[The continent ileostomy--indication and results].

Proctocolectomy with conventional ileostomy cures patients with severe ulcerative colitis, however ileostomy appliances had to be worn for the rest of their lives. The continent ileostomy as devised by Kock provides the patient with an intraabdominal ileal reservoir and a valve constructed by invaginating the efferent ileal segment into the reservoir. The patient empties his reservoir 2 to 3 times a day by inserting a catheter through the valve. Our experience with 45 patients is reported. In most cases the continent ileostomy was constructed as a second procedure after proctocolectomy. The construction of a continent ileostomy as a secondary procedure can be recommended in patients with proctocolectomy. It offers patients a life situation differing only insignificantly from that of normal persons.

[Continent ileostomy - improvement of life quality].

Fifteen patients who had undergone proctocolectomy and conversion of conventional ileostomy into continent ileostomy as devised by KOCK were investigated an average of 4 1/2 years after continent ileostomy. The aspects investigated were skin problems, frequency of ileal pouch emptying, continence, and quality of life. It is concluded that successful conversion to continent ileostomy has eliminated most of the drawbacks of conventional ileostomy. The technically complex method of continent ileostomy should be employed only by surgeons experienced in this type of surgery and at centers where more experience with the method can be accumulated.

Effect of intraduodenal peptone on the lower esophageal sphincter pressure in the dog.

The role of the duodenum in the regulation of the lower esophageal sphincter pressure (LESP) was investigated in 4 unrestrained dogs equipped with a chronic Komarov type esophagostomy, a cutaneous duodenal fistula, a gastrojejunostomy, and a Y anastomosis between the distal end of the duodenum and the midjejunum. Installation of NaCl solution into the duodenum or into the stomach did not effect LESP during the following 60-min observation period. Peptone instillation into the duodenum or into the stomach produced a sustained elevation LESP. The rise of LESP was significantly greater after duodenal than after gastric instillation. Duodenal peptone did not raise serum gastrin. It is concluded that the duodenum may be an important regulator of LESP in the dog and that this effect is not mediated by circulating gastrin.

[How reliable is radiology in the diagnosis of large bowel carcinoma?].

In 262 patients with clinically suspect cancer of the large bowel, results of barium enemas were evaluated. Testing method was coloscopy. Out of 62 cases in which barium enema revealed a "typical" carcinoma, 17 were false positive. In 81 of 110 cases in which the result of barium enema was questionable, a malignant lesion could be ruled out. In 17 out of 90 cases with negative radiologic findings a carcinoma was found.

[An additional use of the small intestinal invagination valve].

Extensive small intestine resection (70--90%) as should be done after mesenteric infarction produces a chronic malabsorption syndrome with its consequences. Applying an invagination valve of the small intestine (Kock) and using the isoperistaltic modification, passage of the chyme is delayed and its resorption improved. A small intestine resection of 90% was done in 8 mongrel dogs. After an average of 2--3 months an obvious cachexia as well as a steatorrhea could be observed. By adapting an isoperistaltic small intestine invagination valve near the colon weight reduction could be stopped during another 6 months and the steatorrhea disappeared No ileus occured.

[The role of the duodenum in the regulation of the lower esophagus sphincter].

Instillation of a 20% peptone solution into the isolated duodenum causes a significant increase of LES-pressure in the dog. No comparable increase of LES-pressure after instillation of a 0.9% saline solution into the duodenum or of 20% peptone solution into the stomach is observed. As shown in this investigation, it is possible that the duodenum regulates the LES-pressure in the dog.

Dynamics of the continent ileal bladder. An experimental study in dogs.

A continent ileal bladder was constructed in six dogs. The most important requirements for a continent ileal bladder as a reliable mechanism of continency and for prevention of back flow in the ureters could be achieved by two intussusception valves (nipples) of small bowel in opposite directions. The dynamics of these "nipple valves" were studied electromanometrically and radiologically. These experiments showed that these intussusception valves did not binder a free flow in the isoperistaltic direction and did prevent reflux in the antiperistaltic direction.

The role of gastrin in duodenal ulcer surgery.

The role of gastrin and of serum gastrin analysis in duodenal ulcer disease and duodenal ulcer surgery is analysed. As far as etiology and pathogenesis are concerned up to now gastrin has never been shown to play a significant role. Neither does it provide any diagnostic help in the typical duodenal ulcer disease (but it will allow for diagnosis of the retained antrum after Billroth II resection and of the Zollinger Ellison syndrome). Gastrin determination therefore is not helpful in the choice of the correct operative procedure for the ulcer disease. In today's clinical practice its major role consists in the control of surgical results. This is illustrated by a prospective randomized study on proximal selective vagotomy with and without pyloroplasty. In these patients serum gastrin analysis has shown that the omission of pyloroplasty is not followed by antral stasis. It furthermore always exhibits the typicel vagotomy profile, although vagotomy is incomplete in the 2-DODG-test.

[Glucagon producing adenomatosis of Islands of Langerhans with polyendocrine symptoms].

A patient in whom Cushing syndrome had been diagnosed at the age of 23 was found 14 years later to have subclinical diabetes mellitus, subcutaneous calcified fat tissue necroses, and hypergastrinemia suggesting Zollinger-Ellison syndrome. Histopathologic investigation revealed pancreatic adenomatosis of the glucagon producing A2-cells with accompanying B-cell hyperplasia, and hyperplasia of the adrenal cortex. The origin of the increased serum gastrin concentration in this patient is not yet known. The significance of A2-cell proliferation in Zollinger-Ellison syndrome and and in multiple endocrine adenomatosis is discussed.

[The hemoccult test in the screening for colonic carcinoma].

The only practicable method for mass screening for carcinoma of the colon is detection of occult blood in stool. In a coloscopically controlled study with the Haemoccult test, dependent from diet, 54-66% of polyps and 77-100% of carcinomas were detected. The Haemoccult test is therefore a suitable method for mass screening for colon carcinoma.
