnosis of Ewing's sarcoma was confirmed, it was shown how by intensifying the chemo- and radiotherapy the survival time of some patients could be prolonged. At the same time surgical therapy has become less important. The diagnostical difficulties of the histomorphological differentiation of Ewing's sarcoma from lymphosarcoma are detailed on the example of another patient. A further general intensification of the chemo- and radiotherapy is not possible as the organism's limit of acute tolerance is already reached. Any future improvement on the results of treatment seems therefore only possible if combination therapy is modified to correspond to the particular needs of the individual patient.

Disaccharidases in rat small intestinal mucosa following cytostatic treatment with adriamycin.

Our studies have shown that appropriate doses of adriamycin can reduce the activities of the intestinal disaccharidases in the rat. At higher doses, in addition to morphological changes in the intestinal mucosa, a considerable fall in disaccharidase activities also occurs. This functional impairment can be seen for a period of several days after the morphological restitution of the mucosa. Appropriate studies of the small intestinal mucosa in man will have to be undertaken to show whether similar impairment occurs at therapeutic doses.

[Clinical and experimental studies on the transplantation of preserved tendons].

The tensile strength of preserved tendons was studied in relation with duration and preserving process. Cialite preserved cow-tendons were implanted in rear limbs of rabbit. After 5, 7 and 9 months the transplants could still be identified. They were covered by unreactive fibrous connective tissue, where by adjustant to the connective tissue the transplant was replaced by multicellular growing tissue. In clinical practice preserved tendons were successfully used for ligament-plastic. It is reported on the possibility of use and the results.
