[Management of complex pelvic injuries].

Compound pelvic fractures can be mastered only by means of an active surgical procedure. If the shock cannot be controlled by conservative treatment, early angiography followed by embolisation should be done. Alternatively to embolisation, control of haemorrhage can be achieved by specific ligation, reconstruction of the vessels or tamponade with mandatory laparotomy. Septic complications can be prevented by a liberal indication of derivative colostomy with irrigation of the rectal segment by copious amounts of saline solution, by suprapubic urinary drainage and meticulous "debridement" of soft tissue. The procedure is presented in detail.

Identification key for coryneform bacteria derived by numerical taxonomic studies.

Six main groups were formed from a complete linkage dendrogram on 557 bacteria tested for 53 physiological features. The organisms were obtained from culture collections and included representatives of the following genera: Arthrobacter, Brevibacterium, Caseobacter, Cellulomonas, Corynebacterium, Curtobacterium, Micrococcus, Microbacterium, Mycobacterium, Nocardia, Oerskovia and Rhodococcus. The six groups were individually subjected to a numerical taxonomic analysis based on linkage maps, which resulted in a total of 33 subclusters. An identification key to determine the affiliation of the bacteria to the six main clusters and five group-specific schemes is presented. Reference strains are proposed for the 33 subclusters.

Trace elements in whole blood of multiple sclerosis.

Fe, Zn, and Cu were determined by atomic absorption spectrometry in samples of whole blood after weighing and wet ashing with HNO3. 35 healthy persons were compared with 119 multiple sclerosis (MS) patients and 13 patients with other diseases of the CNS. The values from normal persons are in full agreement with those cited in the literature, men showing a significantly higher level of Fe than women, whereas for Zn and Cu there is no significant sex difference. In comparison with the reference group the patients show no difference at all for Fe and only a minor one for Cu, but Zn values are greatly increased in MS patients (by 25-75%). The increase is higher in the relapsing form of MS than in the chronic form, suggesting some connection with the activity of the disease. On the other hand, the duration of the illness and (implicitly) the age of the patient seem to be unimportant. As patients with other diseases of the CNS show a similar though somewhat smaller increase, we conclude that Zn elevation in whole blood is not a feature specific for MS. From the present results, however, we cannot decide if such deviations are causally connected with affections of the CNS or if they are only a concomitant manifestation.

Some chemical aspects of labeling human fibrinogen with 99m-technetium.

Human fibrinogen was labeled with 99m-Technetium. Tin(II)-chloride in citric acid served as reducing agent for the pertechnetate-ion, eluted from a Mo-Tc-generator. Before adding the fibrinogen, the citric acid was always neutralized with sodium hydrogen carbonate. The influences on the quantity of fibrinogen bound 99m-Tc of the relative concentrations of Sn(II), fibrinogen and sodium hydrogen carbonate, of the reaction time and temperature were tested by thin-layer chromatography. The reaction temperature of 28 degrees C showed an optimum of fibrinogen bound 99m-Tc for the reaction time from 1 and 2 hours. With a reaction time of 30 minutes not enough 99m-Tc was bound to fibrinogen, the doubling of the reaction time from 1 to 2 hours showed only an increase of binding of less than 4%. The concentration of Sn(II) with respect to the fibrinogen concentration showed no influence on the quantity of fibrinogen bound 99m-Tc at low Sn(II)-concentrations. At values higher than 34 times of the fibrinogen concentration a decrease of the quantity of bound 99m-Tc was observed. The concentrations of sodium hydrogen carbonate showed no influence on the quantity of fibrinogen bound 99m-Tc but on the clottability of fibrinogen, the pH of the solution must be approximately 7.5. In 3 parallel and independent experiments under optimized conditions (1 hour at 28 degrees C, molar ratio of Sn(II) : fibrinogen = 8.5, pH = 7.5) 89.97 +/- 0.92 % of 99 m-Tc were bound to fibrinogen. Controls of these results by column chromatography showed a binding of 81.08 +/- 1.47 % of 99m-Tc to fibrinogen. The clottability of fibrinogen tested by the method of Clauss (1) was entirely preserved.

Reduction of exogenous oxalate in urine of rats by binding with aluminium-oxyhydrate (Andursil) and an anion-exchanger (Colestid) in the intestinal tract.

The influence of calcium carbonate, aluminium oxyhydrate (Andursil) and an anion exchanger (Colestid) on the absorption of oxalate from the intestine in rats was investigated. The animals were administered daily doses of 15mg oxalate as a 14C-sodium-oxalate solution by means of a throat probe, and the substances of interest were mixed with the food. The intake of food and the 14C-activity in the urine were measured during four urine-collecting periods of 3 days each. The quantity of the enterally administered oxalate excreted with urine has a negative correlation to the amount of the investigated test substances ingested with food.

[Fresh ligament injuries at upper ankle joint and foot (author's transl)].

Crush injuries and abnormal pronation and supination are the most common reasons for typical ligament tears in this region. Most of them should be treated conservatively by plaster of Paris and early weight-bearing after exact clinical and X-ray diagnosis. At upper ankle joint, suture of ruptured fibular collateral ligaments should be favoured. For dislocations and fracture dislocations of the Lisfranc type operative reposition and K-wiring is recommended. Our treatment results with fibular collateral ruptures and fracture dislocations of the Lisfranc type are presented.

Linkage-maps and their relations to linkage cluster procedures.

Methods of numerical taxonomy should provide a good representation of the relations of the objects and the structure of the groups. Besides dendograms, shade diagrams and minimum spanning trees, linkage-maps are suggested as a good additive tool to accomplish this representation. In this paper linkage-maps and similar technics are described, their relations to some generally known linkage cluster procedures are shown and their advantages are discussed. Their application is demonstrated using a small microbiological data set.

Absorption of oxalic acid in rats by means of a 14C method.

13 rats received a daily dose of 0.5 ml of a 1% solution of 14C-marked sodium oxalate (specific activity 29 nCi/ml) administered in one application with a throat probe. Feces and urine were collected every 24 h and the amount of 14C-oxalate was determined by liquid scintillation counting. The average distribution for 13 rats during 7 days showed 25.2% of 14C-oxalate in the urine and 73.4% in the feces. The rate of absorption is considerably higher than values given in literature (2.6--12% urine). These results indicate that in the pathogenesis of calcium oxalate stone formation some possible significance must be attributed to the exogenic oxalate.
