[Assessment of cardiopulmonary functional capacity with ergospirometric parameters at a capacity of 1 w/kg body weight].

A sample of 73 men and 52 women aged 20 to 59 years was examined during a work-out on a bicycle ergometer while in a reclining position in order to assess spiroergometric parameters at a power output of 1 w/kg body weight, in regard to cardiopulmonary efficiency. In the male sample, a useful correlation between the maximum O2-intake against several parameters measured at 1 w/kg was determined, especially against the respiratory equivalent, respiratory minute volume, oxygen pulse, and heart rate. In the female sample, the correlation was particularly evident against oxygen pulse, and, within certain age groups, against heart rate, respiratory equivalent, and respiratory minute volume. The results of a physical working capacity 170 did not yield a more advantageous relation to the maximum O2-intake than to the above mentioned parameters. We found the quotient of O2-intake to the product of heart rate and respiratory minute volume at a load of 1 w/kg to offer by far the most accurate correlation. These results show that the use of suitable spiroergometric parameters at the more clinically feasible 1 w/kg standard load provide useful conclusions to the actual maximum efficiency--all the more so since spirographic parameters can be used concomitantly to determine the heart rate capacity.

[On the prognostic value of the 1-h level in the glucose tolerance test. (Five year follow-up with long-term hospitalised psychiatric patients) (author's transl)].

In 922 long-term hospitalised psychiatric patients the glucose tolerance war examined by 1-h level after oral glucose during a five year follow-up study on cardio-vascular risk factors. We were unable to determine the future development of metabolism with reference to normal findings of the test, borderlines levels ans positively impaired glucose tolerance. But it was to be seen than depending on increasing basic levels the tendency towards a decrease in glucose tolerance increased too. As far as prognosis is concerned, a turning-point for the further development either towards a return to normal or a manifest diabetes seems to be at 180 mg% after 1 h. Above this level, the probability to develop manifest diabetes respectively a higher degree of impaired tolerance has become within a few years, just as great as the probability of a remission. The tendency towards a decreasing glucose tolerance or the manifestation of diabetes was furthered by initial existence or development of a higher overweight (more than 30%).

[Mortality in hospitalized psychiatric patients (results from a 5-year study) (author's transl)].

During 5 years we have registered mortality in 1239 chronic hospitalized psychiatric patients (609 men and 630 women) aged between 40-70 years in 6 German hospitals. We examined this patients on incidence of cardial riskfactors. The death rates were compared to the others of BRD. In this connection we found out an intensified mortality in all age groups of psychiatric patients as against to BRD average. The mortality of respiratory system diseases surpassed death rates of BRD many times better. Also psychiatric patients died by cardiovascular diseases more than expected. The death rates from ischaemic heart diseases in men were approximately twice as much than at BRD statistic. Patients who died in arteriosclerotic diseases had more often riskfactors than the other deceased. The great number of sudden and unexpected deaths was remarkable.

[Effectiveness of a preventive mass examination for cardiac risk factors (author's transl)].

18 months after a mass examination for coronary heart disease risk factors in employees of a large industrial firm, 75% of the subjects examined at that time were re-interviewed and measurements of blood pressure and weight repeated. Despite an altogether inadequate longterm control of the risk factors detected, a part of the subjects examined had drawn consequences regarding their way of life by reducing weight, changing smoking habits and increasing physical activity, thus proving the effectiveness of information and advice received in a screening investigation. In about 40% of the overweight subjects a reduction in weight and in half of the hypertensives a lowering of blood pressure to values below 160/95 mmHg was noted.

[Sport and cardiac risk factors in middle age (author's transl)].

1477 employees (868 males and 609 females) were examined for cardiac risk factors. The risk factor frequency was tested in relation to the amount of physical activity. Men and women active in sports are less subject to over-weight, diabetes, high blood pressure, and pathological ECG changes than are physically inactive; males who participate in sports also smoke less. On the other hand, a connection between risk factors and different levels of physical activity on the job could not be established. The mutual relations between risk factors, physical activity, weight, and social level imply that the lower number of risk factors in those who participate in sports is due largely to their consciously healthy way of living.

[ECG changes in psychiatric patients under long-term therapy with psychopharmaca (author's transl)].

Within the scope of an epidemiologic study on risk factors and coronary heart diseases, 1726 mentally ill, aged 40-69, under permanent hospitalization were examined electrocardiographically. There were few pronounced indications of a relationship between pathological ECG findings and psychopharmaca. ECG changes were rarest among patients treated exclusively with non-tricyclic drugs, and most often among those patients who received tricyclics and non-tricyclics simultaneously. Statistically ascertained, however, was only the connection with lesser pathological changes in the rest and load ECG's (T-flattening). In this connection, these disturbances appeared more often among women. A reciprocal effect between sex and the influence of psychopharmaca appeared on the ECG. Contrary to males, females under treatment with tricyclica or with several psychopharmaca simultaneously showed more ECG changes than those treated without medication.

[Coronary heart disease risk factors in white-collar and manual workers at a large Munich industrial firm (author's transl)].

1477 employees of a large industrial firm in Munich (868 males and 609 females, aged 40-59) were examined for coronary heart diseases risk factors. Among males, hypercholesterolemia predominates with a distribution of over 40%. Every fifth male has high blood pressure or is a heavy cigarette smoker. The females under 50 years of age clearly show fewer risk factors, but toward 60 years they exceed the males in frequency of some factors. Among women, too, the top risk factor is cholesterol, followed by overweight and high blood pressure. On the average, about 8% of the men and women have a preclinical or manifest diabetes. About every fifth male examined and every seventh female can be considered as especially endangered by the presence of 3 or more risk factors.

[The mortality from ischaemic heart diseases in hospitalized psychiatric patients (author's transl)].

In 1357 chronic hospitalized psychiatric patients (641 men and 716 women) aged between 40 and 69 years the mortality during 30 months was registered, referring for a prevalent study about riskfactors. The death rates for ischaemic heart diseases in hospitalized patients were 2,5-3,5 times as high as states in the statistic for total population of BRD. The majority of dead patients had two or more of the known riskfactors, more as the half of the dead patients had a manifest diabetes mellitus or a disorder of oral glucose tolerance test.

[The risk of heart disease in various psychiatric diseases].

The distribution of some risk factors on different psychiatric diseases in 1726 hospitalized patients were investigated within a prevalence study. To all tested factors there was a contrary behaviour of oligophrenic patients opposite to the group with a schizophrenia, organic and affective psychotics. Hypertension only was found more frequently in non psychotic psychopathia and oligophrenia than in psychosis. Therefore the prevalence of hypertriglyceridemia, hypercholesterinemia, diabetes, specific Ecg-changes, smoking of cigarettes and severe physical inactivity in oligophrenics was less except in all other group of diseases, also in consideration of the different frequency of psychopharmacal treatment. Electrocardiographical signs for coronary heart diseases also show a similar distribution of frequency. Likewise in psychiatric patients seems to be an agreement with the coronary morbidity and the somatic risk factors.

[Coronary heart disease in hospitalized mental patients. An electrocardiographic prevalence study].

Altogether 1707 permanently hospitalized patients aged from 40 to 69, from 8 psychiatric hospitals in Germany, were investigated for the occurrence of electrocardiographic signs of coronary heart diseases. ECG changes which indicate a past infarction or a coronary insufficiency with great reliability were found almost not at all in patients under the age of 50, in men aged from 50 to 59 years in 7.0%, and aged from 60-69 in 11.8% of cases. In women of corresponding ages the figures were 3.8 and 10.8% respectively. Compared with a non-psychiatric population, there were suggestions of a slightly higher prevalence of coronary heart diseases in the psychiatric patients.

[Risk factors of coronary heart diseases in hospitalized psychic patients].

The first results are communicated of studies which deal with risk factors, prevalence and incidence of coronary heart diseases in hospitalzied psychic patients. 1726 patients aged from 40-69 years hospitalized in 8 psychiatric hospitals and nursing homes in Germany were examined for the presence of risk factors of coronary heart diseases. Only 28.5% of the 867 men examined and 14.8% of the 859 women showed no risk factors. 37.3% of the male and 56.3% of the female patients had to be classified as particularly endangered on the grounds of two or more risk factors. The results were compared with those of some epidemiological studies in average populations. The higher prevalence of serious overweight and diabetic metabolic disorders in the psychiatric patients was striking.

[Psychotropic agents and pathological ECG changes (author's transl)].

1. An epidemiological research programme was carried out on a collective of 1752 psychotic patients from eight psychiatric hospitals during the period from October 1970 until February 1973. The possible relationships as to the intensity of pathological ECG changes and such factors as sex, age, the administration of tricyclic and non-tricyclic psychotropic agents with or without the combination of glycosides, the potency of neuroleptics and their clinical structure, were investigated. 2. No statistical significance could be established between pathological ECG changes and such factors as administration of tricyclic and non-tricyclic psychotropics with or without the combination of glycosides, or the potency of neuroleptics and their chemical structure. With a higher age the frequency of ECG changes increases independent on psychotropics. 3. The difficulties of interpretation, in particular regarding a generalisation of the results, are discussed.
