[Effect of kidney function on the elimination and action of colloidal plasma substitutes].

Hypovolemia and prophylaxis of thrombosis are the main indications for colloidal plasma substitutes. This is also true for patients with renal failure. Due to their difference in molecular structure colloidal plasma substitutes, which are in clinical use--dextran, hydroxyethyl starch, gelatin--, are eliminated tbrough different pathways. However, the kidney plays an important role in the elimination of all three substances. Therefore pharmacokinetics and pharmacodynamics of colloidal plasma substitutes, which are based on dextran, hydroxyethyl starch and gelatin, were studied in patients with normal or impaired renal function. Renal insufficiency proved to affect the elimination and therapeutic efficacy of these colloids in different and characteristic ways. Consequently this requires special indications and dosage recommendations for patients with renal insufficiency.

DNA repair in Proteus mirabilis. VI. Plasmid (R46-) mediated recovery and UV mutagenesis.

The expression of plasmid R46-mediated recovery and mutagenic function (s) was studied in P. mirabilis, which is normally either weakly or non-mutable after UV exposure. The plasmid was found to confer on P. mirabilis enhanced UV resistance as well as UV-induced mutability for various types of forward mutations and reversion of the thr273 mutation. The plasmid enhanced survival of UV-irradiated phages in P. mirabilis both in unirradiated host cells and with increased efficiency after UV-exposure of host cells, as is characteristic of UV-inducible phage reactivation. Spontaneous mutability of P. mirabilis harboring R46 was about 2 to 7 times higher than that of cells without plasmid, depending on the marker, repair type, and plating density of the cells used. All of these R46-mediated rescue and mutagenic functions require the rec672 + gene function. It is assumed that the plasmid R46 adds functions to P. mirabilis comparable to those deficient in umuC and uvm mutants of E. coli (Kato and Shinoura, 1977; Steinborn, 1978) and that P. mirabilis possesses functions homologous to those controlled in E. coli by the recA + and lexA + genes. The significance of plasmid-mediated rescue and mutagenic functions for bacteria which lack the misrepair branch of mutagenesis, is discussed.

[Quantitative measurement of arterial flow by means of directional Doppler technic in hemodynamic animal experiments].

The usability of the continuous ultrasonic Doppler-method for the measurement of the blood flow in the haemodynamic animal experiment was tested. For this purpose measurements at the ascending aorta and the commune carotid artery were performed on different current conditions and compared with the electromagnetically registered blood flow. The causes for the non-conformity are discussed.

Effect of intravenous diazepam on human lower oesophageal sphincter pressure under controlled double blind crossover conditions.

The effect of diazepam on the lower oesophageal sphincter (LOS) pressure is controversial. Therefore, a double-blind crossover study was performed on 18 healthy volunteers to determine the sphincter response to intravenous diazepam--70, 140, 280 microgram/kg, which correspond to a total dose of 5, 10, and 20 mg. respectively. After the 5 and 10 mg dose no signficant effect on LOS pressure could be observed when compared with placebo. After the 20 mg dose a significant rise in pressures (deltaPLOS) was recorded for 40 minutes with a maximum deltaPLOS of + 16.2 +/- 6.6 (mean +/- SEM) mmHg after 50 minutes (P less than 0.01) (46 +/- 1.3% increase above the basal pressure). It is concluded that diazepam does not affect lower oesophageal sphincter competence and therefore does not increase the risk of regurgitation and pulmonary aspiration in premedicated patients.

Comparative evaluation of immunofluorescent (IgM and IgG) and complement-fixing antibodies in influenza A infection.

The indirect immunofluorescent (FA) tests for detection of IgG and IgM antibodies to the internal antigens of influenza viruses were performed using single cell suspensions of baby hamster kidney cells. The IgG-FA test showed a good correlation with the complement fixation (CF) test performed with purified ribonucleoprotein (RNP) as antigen than the CF test with whole virus antigen. Sera which were positive only in the IgM-FA test did not react in the RNP-CF test. A rapid diagnosis of influenza was not possible alone by detection of RNP specific IgM as relatively low titers were obtained in the IgM-FA test.

[Value of the electrophoresis mobility test in the differential diagnosis of obscure kidney findings].

The differential diagnosis of unclear carcinoma-suspicious renal findings often finishes with the test-exposition and nephrectomy. Also when all modern diagnostic possibilities are used not always a clear establishment of the diagnosis may be achieved. The cell electrophoresis mobility test after Field and Caspary may contribute to the differential-diagnostic clarification of the findings, when adequate antigens are used. For this purpose 10 own cases are discussed and these results are compared with suitable comparative groups. Summarizingly by the present study can be shown that the immunologic diagnostics of tumours in the field of unclear renal tumours may be a very good help in the diagnostics. When adequate antigens are used in the cell electrophoresis mobility test a coordination concerning the histospecificity can be achieved.

[Demonstration of antibodies against HBs antigen with the modified MICROELISA-hepanostika-test (author's transl)].

Among various modifications of the Hepanostika test a simple, sensitive and reproducible one is described as being the best. Of 689 sera from patients in a haemodialysis unit 110 had antibodies against HBs antigen in the Hepanostika test and 150 in the Ausab test. Those antibodies which were not demonstrated in the Hepanostika test had a relatively low concentration (mean impulses per minutes 4.3 X NCx). Those sera which were positive in both the Hepanostika and Ausab test contained antibodies in higher concentrations (mean impulse/min 29.5 X NCx). Only 11 of the 689 sera were positive in the migration-electrophoresis test. A particularly advantage of the modified Hepanostika test is that both HBs antigen and antibodies against HBs antigen can be demonstrated simultaneously on the same plate. Using this method 200 sera were examined. The same sera were tested in the Austria-II-125 test for HBs antigen and the Ausab test for antibodies against HBs antigen. The Hepanostika test was positive in five, the Austria test in four sera. According to the Hepanostika test 43 sera contained antibodies against HBs antigen, while 45 did according to the Ausab test. The two that were positive only in the Ausab test were only weakly positive.

[Second increase in plasma volume after single infusion of hydroxyethyl starch (author's transl)].

6 patients without evidence for renal, hepatic or pancreatic disease were treated with intravenous infusions of 500 ml hydroxyethyl starch (6%) over a period of 60 min. In the course of the infusion we observed an increase in plasma volume from 2.72 +/- 0.101 to 3.36 +/- 0.141. After 2 h plasma volume decreased to 3.02 +/- 0.101 but showed a second peak of 3.23 +/- 0.121 after 4 h (p less than 0.01). 24 h following infusion an increase in plasma volume of 4,8% was found as compared to preinfusion values. The second increase in plasma volume cannot be explained by the total concentration of hydroxyethyl starch since the latter decreased continuously. The increase in plasma volume was accompanied by a decrease in average molecular weight (-Mw and -Mn). It is suggested that serum amylase produces small osmotic active molecules by degradation of hydroxyethyl starch, thus leading to an increase in plasma volume. 12--24 h after the infusion of hydroxyethyl starch serum amylase was more than twice as high basal values. This is caused by the formation of a high molecular hydroxyethyl starch-amylase-complex which cannot be eliminated easily. When hydroxyethyl starch is given repeatedly to normovolemic patients, the second increase in plasma volume should be considered as a possible cause for acute hypervolemia. This is especially true for patients with myocardial insufficiency.
