Bronchodilating drugs and airway reactivity.

Tests with various bronchodilator drugs are utilized in the screening of airway hyperactivity in clinical practice. Bronchodilator tests making use of Salbutamol (Ventoline) as aerosols and by mouth, SCH 1000 as inhalations, Thiazinamium (Multergan) by mouth were performed in 42 patients suffering from bronchial asthma or of chronic obstructive disease, who were investigated by measurement of FEV1, Raw, MEFR. The treatment tests have a variable but significant potency in bronchospastic diseases. Taking into consideration one, two or three signs of bronchodilation, the improvement of FEV1, of MEFR, of airway resistance, the decreasing efficacy of the drug is as follows: SCH 1000 aerosol, Salbutamol aerosol, Salbutamol per os, Thiazinamium per os. The measurement of Raw is the more sensitive exploration to detect the bronchodilator effect of these drugs. The observed bronchodilation may be considered as a criterion of hyperreactivity of the bronchial airways. The differences between the effectiveness of the drugs may allow a better knowledge of the mechanisms of the bronchial airway reactivity and of the treatment of bronchospastic diseases.

Immunoradiometric and electroimmuno assay of increased ferritin and apoferritin levels in serum.

Ferritin in serum from patients with increased serum ferritin levels has been studied both quantitatively and qualitatively. All techniques utilized in these studies are suitable to be used as routine screening tests for large numbers of patients. Electroimmuno assay (EIA) has been compared with the solid phase immunoradiometric (IRMA) assay as a technique to determine serum ferritin concentration (r = 0.99) and is suggested as a useful alternative when determining ferritin concentrations above 500 microgram/l. Iron stained EIA gels have been used to indicate the iron content of the ferritin molecule in sera. This simple screening test has demonstrated that apoferritin is found more often than iron-rich ferritin in the serum of patients with elevated serum ferritin levels. Immunoelectrophoresis precipitin bands suggest the heterogeneity of ferritin in serum from different patients.

A new hybrid haemoglobin: haemoglobin Strumica/Beograd occurring in an individual with four haemoglobins.

An investigation of the cord blood from full term twin infants revealed an additional haemoglobin F component due to an abnormal alpha chain. The father of the twins, whose blood picture was normal, was shown to have normal alpha and beta polypeptide chains together with variant alpha and beta polypeptide chains. Electrophoresis showed that he had four major haemoglobin components. Separation of the haemoglobin fractions by column chromatography, globin preparation, chain separation, tryptic and chymotryptic digestion and peptide map preparation led to the identification of haemaglobin A, haemoglobin Strumica (alpha2 112His leads to Arg beta2), haemoglobin D Beograd (alpha2beta2 121 Glu leads to Val) and a hybrid haemoglobin Strumica D/Beograd (alpha2 112His leads to Argbeta2 121Glu leads to Val).

[Determination of global diffusion of CO2 and its partial components under assisted respiration. Technical problems and practical value in respiratory resuscitation].

A study of partial CO2 ductance has been performed in patients under assisted ventilation. The authors propose methods for simultaneous measurement of paCO2, PAECO2, PECO2 and PICO2. This methodology is used in patients presenting different conditions accounting for respiratory resuscitation: barbiturate poisoning with healthy lungs, bronchial stasis, acute lung involvement, obstructive bronchopneumopathy and pulmonary embolism.

Haemoglobin Camperdown beta104(G6) arginine leads to serine.

Routine investigation of ante natal patients revealed a subtle change in the electrophoretic pattern on cellulose acetate of the proposita. Further investigations by isoelectric focussing in polyacrylamide gel suggested the presence of two major haemoglobin components. Using a modified cellulose acetate technique globin chain separation revealed an abnormal beta-chain. Chain separation on a carboxymethyl-cellulose column provided a pure sample of the abnormal beta-chain. After amino-ethylation, tryptic digestion and peptide mapping, amino acid analysis of relevant peptides showed the abnormality in the beta-chain to be a substitution of arginine by serine at the 104 position. The presence of a positively charged residue at this position would appear to be necessary for the stabilization of the haemoglobin central cavity. The replacement by serine in this haemoglobin leads to slightly decreased stability but does not appear to affect the oxygen affinity.

The bone marrow colony forming cell in megaloblastic anaemia and iron deficiency anaemia.

Bone marrow samples from patients with megaloblastosis and iron deficiency have been assayed for their content of in vitro colony forming cells (CFC), and compared with a group of normal patients. The concentration of these cells was found to be significantly increased in the megaloblastic group, while their content in the iron deficient patients was slightly higher than the controls. An in vitro thymidine suicide procedures was utilised to assay the proportion of CFC in the S phase of the cycle. This was found to be increased in the megaloblastic group and only slightly increased in the iron deficient group. The findings in megaloblastosis seem to be consistent with the concept of impaired DNA synthesis. As the CFC monitors an early granulocytic progenitor these data suggest some impairment in DNA synthesis or an abnormal increase in amplification in this myeloid stem cell compartment. Such alterations in granulopoietic proliferation may contribute to the ineffective granulopoiesis of megaloblastosis and accordingly may be an important factor in the development of neutropenia sometimes associated with this condition. The slightly increased CFC concentration and altered cell cycle status found in iron deficiency suggest that iron is not a major requirement for granulopoiesis.
