Influence of theophylline on the renal clearance of erythromycin.

The influence of theophylline on the kinetics of erythromycin was studied in 11 healthy subjects who received both drugs intravenously in therapeutic doses. Total clearance, terminal half-life and volume of distribution of erythromycin were not altered by concomitant theophylline treatment. However, renal clearance of erythromycin was enhanced by theophylline treatment (86.0 +/- 81.1 ml/min vs. 32.5 +/- 15.9 ml/min, with vs. without theophylline). Theophylline also altered urinary flow rate (4.7 +/- 1.6 ml/min vs. 1.9 +/- 1.2 ml/min, with vs. without theophylline) whereas fluid intake was identical (125 ml/h) as well as the pH of the urine (pH 6.5). The possible explanation that theophylline is acting via enhancing urinary flow rate, which reduced tubular reabsorption of erythromycin, has been ruled out by the findings of an additional trial done in 6 out of the 11 subjects. In this trial, renal clearance of erythromycin was not influenced by enhanced urinary flow rate (7.2 +/- 1.9 ml/min) which was due to high fluid intake (500 ml/h) whereas the pH of the urine was kept at 6.5 as in the first part of the study. When other mechanisms to explain the observed interaction were discussed taking into account our published results on theophylline kinetics influenced by concomitant erythromycin administration, the most likely explanation for our finding seems to be the interaction between erythromycin and 1-methyluric acid, one of the theophylline metabolites.

[Paralytic ileus--pathophysiology and clinical aspects].

Causes, pathophysiological aspects, and clinical manifestations of paralytic ileus are summarised in this paper. Reference is made to possibilities and problems in diagnosis and therapy. Prerequisites and indications for and limits to surgical treatment are discussed.

Synthesis, antitumor activity, distribution and toxicity of 4-[4-[bis(2-chloroethyl)amino]phenyl]-1-hydroxybutane-1 1-bisphosphonic acid (BAD), a new lost derivative with increased accumulation in rat osteosarcoma.

The aim of this study was to investigate whether the newly synthesized bisphosphonic acid-linked N-Lost derivative BAD retains bone-seeking and cytostatic properties. The paper describes experiments on mutagenicity in vitro and on toxicity in vivo. BAD is characterized by very low mutagenic activity toward histidine auxotrophic Salmonella typhimurium strains. Cytotoxic effects were tested in rat osteosarcoma and in Walker carcinosarcoma 256B. The LD50 of i.v. injected BAD was 146 mg/kg. Acute toxicity is probably caused by calcium complexing of the bisphosphonate part of the molecule. Labeling experiments showed moderate accumulation in bone and osteosarcoma, as well as in lung metastases. BAD effected high tumor growth inhibition in osteosarcoma and Walker carcinosarcoma-bearing rats and marked prolongation of survival; histologic and radiographic examination revealed rapid calcification of osteosarcoma and lung metastases. BAD-pretreatment produced protective effects against osteolysis induced by intratibially implanted Walker carcinosarcoma ascites cells. The cytostatic efficacy of equitoxic doses of BAD in rat osteosarcoma is comparable to that of dacarbazine and in Walker carcinosarcoma to that of melphalan.

Metal sensitivity: positive history but negative test indicates atopy.

Among patients with a history of metal sensitivity, 50 were patch-test-negative to nickel, cobalt and chromate. 20 of them were further examined epicutaneously and intracutaneously. They were also studied with a clinical diagnostic method for symptoms and signs of atopy. Most patients in this category were found to be atopic.

Eyelid dermatitis: the rôle of atopy and contact allergy.

Patients with eyelid dermatitis were studied with patch tests and a clinical point method for the diagnosis of atopic skin disease. In 38 patients, contact allergy was found in 11. The dermatitis was an expression of atopic dermatitis in 15 patients.

Substance P and vasoactive intestinal peptide in bullous and inflammatory skin disease.

Immunoreactivity (IR) of substance P (SP) and Vasoactive Intestinal Peptide (VIP) were determined by radioimmunoassay in serum of 56 patients with inflammatory skin diseases, in blister fluid of 40 patients with spontaneous blisters and 31 subjects with induced skin blisters. Serum concentrations of SP-IR and VIP-IR were mostly low or non-detectable. Spontaneous blisters contained high amounts of SP-IR, particularly in bullous pemphigoid and in some inflammatory dermatoses, while VIP-IR levels usually were low. Suction blisters from inflamed but not from normal skin often contained SP-IR but more seldom VIP-IR.

The effect of capsaicin on some experimental inflammations in human skin.

Topical application of capsaicin is thought to deplete substance P from local sensory nerve terminals. In experiments on human skin inflammation was induced by injection of substance P (SP) or histamine intradermally, UV irradiation, non-immunologic contact urticaria, tuberculin reaction, contact allergens and benzalkonium chloride with or without capsaicin pretreatment. The flare response to SP and histamine was suppressed by capsaicin pretreatment whereas the wheal was enlarged. Interestingly, capsaicin pretreatment enhanced the responses to all other inflammatory agents.

A diagnostic tool for atopic dermatitis based on clinical criteria.

Forty-seven patients with atopic dermatitis was compared to non-eczematous controls with regard to the occurrence of different symptoms and signs. This comparison made it possible to analyze statistically atopic symptoms and signs with regard to their diagnostic importance. Hereby, a diagnostic point system was constructed which in 96% of probands gave the correct diagnosis. This diagnostic tool might be used when examining patients with obscure eczematous disease.

Single exposure to ultraviolet irradiation and elicitation of human allergic contact dermatitis.

Ultraviolet radiation has been stated to inhibit afferent as well as efferent phases of allergic contract dermatitis. In this controlled study 17 female patients with nickel allergy were studied by three different protocols after an initial determination of their degree of hypersensitivity. They were patch tested with nickel sulfate immediately after UVB, 4-6 days after UVB, or immediately after external PUVA. Neither depressing nor enhancing of the allergic reaction was observed when compared to non-irradiated controls. The role of Langerhans cells as antigen-presenting cell playing an important role in the elicitation phase of allergic contract dermatitis is discussed and questioned.

The influence of locally administered ultraviolet light (UVB) on the allergic contact dermatitis in the mouse.

A quantitative model of contact allergy to picryl chloride in the mouse was used to evaluate the influence of locally administered ultraviolet light (UVB) on sensitization and challenge. UVB was given in three different doses immediately before and 24 h before sensitization and challenge, respectively. A suppressive effect was demonstrated on the afferent as well as on the efferent limb of the allergic reaction.

Thin layer chromatography and high pressure liquid chromatography of musk ambrette and other nitromusk compounds including photopatch studies.

For qualitative and quantitative analysis of musk ambrette and 4 other nitromusk compounds (musk ketone, moskene, musk tibetine, musk xylene) thin layer chromatography (TLC) and high pressure liquid chromatography (HPLC) techniques were developed. By TLC a reasonable separation was obtained and the limit of detection was 2-5 x 10(-7) g. By HPLC the separation was even better and the limit of detection for musk ambrette was 2 x 10(-9) g. The correlation between the amount of musk ambrette/ketone and the HPLC peak was linear. The TLC and HPLC techniques were used to demonstrate the presence of nitromusks in several commercial products, mainly aftershave lotions and eau-de-toilettes preparations. By ultraviolet spectrophotometry, absorption spectra were studied for the nitromusk compounds. The absorption maximum for musk ambrette was at 264 nm, that for moskene at 253 nm. Photopatch testing was carried out in 13 patients photoallergic to musk ambrette. Only 3 patients also reacted to other nitromusks. Photoallergey to musk ketone and musk tibetine is reported for the first time.

Lack of clinically important interaction between erythromycin and theophylline.

In 11 healthy volunteers the kinetics of theophylline and the plasma levels and the urinary excretion of its metabolites were studied before and after treatment with erythromycin for 10 days. Theophylline was administered as an intravenous bolus injection (280 mg) followed by a constant intravenous infusion (23.8 +/- 4.1 mg/h) for 6 hours. The total clearance of theophylline at steady-state (63.4 +/- 9.9 vs 63.8 +/- 14.4 ml/min, before vs after erythromycin treatment) and the elimination half-life after cessation of the infusion (6.7 +/- 2.6 vs 7.5 +/- 1.8 h, before vs after treatment) did not change during the treatment with erythromycin. No difference in the formation of metabolites before and after treatment with erythromycin was detected; the findings in urine were 40.4 +/- 5.0 vs 42.1 +/- 5.4% 1,3-dimethyluric acid, 29.6 +/- 4.6 vs 30.1 +/- 5.9% 1-methyluric acid and 13.4 +/- 3.5 vs 12.5 +/- 2.2% 3-methylxanthine before and after erythromycin treatment, respectively. It is concluded that a clinically relevant interaction between erythromycin and theophylline does not occur.

Attempts to induce contact allergy to nickel in the mouse.

Various attempts to sensitize mice to nickel salts are described. The antigen was applied once or repeatedly by the epicutaneous or intracutaneous route and by combinations of the two. The procedures included prior irritation of the skin, treatment with Freund's complete adjuvant, cyclophosphamide or Tetramisol, and prior sensitization to picryl chloride. Contact allergy was achieved only by repeated epicutaneous application of a strong nickel solution over a 3-week period. The resulting dermatitis was demonstrated by a weak but significant wet weight increase of inflamed skin.

Flare-up of allergic contact dermatitis in the mouse after topical distant provocation.

Flare-up of contact dermatitis after exposure to the antigen on another skin area was studied in the mouse. Animals were sensitized and challenged with picryl chloride. After healing, the ear dermatitis flared up following provocation on abdominal skin. Likewise, nickel-sensitized mice flared in one ear when exposed to the antigen on the other ear. This model could be of use in pathogenetic studies on secondary eruptions which occur so frequently in contact dermatitis.

Investigation of the antitumor activity of new epoxide derivatives. Part II: N-glycidylated oxo-nitrogen heterocycles.

A number of N-glycidyl compounds derived from oxo-substituted nitrogen heterocycles such as diazines, triazines, diazoles, triazoles as well as condensed ring systems containing these units have been synthesised. All products were tested for their antitumor activity against leukaemia in the mouse. Almost all of the approx. 50 di- and triglycidyl compounds showed activity, whereby 5 of them exhibited an increase of lifespan of 200% and more. The most active compound was 1,2,4-triglycidyl-urazol.
