[Clinical transcutaneous measurement of oxygen pressure during the induction of anesthesia with barbiturates].

Intravenous injection of barbiturates is followed by a depression of the respiratory centre depending on the dose. Blood carbon dioxide increases. This can be measured continuously in the expired air. In the same time blood oxygen modifies. The hypoxia produced by this depressant action was determined by a non-invasive transcutaneous blood gas measurement technique during the induction of anesthesia.

[Adverse effects of modern inhalation anesthetics. 2. Control of amounts and elimination of escaping anesthesia gases].

This paper presents a review on the pathological effects caused by acute or chronic exposure to the inhalation anesthetics halothane, methoxyflurane, or enflurane. Methoxyflurane has a dose-related nephrotoxicity due to its metabolic degradation with release of fluoride ions whereas suggested pathological renal effects of halothane or enflurane are still under discussion. As to the syndrome of halothane- (or enflurane-, methoxyflurane-) associated hepatitis no dose-dependent hepatotoxicity has been proven but interactions with hypoxia, hypotension, drug-pretreatment, and perhaps genetic abnormalities should be kept in mind. Severe hematologic alterations are effected by prolonged exposure to N2O or halothane and alterations of tumor immunity caused by anesthetic agents are reported, too. From clinical studies and observations of pregnant animals, a correlation between the incidence of miscarriages or malformations and chronic exposure to low doses of inhalation agents may be stated. Nevertheless, an inhalation agent is easy to control because it can be eliminated quickly in the case of complications. Therefore, one would not like to miss these inhalation agents in clinical practice but the immission into the operating room should be limited. Moreover, the best way to keep the operating room clean from waste anesthetic gases is the installation of a scavenging system which is connected to suction.

[Distal ureteral stenosis following kidney transplantation].

75 patients after renal allotransplantation were investigated for postoperative urological complications. 11 patients were found to have a stenosis of vesicoureteral anastomosis. According to clinical and radiological findings a classification into three stadiums was performed. Stage I indicates no necessity for operation. Two patients in Stage II and three patients in Stage III underwent surgical treatment for stenosis of ureteroneocystostomy. Treatment led to improved renal function and loss of urinary tract infection in 4 patients. The results of the study indicated treatment of stenosis after ureteroneocystostomy in Stage II prior to severe damage of renal allograft.

[Anesthesia in pacemaker implantation. Experiences in 935 cases of anesthesia for the implantation or revision of a cardiac pacemaker].

Observations during 935 anesthesias for implantation or revision of permanent pacemakers are presented. Using different methods of anesthesia we found the light halothane anesthesia introduced by inhalation to be best, provided that only atropine was used for premedication. Applying this method we saw asystolies or ventricular fibrillation in 3% of all cases--3 patients (i.e. 0.4%) died in tabula. Tachycardia (2.4%) occuring mostly during the introduction period were successfully treated by verapamil or practolol. Hypotension (5.4%) mostly took place in the course of anesthesia after implantation of the pacemaker. This depression may be due to a normalisation of the enhanced stroke volume whedication with pethidine or induction with propanidid was followed by comparatively more complications such as exitus letalis (2% resp. 1.5%), cardiac arrest (6.5% resp. 9%) and hypotension (24% resp. 10.5%). Regional anesthesia did not bring specific advantages. The good experiences with soft halothane anesthesia for implantations or revisions of pacemakers include 125 high risk patients (ASA classification IV to VII).

[Anastomosis of the vas deferens employing concentrated fibrinogen: an experimental study in rabbits (authors transl)].

Bilateral vaso-vasostomy was carried out in 30 rabbits using highly concentrated fibrinogen in combination with thrombin and clotting Factor XIII for tissue adhesion. The patency of the anastomoses was confirmed radiologically and by the injection of a dye, as well as histologically in paraffin-embedded and Goldner-stained serial sections. The overall patency amounted to 58% in animals investigated 4--6 weeks postoperatively (i.e. 17 out of 30 anastomoses). Stenosis of the junction was observed in all patent anastomoses investigated. The results of vaso-vasostomy employing a biological tissue adhesive were markedly superior to those with cyanoacylate.

[Congenital polyfibromatosis: clinical and genetic studies].

Clinical and genetic investigations were performed on seven members of a family suffering from multiple hereditary fibromatosis. These studies indicate that the responsible gene appears to be coupled and transmitted with that of the rhesus system, located at chromosome one. The diagnosis, differential diagnosis and therapy, as well as histological alterations are pointed out and the importance of hormonal factors is discussed.
