[State and developmental trends of urogenital tuberculosis--20 years clinical and pathologic-anatomical experiences].

On the basis of own experiences of twenty years and taking into consideration the knowledge of other authors the developmental tendency in the treatment of the urogenital tuberculosis is demonstrated, the indications to nephrectomy, resection of the kidney, and epididymectomy are reified as well as kind and sequelae of errors in chemotherapy analysed. From 1958 to 1977 2,868 patients with urogenital tuberculosis were hospitalized in the Central Clinic Bad Berka. Of them about 41% had to undergo an operation. A decrease of the quantity of operations can be observed only in the renal resections, whereas the nephrectomy rate is unchangedly high (16.3% of the own patients) and even shows an increasing tendency since 1975. Comparative examinations show that the pathologo-anatomical picture of the ectomized kidneys has not changed during the period of observation. The knowledge of the symptom-poor course obliges also further on differential-diagnostically to take into consideration the urogenital tuberculosis. Only on this way the still large number of organ endangering complications may be encountered in time.

[New methods of chemotherapy also for extrapulmonary tuberculosis?].

The chemotherapy of all forms of tuberculosis is basically performed according to homogeneous principles. Different opinions exist within the extrapulmonary forms particularly with regard to the duration of the treatment. Here, the so-called long-term therapy is frequently still postulated with 18--24 months. But newly developed, highly effective antituberculotic medicaments do not only allow but actually demand a shortening of the time of treatment in interest of the patient. Apparantly rifampicin and also pyrazin amide are able to effect on the germs in rest -- the persisters. Up to now the last mentioned gave the actual rise for the long-term application. Under the notion short-term therapy on the one hand by the application of the preparations mentioned and on the other hand basing on the experiences in the medicamentous treatment of the pulmonary tuberculosis now in the same measure the possiblity of a total duration of the therapy of on an average twelve months develops for the extrapulmonary manifestations of tuberculosis. Here, the two-phase regime is preferred with a continuous application of medicaments lasting for three months and following controlled phase with intermitting application of antituberculotics twice a week. The duration of application for rifampicin in these cases should neither trandgress nor be below three months.

Application of teem-test for investigations of cellular immune phenomena in multiple sclerosis.

Some evidence is presented for usableness of tanned erythrocytes in electrophoretic mobility (TEEM)-test. Two possibilities of application of TEEM-test for immunological investigation in multiple sclerosis are discussed: detection of lymphocyte sensitization against a soluble antigen (3 M KCl extracted) derived from a normal brain and measurement of mixed lymphocyte reactin (MLR) after a short-time lymphocyte culture. Results show some connection between autosensitization (TEEM-test using normal tissue antigen) and predisposition (TEEM-MLR-test).

A contribution to immunological tumour diagnostics in urology.

Fifty-four patients with malignant and other diseases of the urinary bladder and the prostate were examined for lymphocyte sensitization by means of electrophoretic mobility test (EM test). In this antigens from brain tissue (EF) and from malignomas of kidney, urinary bladder and prostate were used. Positive HEP values were obtained in 40 patients out of 42 with malignant tumours and in 5 patients out of 12 with other kidney diseases. With the corresponding tumour-associated antigens (TAA) in the EM-test, the histological tumour diagnosis could be confirmed in every case. Positive findings after using EF as well as the corresponding TAA are with great probability an indication of a malignant disease.

Adenocarcinoma

Clonal dominance: loss and restoration in adoptive transfer.

An adoptive transfer system was used to study the mechanism responsible for clonal dominance of the anti-phosphorylcholine response in BALB/c mice. The adult spleen contains phosphorylcholine-specific precursor cells that are capable of developing into antibody-producing cells after transfer into lethally irradiated animals. The neonatal liver of the BALB/c mouse lacks precursor cells specific for phosphorylcholine but contains immature cells that differentiate into specific precursors during the normal course of ontogeny. The transfer of fetal or neonatal liver cells into lethally irradiated recipients prevents the appearance of the dominant H8 clone which constitutes the majority of the clones responding to phosphorylcholine in adult BALB/c mice. However, if these cells are transferred into neonatally suppressed recipients that lack the H8 idiotype, dominance of the H8 clone can develop. The conversion of the committed immature progenitor cell into a responsive B lymphocyte precursor is a regulated event. Regulation at the level of progenitor cells determines the eventual clonal profile of the immune response to phosphorylcholine. It is suggested that selection of the dominant clone occurs at this level.

Treatment with essential amino acids in patients on chronic hemodialysis: a double blind cross-over study.

Patients on chronic hemodialysis may suffer from a latent protein deficiency, and therapy with essential amino acids has been recommended. In a double blind cross-over study, 13 hemodialysis patients received orally 15.7 g of essential amino acids daily over a 3-month period. Patients were on a liberal diet, containing 1 g of protein per kilogram of body weight per day. Hemodialysis was adequate. Therapy resulted in an increase in urea, uric acid, C3 c complement factor and a fall in C4. Lysine levels increased and phenylalanine fell. Malnutrition could not account for the observed metabolic changes, which are more likely due to uremic metabolic disturbances. A liberal diet of 1 g of protein per kilogram of body weight appears sufficient for patients on hemodialysis. Treatment with essential amino acids offers no advantage.

[Results of islet transplantation in diabetic dogs].

The influence of histocompatibility on islet transplantation in dogs was studied. Donors and hosts were chosen by help of the macrophage-electrophoresis-mobility test. By subtotal pancreatectomy and twice applying streptocotocin diabetes was successfully induced in recipient animals. Isolation of pancreating islets from the donor was done by means of the collagenase method combined with the Ficoll-gradient-separating technique. The grafting occurred via portal vein into the liver. Significant decrease of blood sugar during 10 weeks was observed in animals with good compatibility. This effect lasted only 14 days in cases of bad compatibility. Concentrations of insulin in the portal vein and superior caval vein 4 weeks after grafting was lower in cases with high-grade histoincompatibility. The duration of functioning of transplanted islets depends on the histocompatibility.

Studies on column fractionation of immune cells. VII. Electrophoretic mobility and functional behavior of mouse spleen cells after fractionation on a weakly basic ion exchanger.

After passing normal mouse spleen cells through columns of a weakly basic ion-exchanger, the percentage of cells with B cell-typical electrophoretic mobility (EPM) is 90% after fractionation at pH 8.0 and 10--20% at pH 5.0. However, the characterization of fractionated cells with regard to theta-antigen as well as graft-versus-host reaction shows that separation of T- and B-lymphocytes had not occurred. After incubation of the separated cells for 2 h at 37 degrees C the EPM returned to that of unseparated cells. It appears that the net charge of T- and B-lymphocytes is changeable depending on pH, temperature and separation material.
