Supratentorial astrocytomas of the midline in childhood--a review of treatment and prognosis.

Supratentorial astrocytomas in childhood are rare. In our series of 47 children (1979-1983) suffering from intracerebral tumors we observed three with astrocytomas situated in the midline. They had invaded the walls of the ventricles and caused hydrocephalus. The children were operated on by a right transventricular approach. We succeeded in total tumor removal in two cases and these children are well. One child (only subtotal extirpation had been possible) died. In our opinion, resignation is unfounded in such cases.

[Clinical aspects, therapy and morphology of epidermoids in the 3d ventricle].

Statistically, there is one epidermoid in the IIIrd ventricle among 10000 space occupying intracranial processes. The symptoms are uncharacteristic. The surgical accesses to the IIIrd ventricle are discussed on the basis of examples of such cases. When a foramen monroi blockade is present, the tranventricular approach via the right anterior horn appears to be advisable. The epidermoids in the IIIrd ventricle are frequently changed in a considerably regressive manner and thus cystic. As malformations they are probably identical with the so-called foramen Monroi cysts and the Septum Pellucidum cysts.

[Therapy of giant aneurysms in the region of the base of the skull].

The introduction of microsurgical techniques has given a new impetus to operative therapy of cerebral aneurysms. Method of choice is clip occlusion of the aneurysmal neck, but this goal cannot always be attained. Usually, fusiform shaped aneurysms and so-called giant aneurysms can only be treated by occlusing the feeding vessels. In such cases the preliminary extra-intracranial bypass provides optimal conditions for interrupting the main feeder. Special points of view regarding these rare aneurysms and their haemodynamic relationships to cerebral circulation are discussed. A therapeutical concept is proposed on the basis of three case reports.

[Gastroenterological endoscopy of the upper digestive tract from the surgical viewpoint].

On the basis of an EDP-supported analysis of data which were got in 1,808 endoscopized patients general problems are treated, basing on experiences of a surgicoendoscopic working team. Besides statements concerning the structure of age and sex, indicating signs as well as complication rate the authors adopt a definite attitude to problems of endoscopic care, documentation and training.

[Diagnosis and therapy of intracranial epidermoids].

After a short description of the pathogenesis and morphology of intracranial epidermoids, the clinic symptoms and the diagnosis are dealt with. Therapeutically, it is exclusively a total extirpation with the use of a surgical microscope that can lead to curing. A report is given on nine patients, eight of whom were successfully operated on. It is hardly possible to establish a founded preoperative diagnosis of the kind of the epidermoid. Hypodense space occupying processes in the computer tomogram should, however, give rise to consider differentialdiagnostically also the presence of epidermoids.

[Diagnosis and therapy of cerebellar angioblastomas].

The mainly infratentorially occurring angioblastomas belong to the group of benign vascular tumours. Histologically, however, one can distinguish from them pleomorphically undifferentiated forms which have an unfavourable prognosis. The article reports on a group of 20 patients who were operated on in the period from 1978 to 1982. The frequency among the brain tumours seen in the same period of time was 4.5 per cent, only related to the infratentorial tumours. Symptomatically, the signs of occlusive hydrocephalus were well to the fore. The exactness of the topical diagnosis by computer tomography in conjunction with microsurgical operation techniques enables a reduction of the operation risk. The postoperative lethality rate was 3 among 20 patients. Attention was drawn to the observation of a family with a genuine v. Hippel-Lindau syndrome in an incomplete form in a descending line with probably autosomal, dominant heredity.

[Simulation of a hypophyseal tumor in carotid-ophthalmic aneurysms].

Case report. A 59 year old woman was thought to suffer from a neoplasm of the hypophysis. This "tumour" was simulated by a giant aneurysm of the infraorbital segment of the left carotid artery. For technical reasons the left carotid artery had not been demonstrated in the angiogram. For final clarification CT as well as bilateral angiography of the carotid arteries will be helpful.

[Angiographic diagnostics of extracerebral haematomas (author's transl)].

This paper deals with 492 patients suffering from cranio-cerebral injuries. Intracranial haematomas were assumed to exist. The angiographic findings were compared with operation reports and postmortem findings respectively. It could be stated that extracerebral haematomas were detected by angiography in a percentage of more than 95.

[Pneumocephalus as sign of frontobasal skull fractures (author's transl)].

Pneumocephalus occurs as a complication after frontobasal skull fractures and points to a pressure difference between the extracranial and the intracranial space. From 8 cases observed during a period of 10 years, four case histories demonstrate the diagnostic principles and the surgical consequences. The diagnostic value of tomography of the frontal base for gaining an accurate picture of the topographic situation, and for proper application of intradural duraplasty, is underlined and characterized as a reliable procedure.

[Lumbar disc herniation; neurosurgical view (author's transl)].

Evidently, the choice of treatment in lumbar disc herniation seems to depend more on the medical knowledge of the first consulted doctor than on actual scientific considerations. Thus, many patients having been insufficiently treated for a long time, are admitted to the neurosurgical department very late. Nowadays, myelography using modern contrast media enables the protruded disc to be exactly localized and to be removed with minimal damage to the tissue.

[Angiographic diagnosis of subarachnoid hemorrhage--a study of 160 patients].

Among 1916 cerebral angiographies carried out in the years 1973 to 1977, the examination was made in 160 patients (= 8.4 per cent) after verified subarachnoid haemorrhages. In 107 cases (= 67 per cent) a morphological substrate was secured angiographically. Arterial cerebral aneurisms and arteriovenous angiomas were the most frequent causes of a haemorrhage with an occurrence of 32.5 and 16.3 per cent, respectively. Further causes were: degenerative diseases of the cerebral vessels in 11.9 per cent, intracerebral haematomas in 3.1 per cent, haemorrhages in tumours in 1.9 per cent and stenosis of the aortic isthmus in 1.3 per cent of the cases. In 33 per cent of the cases a morphological substrate could not be ascertained angiographically. Including the authors' own experiences, diagnostic criteria are derived which to a large extent co-determine the decision with regard to an operation including the respective technical procedure.

[Possible diagnostic misinterpretations in chronic epidural hematoma].

Of fifteen epidural hematomas with a protracted course of growth, three were preoperatively diagnosed on the suspicion of tumorous growth. By taking into account the entire course of disease, evidence of posttraumatic complications was obtained only from anamneses and, in one single case, from a fracture of the skull. Using the methods of instrumental neuroradiological diagnosis, the intracranial occupation of space could not be observed to be outside of the cerebrum.
