Complete reversibility of paraneoplastic acanthosis nigricans after operation.

A patient with widespread acanthosis nigricans is described. No abdominal tumour was found. Explorative thoracotomy disclosed numerous enlarged lymph glands containing squamous cell carcinoma. The left lung was removed but meticulous search did not disclose any tumour. The glands were removed as radically as possible. After the operation the skin improved and the lesions have disappeared completely. The observation time is over three years.

Pyoderma gangraenosum (dermatitis ulcerosa) and monoclonal (IgA) globulin healed after melphalan treatment. Case report and review of the literature.

Since 1968 we have been treating a patient, who has had a combination of pyoderma gangraenosum (dermatitis ulcerosa) and signs that may indicate early multiple myeloma. She also had carcinoma of the colon, which was successfully operated. The pyoderma healed later after intensive and successful cytostatic treatment of the "myeloma". The ulcers remain practically healed and the protein pattern is normal in May 1977. Such cases are rare and a search in the literature has not been very rewarding. In our own series of more than 200 cases with myeloma this combination is unique. The lieterature is discussed in detail with data on the follow-up on some of the patients.

Release of nickel from cooking utensils.

The release of nickel to boiling water from new and used saucepans of different material was measured. No nickel was released from aluminium, teflon and enamel. Certain amounts of nickel were released from stainless steel, but only at acid pH.

Phototoxicity of Dictamnus alba.

Phytophotodermatitis from Dictamnus alba was observed in three patients. Thin-layer chromatography and spectrophotometry disclosed the occurrence of 5-methoxypsoralen and 8-methoxypsoralen in the plant extracts which was also demonstrated by their phototoxic activity in vitro.

Drug phototoxicity in mice.

A series of agents with alleged photosensitizing properties has been studied in mice by a quantitative in vivo method for acute drug phototoxicity. The method, originally developed for phenothiazine studies, was found suitable for several drug groups with varying mechanism of action, such as tetracyclines, protoporphyrin and psoralens. In the sulfa group only sulfanilamide was active, although weakly. Phototoxicity in vivo, hitherto not demonstrated, was observed with griseofulvin, nalidixic acid, imperatorin, kynurenic acid and amiodarone.

Intermittent treatment of psoriasis with clobetasol propionate.

Clobetasol propionate cream, applied intermittently for 2 weeks, induced a rapid clinical and histological clearing of psoriatic infiltrates. The serum cortisol level was transiently lowered. By maintenance therapy, with the corticosteroid given only one day a week, 8 of 12 patients were kept in remission for an average period of 5 months.

The adjuvant effect of tuberculin in experimental sensitization to merthiolate.

When guinea pigs were sensitized to tuberculin by exposure to complete Freund's adjuvant the size of the intradermal PPD test was positively correlated to the body weight of the animal. Old Tuberculin showed stimulating effect in a subsequent sensitization to merthiolate (thimerosal). The high frequency of delayed cutaneous allergy to merthiolate in Swedish population is probably explained by sensitization through intradermal testing with tuberculin containing merthiolate as a preservative.

Phenothiazine phototoxicity: an experimental study on chlorpromazine and its metabolites.

Chlorpromazine (CPZ) metabolites were studied for their phototoxic potency with the mouse tail technique. The demethylated metabolites were more phototoxic potency with the mouse tail technique. The demethylated metabolites were more phototoxic than CPZ, while CPZ-sulfoxide, 7-hydroxy-CPZ, and 2-chlorphenothiazine were less phototoxic. The phototoxicity of CPZ and desmethyl-CPZ preexposed to UVA was lost, while that of CPZ-sulfoxide was retained. By thin-layer chromatography, CPZ-sulfoxide and promazine were identified as photoproducts of CPZ; the sulfoxide was fairly stable to radiation.

Phenothiazine phototoxicity: an experimental study on chlorpromazine and related tricyclic drugs.

A large number of phenothiazines and chemically related tricyclic drugs have been studied with respect to their phototoxic potency. Two methods were used, an in vivo technique based on the inflammatory response of the mouse tail after systemic administration of the drug plus UVA irradiation, and an in vitro method based on growth inhibition of Candida albicans. Of 27 commercial tricyclic drugs tested in vivo, the most potent were chlorpromazine and two other chlorinated compounds, prochlorperazine and perphenazine. Tricyclic drugs lacking nitrogen, sulphur, or both in their ring system showed no activity. All compounds phototoxic in the mouse were so in the yeast test as well. Here, however, the thioxanthenes (lacking nitrogen) were also highly active.

Phototoxic reaction to chlorpromazine as studied with the quantitative mouse tail technique.

A quantitative in vivo technique for the study of the phototoxic reaction to systemic chlorpromazine has been developed, based on the increasing tissue fluid content of the albino mouse tail. A maximum increase in tissue fluid of about 25% was registered 24 hours after starting the long-wave ultraviolet light exposure. The minimum phototoxic dose of chlorpromazine was 2.5 mg/kg.

[Acute adrenal hemorrhage in newborn infants: the importance of early diagnosis (author's transl)].

Two case reports demonstrate the importance of an early diagnosis for the therapy of adrenal hemorrhage. In the first case with unilateral hemorrhage, the hemorrhage as such was of importance. In the second case with bilateral hemorrhage, the course was dominated by acute adrenal insufficiency with circulatory collapse and major salt loss. The diagnosis was supported by the typical changes in electrolyte levels as well as by radiography, and endocrinological results. With early therapy (in the first instance by surgery, and in the latter by substitution of corticosteroids), both children safely passed the critical period. In the second child ACTH administration did not, in the first phase of the disease, result in a significant rise of plasma cortisol. However, two months later there was a normal rise of the cortisol level following ACTH administration. From this observation we conclude that a favourable regeneration of the adrenal glands has resulted.

Phototoxic textile dermatitis ("bikini dermatitis").

Phototoxic textile dermatitis with subsequent hyperpigmentation developed in two patients after they wore bikini bathing suits. After extraction of the dye from the bathing suits, 15 fractions could be visualized by two-dimensional thin-layer chromatography. Two of these fractions are found in Disperse Blue 35, an anthraquinone dye known to give rise to occupational phototoxic dermatitis, but which, to our knowledge, has never been reported to cause dermatitis in consumers. One of the two fractions was also found to cause phototoxic reactions in normal subjects.
