Gallium-67 citrate uptake in experimental tumors and inflammatory lesions--an histo-autoradiographic correlation.

In a DAB-hepatoma organic tumor, Ga-67 accumulated markedly in the hepatocellular carcinoma with scant stroma but not in the cholangioma that was rich in connective tissue. No granulation tissue was formed around the tumor. In transplanted tumors (Ehrlich's tumor and Sarcoma 180), Ga-67 accumulated in the granulation tissue within the fibroblasts, leukocytes and capillaries surrounding the tumor, rather than in the tumor. In inflammatory lesions, Ga-67 uptake was similar in granulation tissue with large numbers of phagocytes, such as leukocytes and histiocytes, and capillaries. There was a very good correlation between the degree of Ga-67 uptake and of cellular infiltration and the proliferation of capillaries. Ga-67 uptake, both in inflammatory lesions and in transplanted tumors, was observed in the granulation tissue. This result implies that the mechanisms of Ga-67 uptake in tumor and inflammatory tissues are not the same, because Ga-67 accumulated in the DAB-hepatoma that had no inflammatory granulation tissue. This study indicates that a non-transplanted tumor is required to study Ga-67 accumulation in tumors, as different results may occur in tumors with and without inflammatory granulation tissue.

[Hilar and mediastinal lymph node metastases from lung cancer; detection with CT and MR imaging].

Sixty-one patients with primary lung cancer, who had CT and MR imaging before surgery, were studied. MR imaging used spin-echo sequences with T1 weighted image and T2 weighted image (C-MRI), and included STIR technique (Stir-MRI). The accuracy of diagnosis of mediastinal lymphadenopathy (regarding 10 mm or larger in short transverse diameter as positive) was 89% by CT, 84% by C-MRI and 89% by Stir-MRI. The accuracy of diagnosis of hilar lymphadenopathy was 42% by CT, 67% by C-MRI, and 75% by Stir-MRI. The accuracy of diagnosis of mediastinal lymph node metastases was 89% by CT, 94% by C-MRI and 96% by Stir-MRI. The accuracy of diagnosis of hilar lymph node metastases was 78% by CT, 87% by C-MRI and 89% by Stir-MRI. The possibility of enhancement of diagnostic accuracy of lymph node metastases from lung cancer was suggested by combining MR imaging included STIR technique with CT.

[Increased gallium-67 citrate uptake in the breast of a patient with gynecomastia].

The patient was a 60-year-old male. He had undergone orchiectomy 5 years earlier because of prostatic cancer. During this interval, he has taken the estrogen. Scintigraphy revealed on accumulation of Ga-67 citrate in the primary/metastatic foci. Of interest, the image of the accumulation of in the breast demonstrated a "doughnut" pattern.

[A successful surgical treatment of WPW syndrome associated with tricuspid atresia].

The occurrence of both tricuspid atresia and WPW syndrome is quite rare. To our knowledge, only two operated cases have been reported. In this report, a 8-year-old boy with this combination was reported. Tachycardia via the right side accessory pathway was for the first time pointed out after Kreutzer's operation. Epicardial mapping showed the earliest excitation at the right free wall. When fatty tissue around this area was removed by CUSA, a large cardiac vein appeared. By holding this vein with a forceps, the delta wave disappeared temporarily. ACP seemed to be present near this vein. Permanent disappearance of the delta wave was obtained after ligation and division. Four months after surgery, he is free from tachycardia attacks.

[MR imaging of bone and soft tissue tumors].

One hundred sixty-one patients with tumors of bone or soft tissues underwent MRI. We believe to be able to differentiate benign from malignant lesions on the basis of imagings at internal, marginal, and peripheral properties. MR imagings demonstrated specific morphologic features that suggest the correct tissue diagnosis. MR imagings with STIR method or Gd-DTPA were more valuable. Because of the high contrast between the tumors and normal tissue, especially by STIR method, MRI provides improved detection and delineation of bone soft tissue tumors when compared to other diagnostic modalities.

[Emergency perfusion lung scan for pulmonary embolism].

Six cases of pulmonary embolism were reported. As pulmonary embolism often becomes critical condition, early diagnosis and medical treatment is need. Perfusion lung scan for pulmonary embolism may be useful for a diagnosis and evaluation of the degree of its disease and therapeutic value.

[Computed tomography of obturator foramen hernia].

A 89 year-old female was admitted with the complaint of right hypochondralgia. And also she complained of right thigh pain after admission. On hospital day 4, she underwent CT examination in order to evaluate right thigh with severe pain. The preoperative diagnosis of obturator foramen hernia was performed.

[Diagnosis of acute gastrointestinal bleeding using scintigraphy with 99mTc-labeled red blood cells].

To evaluate the usefulness and clinical significance of scintigraphy using 99mTc-RBC in urgent abdominal bleeding, we performed abdominal bleeding scintigraphy in 20 patients with melena. These patients were admitted to the critical care center. Nine of 20 patients showed positive images with bleeding scintigraphy. Five of 20 patients were referred because of small intestinal bleeding. Four of these 5 patients revealed positive images. Seven of 9 patients who had positive image and four of 11 patients who were negative had open-surgery performed. While angiography was done in 8 patients, only 3 of the 8 patients were proven to have bleeding. These 3 patients also revealed positive images by bleeding scintigraphy. This technique facilitates the screening of bleeding in the ileum, where endoscopic techniques may prove difficult to detect the site of bleeding. We emphasize that abdominal scintigraphy should be performed on patients with G.I. bleeding since this is a more accurate examination than angiography.

[Bronchial atresia of additive anomalous bronchus].

A case of bronchial atresia of additive anomalous bronchus in right middle lobe was reported. A mass and emphysematous lung were found on the chest roentgenogram. CT, and MRI, and it is not difficult to diagnose.

[The role of surgery in the management of small cell lung carcinoma].

Small cell lung carcinoma is responsive to chemotherapy and radiotherapy, but long-term survival of patients is rarely seen. We analyzed resected 19 patients with small cell lung carcinoma. The overall 5-year survival rate of resected patients was approximately 30% and mean survival time (MST) was 50 months. 2 patients of them survived more than 5 years. The survivors were received chemotherapy and radiotherapy several times after surgery. When we consider the role of resection in the treatment of small cell lung carcinoma, we can realize multimodality therapy including resection is important to obtain long-term survival. Moreover, the surgical resection can identify mixed type with non-small cell elements or residual tumors after chemotherapy and determine optimal treatment methods from their histopathologic findings.

Isolation and Characterization of an Enterobacter cloacae Strain That Reduces Hexavalent Chromium under Anaerobic Conditions.

An Enterobacter cloacae strain (HO1) capable of reducing hexavalent chromium (chromate) was isolated from activated sludge. This bacterium was resistant to chromate under both aerobic and anaerobic conditions. Only the anaerobic culture of the E. cloacae isolate showed chromate reduction. In the anaerobic culture, yellow turned white with chromate and the turbidity increased as the reduction proceeded, suggesting that insoluble chromium hydroxide was formed. E. cloacae is likely to utilize toxic chromate as an electron acceptor anaerobically because (i) the anaerobic growth of E. cloacae HO1 accompanied the decrease of toxic chromate in culture medium, (ii) the chromate-reducing activity was rapidly inhibited by oxygen, and (iii) the reduction occurred more rapidly in glycerol- or acetate-grown cells than in glucose-grown cells. The chromate reduction in E. cloacae HO1 was observed at pH 6.0 to 8.5 (optimum pH, 7.0) and at 10 to 40 degrees C (optimum, 30 degrees C).

Detection of human malignant melanoma with gallium-67 citrate and N-isopropyl-(I-123) p-iodo-amphetamine.

Using N-isopropyl-(I-123)p-iodo-amphetamine (I-123 IMP) and gallium-67 citrate, it was possible to detect human malignant melanomas. I-123 IMP imaging was helpful in detecting melanomas occurring in the legs and arms, and the 4 hour image was better than the 1 hour image. Moreover, while gallium scintigraphy more clearly detects human malignant melanoma, I-123 IMP allows for the detection of human malignant melanoma within 1 hour after administration.

[Distortion of working models using different silicone impression techniques].

The influences of differing strains levels of impression materials in compression and impression techniques on the distortion of working models were examined. The working models, which differed in the size of cervical spillway (2mm and 9mm) and the impression techniques applied (the two-phase technique and the wash technique), were produced by using putty-type impression materials with three different strain levels (Reprosil, Express and Exaflex) following the metal master model of mandibular denture in an abutment form. Distortion of the working models was then measured by a three-dimensional measuring system. The results were as follows. 1) The models produced through the impression technique using hard impression materials with low strain levels displayed decreases both in the degree of inclination to the X axis of the abutment tooth variations distance between the left and right abutment teeth. This procedure was found to yield a model which was proximate to the master model. 2) The impression technique incorporating a small cervical spillway resulted in decreases in variations in the distance between the right and left abutment teeth and the diameter of the molar part of the abutment tooth, and the horizontal distortion of the alveolar ridge; the model thus produced was found to be proximate to the master model. 3) Application of the two-phase technique reduced variations in height of the abutment tooth, and the use of the wash technique led to decreased variations in the distance between the right and left abutment teeth and diameter of the molar cervix of the abutment tooth. 4) Differences in strain levels of putty-type impression materials, the size of cervical spillway and the impression technique used did not influence deviation of the model toward the Y axis (vertical direction). 5) The models with least distortion of the abutment tooth were obtained by the two-phase technique using hard impression materials with lower strain levels or by the wash technique using soft materials with higher strain levels.

[Lung perfusion scintigraphy by SPECT--new application of Bull's eye analysis].

The present study reports the characteristic performance using lung segmental phantom filled in Tc-99m-pertechnetate. To evaluate the segmental defect in lung perfusion scintigraphy, we applied Bull's eye analysis in addition to planar image set. Bull's eye analysis especially facilitated the interpretation in both middle and lower lobes.

[Clinical evaluation of serum level of NCC-ST-439 as a new tumor marker for colorectal diseases--fluctuation of serum NCC-ST-439 in patients with colorectal cancers before and after surgery].

Serum level of NCC-ST-439, a new tumor marker, was clinically evaluated in patients with benign and malignant colorectal diseases in comparison with serum CEA and CA 19-9. Fluctuation of serum levels of NCC-ST-439 in patients with colorectal cancers was especially studied. Serum level of NCC-ST-439 was determined by sandwich type NCC-ST-439 EIA kit, which was developed by Nippon Kayaku Co., Ltd., in 93 cases of primary colorectal cancers, 98 cases of recurrent colorectal cancers and 60 cases of benign colorectal diseases. Positive rate of serum NCC-ST-439 in patients with primary colorectal cancers was 8.6% in Dukes A cases, 14.3% in Dukes B cases, 37.5% in Dukes C cases and 78.9% in Dukes D cases. Positive rate in patients with recurrent colorectal cancers was 42.8%. On the other hand, false positive rate in patients with benign colorectal diseases was 5.0%, which was relatively low in comparison with serum CEA and CA 19-9. While serum NCC-ST-439 was well correlated with serum CEA and CA 19-9 in primary colorectal cancers, NCC-ST-439 value in benign colorectal diseases showed no correlation with serum CEA and CA 19-9. In patients with primary colorectal cancers, curative resection was applied for in 40 cases in which the value recovered to the normal range after resection. In contrast, all 8 cases, which received non-curative resection, showed the increase of serum NCC-ST-439 value. In cases of recurrent colorectal cancers, some cases, which responded against chemotherapy, obviously showed a decrease of NCC-ST-439 value. These facts indicated that serum level of NCC-ST-439 could monitor the clinical effects of surgical treatment, chemotherapy and radiation therapies and it could be used as a marker for colorectal cancers. In conclusion, determination of serum NCC-ST-439 value was clinically useful for the diagnosis and monitoring for patients with colorectal cancers as a new distinct tumor marker.

Evaluation of renal first pass blood flow with a functional image technique in hypertensive patients.

The renal circulation of patients with essential hypertension and renovascular hypertension was evaluated using 99mTc-DTPA. The first renal peak count (the first Cmax; FCmax), time phase distribution (the first Tmax; FTmax), and blood velocity (the FCmax/FTmax) were calculated by digital imaging. This yields a visual image of the renal circulation. We consider that the increase in the renal first pass blood flow in patients with essential hypertension is best observed pixel by pixel. The FCmax and FCmax/FTmax images before and after treatment by percutaneous transluminal renal angioplasty in patients with renovascular hypertension clearly show its therapeutic effect. The FI technique, therefore, has the advantage that it can be performed at the same time as the conventional routine examinations of renal function. This makes it very useful clinically.
