[Experimental studies on the problem of improving the myocardial blood circulation in an intraoperatively resuscitated heart].

A pumping system for supporting an intraoperatively failured heart is presented. This system consists of a bell-shaped receptacle for the heart and a pump with a working volume of 49 ml and a working pressure of 220 Torr. The pumping power is transmitted to the heart by means of an external pneumatic pressure line. The efficacy of the pump in the high pressure system amounted to 20-30 per cent. An effective coronary perfusion in case of a cardiac failure is guaranteed by a venous underpressure pump of this pumping system. A favourable influence was also observed in case of the low pressure system.

[Embolism caused by cardial myxoma].

Myxomas of the heart may present a systemic embolization before heart valve occlusion. A histological examination of peripheral emboli may result in the diagnosis. The tumour should be removed early before the occurrence of irreversible embolic complications. An embolic history could be found in 5 of our 8 cases. Two patients died immediately after complete peripheral arterial obstruction.

[Primary tumors of the heart and pericardium].

Primary tumors of heart and pericardium are rare. In consequence of modern noninvasive diagnostic procedures, some of these tumors today can be detected intra vitam and become accessible to operative therapy, too. The authors describe 10 primary cardiac tumors (7 atrial myxomas, 1 endocardial endothelioma of the right ventricle, 1 papilloma of the pulmonary valve, 1 sarcoma of the right atrium) and 3 diffuse malignant mesotheliomas of the pericardium. On the basis of the own material the histomorphologic variability and the dangerous complications of these neoplasms are discussed.

[Mycotic prosthetic valve endocarditis].

Five autoptically proven cases with mycotic prosthetic valve endocarditis are reported with special reference to morphologic findings. The clinical signs in such patients manifest in the early post-operative phase and are accompanied with a high mortality rate. At autopsy, there were in all cases thrombotic vegetations upon the prosthetic valves which often caused severe arterial and cerebral embolic events. Histologically, fungi are demonstrable within the thrombotic material without any difficulties, and after embolectomy intra vitam, too. In addition to pathological aspects, the author briefly summarizes the most important symptoms of this affection, its arduous diagnostic management, its possible mode of infection, and its treatment.

[Primary benign intramural intracavitary cardiac tumors--clinical, diagnostic and therapeutic aspects].

10 cases of benign primary tumours of the heart (5 myxomas of the left atrium, 2 myxomas of the right atrium, 1 right-ventricular endothelioma of the endocardium, 1 left-ventricular pseudorhabdomyoma, 1 tumour of the interventricular septum) are described. The diagnostically decisive method was in 5 cases the heart catheterization with angiocardiography, furthermore in one case each the roentgenogram of the thorax, the echocardiography, the histology of embolic material, the operation as well as the autopsy. The echocardiography was available for 4 patients; in one case it was diagnostically decisive, delivered important additional informations in 2 patients and failed in one patient. With increasing propagation of the echocardiography, which in the opinion of the authors at least in all cases should be used before a planned invasive cardiological diagnosis, is to be reckoned with an improvement of the diagnosis of tumours of the heart and with an increase of the chances of a causal, namely the timely cardiosurgical therapy.

[Intrathorakal nonchromaffin paragangliomas (chemodectomas) (author's transl)].

Report of 3 benign nonchromaffin paragangliomas (chemodectomas) localised in the anterior mediastinum (female, aged 25), the posterior mediastinum (female, aged 55) and the right upper lobe of the lung (male, aged 40). These tumours arise from parasympathetic paraganglia. About 5-10% of the tumours are malignant. A survey of the literature about intrathoracic chemodectomas is given and age sex distribution, histology, morphological differential diagnosis, dignity and therapy of these tumours are discussed.

[Significance of blood flow in electrically induced thrombosis].

The thrombosis in dependence on the stream velocity in the electrically induced thrombosis was investigated by blood flow measurements in the femoral artery in 16 mongrel dogs. Thromboses were shown histologically in all 16 animals. 8 of them did not obturate. Obturating thromboses were produced with the application of anodic direct current of 1 mA during 60 minutes only with a blood stream velocity between 0.0465 and 0.0928 m/s. Not any obturating thrombosis was visible outside of this range.

[Cause and manifestations of amniotic malformations (author's transl)].

An account is given to morphological manifestations of amniotic malformations in the regions of extremities, head, and trunk. The incidence recorded was 1:13,000 (or 0.008 percent). -Definition and differentiation were based on Schwalbe's criteria. Aetiology and pathogenesis of such rare abnormalities are discussed with reference to literature. Histological results have supported the assumption that whenever it comes to the buildup of an amniotic membrane placopathy with dissection of internal placental membranes between the connective tissue layer of the amnion and the membrana chorii should be taken into consideration.

[Vascular injuries caused by electric direct current in experimental thrombosis models].

The eventual causes of the lesion of the vascular wall through electric direct current (D. C.) in experimental artificial thrombosis are discussed by means of microscopic investigations by electron microscope and light microscope in altogether 73 mongrel dogs and 14 mini pigs. No thermic lesions of the vascular wall were observed in the coronary thrombosis pattern on the closed thorax by means of 1 mA amperage and an induction time of 60 minutes. Secondary processes for cell lesions are to be made responsible in which the electrolysis on the cell membrane and the influence of functional ranges in the artery wall play an important part.

[Pulmonary arteriovenous malformation (report on 11 cases, 1970-1979) (author's transl)].

The clinical features and the diagnostic difficulties in 11 cases of pulmonary arteriovenous (av) malformation, diagnosed in the last 10 years, are discussed. It is possible to get the correct diagnosis with the help of the typical thorax-X-screen, inclusive tomography in combination with the clinical signs of the right to left shunt. Pulmonary angiography is indicated (first) before any kind of therapy in order to exclude av malformations in other localisations, (second) in all pulmonary lesions, where av malformations are part of the differential diagnosis, and (third) in patients suffering from symptoms of a right to left shunt after exclusion of a cyanotic vitium cordis.

[The clinical picture and pathogenesis of cervical pregnancy].

A report is given on a true cervical pregnancy connected with placenta percreta destruens in the sixth month of pregnancy in a twenty six-years old woman after her first pregnancy had been interrupted. A pathogenetic correlation between the induced abortion and the cervical pregnancy seems to be very probably. A damaged cervix probably offers the fertilized egg a more convenient oppertunity for the nidation. The prognosis of those dramatic ectopic pregnancies firstly depends on the early decision on carrying out hysterectomy in order to stop the dangerous bleeding.

[Bronchial haemangiopericytoma (author's transl)].

Primary haemangiopericytoma of the lung is very rare. Up to now 36 cases of lung origin have been reported in the world literature. All of them were peripheral lung tumours. In this paper report is given on a malignant (central) haemangiopericytoma of the right main bronchus in a 27 years old female. The clinical features, the morphological findings, the differential-diagnosis, the therapy and the prognosis of these lung tumours are discussed.

[Necrotizing "sarcoid" angiitis and granulomatosis of the lung (author's transl)].

Report of 2 cases (59 years male, 53 years male) of necrotizing "sarcoid" angiitis and granulomatosis of the lung. This disease has the character of a local pneumonia or of a coin-lesion and a favourable course. Emphasis is put on the morphological set of problems; the differential diagnosis concerning other pulmonary forms of granulomatosis and angiitis is discussed. The diagnosis is confirmed only by investigation of biopsy tissue; mere clinical features prove to be insufficient.

[Mortality rate after lung resection for bronchial carcinoma (author's transl)].

1351 patients with lung resection were analyzed. Lethality during hospital treatment was 6.2%. 11 patients, who died during the 6 first weeks after the resection, had occult metastases. The causes of death among these 6.2% were pulmonary embolism (40.5%), insufficiency of the bronchial stump and following complications (17.9%), pneumonia (10.7%), cardiorespiratory failure (9.5%) and intraabdominal complications (8.3%). Improving these figures should begin with reducing the cases with pulmonary embolism.

[Histiocytosis X (eosinophilic granulomatosis) of the lung (author's transl)].

Report of a 21-year old male with biopsy-proved (primary) isolated Histiocytosis X (eosinophilic granuloma) localized in the lungs. The disease prefers young males and leads to a non-characteristical fibrosis of the lung in cases of chronical course. Lung biopsy is the only certain method for diagnosis. Morphology, differential-diagnosis and aetiology of this rare disease are also considered.
