Periosteal chondroma: a radiographic spectrum.

The radiographic features of 22 cases of histologically proven periosteal chondroma are analyzed in detail. The typical tumor consisted in an area of outer cortex remodeling (scalloping) involving the metaphyseal lesion of a long bone with slightly overhanging edges and a small amount of cartilage calcified matrix adjacent to the scalloping. The presence of a visible soft tissue mass was uncommon, present in only third of the cases. The radiographic differential diagnosis is analyzed in detail and illustrated.

Florid reactive periostitis of the tubular bones of the hands and feet. A benign lesion which may simulate osteosarcoma.

Twelve patients with florid reactive periostitis of the small bones of the hands and feet are presented. Clinically, the lesion usually presents as a swelling of the affected part, sometimes accompanied by pain, tenderness, and redness. Histologically, the lesion may be mistaken for benign and malignant neoplasms such as osteosarcoma or parosteal osteosarcoma or osteochondromas. The features that distinguish reactive periostitis from the several diagnostic possibilities are presented. It is important for the clinician and the pathologist to recognize the existence of florid reactive periostitis in the hands and feet and to treat accordingly. Local excision appears to be adequate treatment; follow-up indicates little risk for local recurrence.

Image analysis of atypical bronchial squamous epithelial cells. A quantitative and qualitative examination of squamous cell carcinogenesis.

Atypical bronchial squamous epithelial cells from cigarette smokers were digitized using a high-resolution microphotometric scanning system operating at a wavelength of 530 nm. All cells used for the purpose of developing a training set as well as those used as a test set were classified by a team of three cytopathologists. Reduction in the dimensionality of the feature space and cell classification were performed using a new system known as the atypia status index (ASI); this dual-purpose system will specifically classify cells into five distinct classes of progressive atypia as well as quantitate the degree of atypicality of each individual cell. The distribution of cellular ASI values was arranged from lowest to highest and plotted for each subject as a profile. The ASI range has well-defined intervals, representing the five different atypia classes used in the study (squamous metaplasia, mild atypia, moderate atypia, severe atypia and carcinoma). Subjects at different stages of carcinogenesis were found to have significantly different profiles (P less than 0.01). Changes in visual morphologic characteristics and their relation to changes in ASI values for the five different classes of atypia are presented.

Ultrastructural analysis of human penile corpus cavernosum. Its significance in tumescence and detumescence.

The erectile tissue from 15 normal and impotent male patients was studied by electron microscopy to determine if any ultrastructural features could contribute to the basic understanding of the physiology of penile erection and detumescence. It was found that the endothelium lining the cavernous lacunae contained both contractile elements and Weibel-Palade bodies which have a possible role in vasoconstriction. Within the trabeculae of the cavernous body there was an abundance of elastin as well as oxytalan and elaunin fibers around bundles of smooth muscle. With these elements affording an anchorage system for contraction of smooth muscles within the trabeculae, an alternate contraction of smooth muscles and endothelium could account for the erectile and detumescent states.

Hyperplastic polyps of the large bowel.

One hundred seventy-one hyperplastic colonic polyps were examined and found to be less homogenous histologically than has been generally considered. For example, 13% exhibited adenomatous changes, 7.6% contained Paneth cells, and 3% had distinct stalks. They are at times pedunculated and relatively large; some cannot be distinguished grossly from small sessile polyps of other histological patterns and removal is required for proper identification. Other epithelial polyps of equivalent size were compared with the hyperplastic polyps to determine the frequency of the various types. Hyperplastic polyps comprised 43.5% of all surgically removed epithelial polyps of the colon measuring 5 mm or less.

Aytpia status index of respiratory cells: a measurement for the detection and monitoring of neoplastic changes in squamous cell carcinogenesis.

A statistical procedure was developed for calculating an atypia status index (ASI) for cells from the bronchial mucosa. These indices represent the degree of abnormal changes in these cells and classify them as squamous metaplasia, mild atypia, moderate atypia, servere atypia or carcinoma. The classification accuracy of the procedure was more than 99% on trained data and was accomplished by minimizing the overlapping areas between the adjacent multivariate distributions of cell groups for selected features whose group means represent a monotonic function and for which the cell categories are distinguishable. The calculated ASI may reflect abnormal changes in the cell that may not be clearly evident visually. It appears that progression or reversal of bronchial epithelial atypia can be accurately monitored by studying the changes in the ASI, not only for preneoplasia but also for reactions to chemotherapy and various pulmonary infectious disease processes, such as influenza.

The Clara cell: a comparative ultrastructural study in mammals.

Clara cells in the terminal bronchoiles of mouse, rat, rabbit, calf and human were compared by light, transmission and scanning microscopy, and species-differences were clearly present. Mouse Clara cells were most numerous and mouse and rabbit Clara cells had large dense mitochondria. Rabbit and calf had glycogen in Clara cells and rat Clara cells had the most variability in secretory granules, some of which had a crystalline structure. Calf Clara cells had deeply indented nuclei. Human Clara cells had the most prominent nucleoli and lacked smooth endoplasmic reticulum, which was a prominent feature of most other species. No evidence of apical extrusion or apocrine secretion of Clara cell secretory granules was observed.

Epithelial polyps of the large bowel: a pathological and colonoscopic study.

Ninety patients who had epithelial polyps or carcinoma of the large bowel and underwent at least 2 colonscopic examinations were studied. The cases were divided into three groups: patients who ;ad only one polyp; those in whom more than one epithelial polyp were found and those who had adenocarcinoma of the large bowel and underwent follow-up colonoscopic examinations. The distribution of the polpys was plotted as to type and distribution in the colon. There seemed to be a trend for the polyp found in follow-up examinations to be located proximal to the splenic flexure. In this regard it was noted that the subsequently found polyps in group II were smaller than those removed on the initial colonoscopic examination. No relationship could be found between any histologic type of epithelial polyp and the presence of carcinoma.

Ultrastructure of endometrial stromal foam cells.

An endometrium with cystic-glandular hyperplasia contained large foam cells in the stroma. The majority had ultrastructural characteristics of endometrial stromal cells, although a few were interpreted as macrophages. Endometrial stromal cells that appeared normal by light microscopy contained varying quantities of lipid on ultrastructural examination, and differed from the predominate type of foam cell only by having less cytoplasmic volume and maintaining an elongated shape. Cells intermediate in size and shape between normal stromal cells and foam cells further substantiated the origin of most of the foam cells from endometrial stroma.

The small carcinoma of the large bowel.

Four small carcinomas (3, 6, 10, and 15 mm) of the large intestine have been examined for residual epithelial polyp; none was found. It is felt that these carcinomas arose from colonic mucosa without a preexisting visible epithelial precursor lesion; however, occasional glands having adenomatous change were seen near the edges of the carcinomas.

The radiographic spectrum of periosteal osteosarcoma.

Fifteen cases of a recently described variety of osteosarcoma, periosteal osteosarcoma, were studied to define their radiographic characteristics. All the lesions were limited to the cortex that was always thickened but without invasion of the medulla cavity. The tumors presented as non-homogeneous masses of spiculated osteoid matrix progressively denser from the periphery to their cortical base. The average age was 28 and the most common location was the proximal third of the femur; all the lesions were diaphyseal in location. The spectrum of radiographic features and differential diagnosis are described in detail.
