Experimental studies on the mechanism of benoxaprofen photoreactions.

Benoxaprofen (BP), a non-steroidal antiphlogistic drug causing skin and nail photoreactions, has been evaluated for photoactivity using three experimental techniques. In vivo in the mouse, BP was phototoxic in doses of 25 mg/kg in combination with UV-A 54J. The phototoxic potency could be confirmed in vitro with the Candida albicans test. In vitro, using photohemolysis, BP showed a dose-dependent activity causing 40% hemolysis at a concentration of about 25 micrograms/ml with UV-A. Also, small UV-B doses caused red cell lysis with a moderate BP concentration. Pre-irradiation experiments showed that UV-A, but not UV-B, photoproducts could account for some of the activity. The action spectrum of BP photoactivity lies mainly in the UV-A, but may also extend into UV-B. Compared with chlorpromazine in vivo and in vitro, and with doxycycline in vivo, BP showed intermediate phototoxic activity.

Intermittent maintenance therapy in chronic hand eczema with clobetasol propionate and flupredniden acetate.

The clinical effect of two topical corticosteroids, one of very strong potency (clobetasol propionate), and one of medium potency (flupredniden acetate), was studied in the maintenance therapy of 55 patients with chronic hand eczema. Initially, 61 patients were treated on both hands continuously for 1 to 3 weeks with clobetasol only which brought about healing in 90% of cases (mean time to healing: 11 days). In a subsequent double-blind left/right study, the capacity of the two corticosteroids for keeping the dermatitis in remission was compared using an intermittent schedule of 2 applications a week. The protocol was followed by 46 patients and the mean observation period was 138 days. Treatment with clobetasol kept patients free from relapses during the entire observation period in 70%, with flupredniden in 30%. Relapses occurred with clobetasol after a mean of 66 days, with flupredniden after 36 days. Side-effects, occurring with similar frequency with both drugs, were few and mild. It is suggested that an intermittent schedule is advantageous when using a corticosteroid of high potency.

Allergic contact dermatitis of the mouse ear.

Allergic contact dermatitis to picryl chloride on the mouse ear was registered by measuring the increasing wet weight during the inflammatory reaction. This quantitative technique permitted the use of small experimental groups of regular laboratory mice. Full sensitization to picryl chloride is achieved as early as 3 days after a single painting with the hapten. The allergic reaction peaks at 24 h after challenge. Sensitization and challenge are not inhibited, whether by antihistamine, histamine liberator, or antiserotonin. The hypersensitivity state lasts at least 4 months.

[Tritium- and carbon-14-contents of wines of different vintage from the northern and southern hemisphere (author's transl)].

The carbon-14 and tritium radioactivity contents of up to 19 vintages of German and South African wines were compared. A similar large dependence of the 14C- and of the 3H-activity in the German wine on the nuclear weapon tests of the years 1962/63 was found out. The radioactivity level is also 1977/78 still essential higher than before 1950. The South African wines have been influenced considerably smaller by nuclear explosions. The highest 3H-values were found in the vintage 1963 of the German wine with 5910 pCi/litre and in the vintage 1964 of the South African wine with 510 pCi/litre.

Photoaugmentation in drug phototoxicity.

The phototoxic reaction to chlorpromazine and other drugs is provoked by long-wave ultraviolet light (UVA). It was shown by the in vivo mouse tail technique that the reaction is enhanced by medium-wave ultraviolet light (UVB), thus demonstrating the importance of photoaugmentation in this process.

Metal sensitivity in patients with metal-to-plastic total hip arthroplasties.

Out of 134 patients patch tested after total hip replacement (metal-to-plastic) 13 were sensitive to one or two of the implanted metals. In a prospective series of 112 patients, nine were sensitive to nickel before and another three developed sensitivity to nickel or cobalt after the arthroplasty. In the retrospective as well as in the prospective series the complications could in all cases but one be explained on grounds other than allergy. The incidence of contact allergy to nickel before a planned arthroplasty was 12 per cent in females and 4 per cent in males. It is doubtful whether metal sensitivity could be induced by a total hip arthroplasty. However, in cases of a preexisting metal allergy, a certain degree of caution is recommended.

Methacycline hyperpigmentation: a five-year follow-up.

A five-year follow-up is presented of a unique material of patients who acquired a greyish black hyperpigmentation during long-term methacycline therapy for chronic bronchitis. The melanosis disappeared in cases in which tetracycline therapy was discontinued during the observation period but not in those in which doxycycline had been substituted. In two autopsy cases pigment deposits were also found in cartilage and atherosclerotic lesions. Overall clinical and microscopic findings suggest a similarity to the pigmentation occurring in iatrogenic ochronosis.

Nickel concentration of blood, urine and sweat after oral administration.

Healthy volunteers without nickel hypersensitivity were given 5.6 mg nickel orally. The nickel concentration was assayed in urine, serum and sauna-provocated sweat from hands and trunk before and after nickel ingestion. The nickel concentration in serum increased in all subjects, and in most cases in urine, but not decisively in sweat.

Contact allergy without skin disease.

Patch tests with a standard series were performed in 274 non-dermatologic patients undergoing orthopedic surgery. Allergic reactions were observed in 22%, particularly to nickel (7%) and balsam of Peru (5%). Contact allergy in patients without previous or present skin disease was observed in 9%. A certain frequency of contact allergy in the general population should be held in mind when considering the incidence in patients with skin disease.

C-reactive protein, C3, C4 and properdin during the Jarisch-Herxheimer reaction in early syphilis.

The occurrence of plasma CRP during the febrile response after the first injection of penicillin was followed in 10 patients with early syphilis. An increase in CRP was noted after 12 hours, with a maximum after 24 hours. The appearance of this acute phase protein could not be correlated to cutaneous reaction, increased body temperature or leukocytosis, nor were baseline values of CRP correlated to clinical or serological activity of the infection. Patients with high levels of CRP prior to treatment suffered the most intense systemic reactions. No activation of the complement system during the Jarisch-Herxheimer reaction was detected from sequential determinations of C3, C4 and properdin.
