Clinical significance of serum and CSF findings in the Guillain-Barré syndrome and related disorders.

Clinical data and the serum and cerebrospinal fluid (CSF) findings of 71 patients with Guillain-Barré syndrome (GBS), 7 with Fisher syndrome and 24 with chronic inflammatory polyradiculoneuropathy (CIP), were analysed. Isoelectric focusing of serum and CSF together with different formulae and diagrams were applied to study blood-CSF barrier (BCB) function and possible intrathecal IgG synthesis. The CSF total protein concentration and its IgG percentage depended mainly on the degree of BCB damage, which correlated with the clinical course. Our investigations suggest that oligoclonal IgG of CSF from these patients comes essentially from serum. In the group of GBS patients, oligoclonal IgG was transitory and correlated significantly with the development of BCB damage, cranial neuritis and severity of the disease. CIP patients showed a stable IgG pattern, which varied slightly after immunosuppressive therapy.

[Further studies of centering posterior chamber lenses in attempted sulcus fixation].

A total of 301 posterior chamber lenses implanted consecutively by one surgeon were examined on postoperative days 3-7 with special regard to both centering and the position of the haptics. It was possible to evaluate 296 eyes. In these, 83.1% of the lenses were ideally centered, while 16.9% were found to be slightly eccentric. There was one serious lens dislocation. The position of the haptics was evaluated in 245 eyes. The attempt to fix both haptics in the sulcus was achieved in 74.7% of the cases. Of the implants fixed in the sulcus, 96.2% were ideally centered. In the remaining cases 11 lenses were fixed entirely in the capsule but in 51 one loop was in the sulcus and the other in the capsule. While the lens is usually centered well when both loops are in the capsule (81.8%), it is significantly less well centered if one loop is fixed in the sulcus and the other in the capsule (41.2%). The data are discussed.

[Practical and psychological problems in invalidation evaluation].

Practical and psychological problems to give opinions of invalid were discussed and the defects of the practice in the neurologic and psychiatric branch of science are shown. The declaration of psychiatric findings is called to special attention.

[An electronic communication aid with a large keyboard for artificial respiration patients in the intensive care unit].

An increasing number of patients under mechanical ventilation in ICUs suffers from being cut off from normal communication. The existing communication facilities, such as letter-boards, magnetic letters, pictograms, Yes-no-signals and attempts at written communication are very often insufficient or not to be mastered by the weakened ICU respirator patient. We, therefore, present an electronic communicator consisting of a keyboard with big push-buttons and an illuminated running-letter display. Up to one page of text can be stored in memory and recalled later as running or printed text. Initial experiences with the electronic communicator show that even weakened respirator patients with diminished sensomotoric abilities successfully manage to communicate with the staff.

[Correlation between CT findings and clinical course in viral and bacterial meningoencephalitis].

The computed axial tomograms (CAT) of 50 patients with viral or bacterial meningoencephalitis are correlated with the clinical course, the laboratory data, and the EEG changes. The diagnostic value of CAT in the early diagnosis of herpes simplex encephalitis is of specific importance. Hypodense regions are demonstrated after the fifth day of illness in the temporal lobe. All other viral or bacterial meningoencephalitis cases have no specific changes on CAT examination compared with clinical or laboratory data. However, in localised encephalitis CAT may reveal hypodense regions. For follow-up studies of meningoencephalitis CAT is of important diagnostic value in demonstrating complications such as abscess or occlusive hydrocephalus.

Pulmonary circulation of the llama at high and low altitudes.

We have studied the pulmonary circulation of three closely related animals, the llama, alpaca, and guanaco. The mean pulmonary arterial pressure of 12 llamas and one alpaca indigenous to high altitude in the Andes was found to be slightly but significantly higher than that of three llamas and three guanacos born and bred at low altitude in England. On the other hand, the medial thickness of the muscular pulmonary arteries and the relative weights of the two ventricles were the same in three llamas and one alpaca at high altitude as they were in one llama and two guanacos at low altitude. It is concluded that the pulmonary vasoconstrictive response to hypoxia, while present, is greatly reduced in this species and it is suggested that this may have taken place by evolutionary adaptation.

[Psychiatry in Emden--the 1st year].

The article reports on the first developmental stage of a psychiatric department in a location close to the place of residence of the majority of patients. This psychiatric clinic has at present 60 beds and facilities for 20 outpatients. The location of the clinic is in the area of a medium-sized town in a rural district classified as an area in which Public Health services are required to be active. First of all, the article gives a detailed description of the clinic itself and of the population and district it has to look after. Secondly, the article describes the present state and development of negotiations regarding the setting-up of an outpatient treatment ward. In this connection, it became unavoidable to point out the discrepancies between the need of the population to receive adequate psychiatric outpatient treatment on the one hand, and the policy followed by the relevant authorities on the other, the latter being unfortunately coloured by vested interests, both politically and by the class-consciousness of representatives of certain trade guilds and corporations. This is followed by a statistical evaluation of the patients treated on an inpatient, outpatient and consultant basis, together with an attempt to give a brief outline of the relevant epidemiology. Since the Social Psychiatric Service of Emden has close associations with the clinic--a fact which is rather untypical--the authors included a report on the starting-up phase and activity characteristics of that service.

Demonstration of oligoclonal immunoglobulin G in Guillain-Barré syndrome and lymphocytic meningoradiculitis by isoelectric focusing.

Isoelectric focusing (IEF) of serum and CSF revealed oligoclonal IgG in 13 out of 16 patients with the Guillain-Barré syndrome (GBS), in 10 of them in serum only and in 3 in serum and CSF. Seventeen out of 19 patients with lymphocytic meningoradiculitis (LMR) showed oligoclonal IgG, 12 of them in CSF only. These findings, together with additional results, mean that in GBS oligoclonal IgG is synthesized mainly outside the CNS and in LMR within the CNS. Follow-up studies revealed changes in the oligoclonal IgG during the course of GBS and LMR. After treatment by plasma exchange the disappearance of oligoclonal IgG bands was followed by an improvement of GBS symptoms. The oligoclonal IgG bands returned in correlation with worsening of the disease. We were not able to elucidate the antibody character of oligoclonal IgG in GBS or LMR. No antibodies against the myelin basic protein (fragment 89-169) were detectable in the sera and CSF in any of the patients tested.

[Configuration-frequency-analytical assessment of psychometric procedures in brain damage diagnosis].

In a study of 462 neurological patients, the psychodiagnostic procedures of a tachistoscopic examination of the figure-ground differentiation, test dz and Bourdon-Wiersma test were assessed configuration-frequency-analytically. It was found that the procedure configuration of conspicuous tachistoscopie findings with a distinct slowing-down in the Bourdon-Wiersma test as well as reduced values in the test d2 (GZ--F) among all examined configuration indicate most clearly a cerebro-organic impairment.
