Cardiovascular diseases are the leading cause of death in Western countries, with an enormous increase in death rate and involvement of younger age groups during the last decades. This applies especially to coronary heart disease and is mainly caused by first-degree risk factors: hypertension, hyperlipoproteinemia, cigarette smoking, gout, obesity, polycythemia, lack of physical activity, and stress. These risk factors are discussed with special reference to overnutrition and increased cholesterol levels. Recent resuults of research concerning lipids and their relation to atherosclerosis are reviewed.

In 158 uraemic patients on haemodialysis complaints resembling Raynaud's disease were observed more frequently (85 patients = 54%) than in an equal number of age and sex matched control persons (10 probands = 6%). The complaints were also registered in the arm without the fistula. Triggering causes were exposure to cold or isometric muscular exercise. The complaints were slightly more frequent in women and occurred in 28% prior to dialysis in the terminal stage of renal insufficiency. They were improved only in a minority of those treated with haemodialysis, however they disappeared after successful renal transplantation. Veno-occlusive plethysmographic investigations showed that in asympatomatic patients the resting perfusion at room temperature, the perfusion in reactive hyperaemia, and perfusion at 0 degrees C were not different from control persons. Patients with Raynaud's phenomenon had on average a diminished resting perfusion and a normal perfusion after reactive hyperaemia. The perfusion at 0 degrees C was lower than in asymptomatic patients but still higher than in patients with Raynaud's disease. Increased cooling of the skin and delayed rewarming were objectively measured by using skin temperatures. There was not correlation between the complaints or perfusion values and the vibration threshold.
Hemodynamic investigations were carried out in the various stages of a peripheral arterial occlusive disease, primarily using venous occlusion plethysmography, and simultaneous metabolic determinations were performed in groups of 60 patients. As a rule, there were poorly collateralized occlusions at several levels in the severe stages of the peripheral blood-flow disorders. Venous occlusion plethysmography shows a pathological profile of reactive hyperemia. The parameters of resting blood flow in the severe stages were not found to be lowered in any patient; their average value was higher than that of a healthy comparison group (60 limbs). The oxygen saturation in the femoral vein in almost equal to that in the femoral artery in the exercise period in these severe stages, i.e. no oxygen utilization takes place in the periphery. While a rise in various enzyme activities could be observed after ergometric exercise in patients in stage II according to Fontaine, this is not possible in stages III and IV. There were no significant differences in measurements of the arteriovenous lactate-pyruvate difference, the pO2, the pH, the standard bicarbonate the hematocrit and the viscometry. In the severe stages, there is thus a situation of anaerobic metabolism and an accumulation of acid metabolites. An attempt is made to explain why a systematically perfomed vasodilatation has no positive effect but can lead to undesired side-effects such as a borrowing-lending phenomenon. Intra-arterial vasodilatation with short-acting vasodilators can only lead to a necessary blood-flow when the vascular occlusion is compensated by a well collateralized cirulation. In the severe stages, a fall in the perfusion pressure and blocking of the terminal blood vessel is possible. In advanced stages of a blood flow disorder it is therefore necessar during the first infusion to demonstrate by venous plethymography a quantitative increase in blood flow and to exclude a local peripheral steal effect. Various other possibilities of therapy are indicated.

127 patients with pulseless disease were compared with normal persons of the same age group, localization and distribution of the occlusions, risk factors and complaints were analysed. Pulseless disease occurs predominantly in elderly men. In all patients, occlusions were found more often in the left supraaortic branches than in the right side. In men, stenosis of the left subclavian artery, in women stenosis of the right carotid artery is found most frequently. In most cases only one aortic branch was diseased. Vertigo, tinnitus and deafness was encountered in 32 patients (25.20%) but only in 3 persons of the control group (2.36%). In 29 of these 32 patients uni- or bilateral stenosis of the carotid artery was present. In the cases with unilateral stenosis (16) tinnitus and deafness could be localized to the respective side. In the other cases an isolated stenosis of the subclavian artery was found. In the 32 patients with otogenic symptoms hypertension was the risk factor number 1. Early diagnosis by only auscultation and palpation as well as medical treatment is pointed out.
Report dealing with the clinical and pathoanatomical course as well as the autopsy findings in a 54 year old female suffering from panarteritis nodosa. Onset of the illness with polyneuritis and arthralgia. One year later diagnosis of panarteritis nodosa verified by muscle biopsy. Deterioration of the disease leading to the development of peripheral gangrene could not be prevented in spite of intensive therapy with steroids, immune suppressive agents, digitalis and antihypertensive drugs. Death 4 years later by myocardial infarction. Autopsy revealed generalized healed panarteritis nodosa with scarring and obliteration of vessels. A short description of the symptoms of the disease is given and the efficacy of the therapy with steroids and immune suppressive drugs is discussed from the clinical as well as the pathoanatomical point of view. Immunopathologic mechanisms are considered to be the responsible factors for pathogenesis.
