[How effective are simple laboratory tests (Hb, BSG) in the early diagnosis of stomach neoplasms].

Up to now only early resection has proven of value in healing gastric carcinoma. It is therefore mandatory that the pathologic lesion be diagnosed as early as possible. Thus, patients with persistent epigastric complaints under symptomatic treatment should undergo endoscopy even if blood parameters are still within normal limits. The fact that, among all our gastric cancers diagnosed in 1977 by endoscopy, 23% were classified as early cancers indicates that the method is well suited to establishing true early diagnosis.

[What is the contribution of colposcopic polypectomy?].

Coloscopic polypectomy saves patients with polyps of the colon from diagnostic laparotomy and also serves for the diagnosis and in the majority of patients even therapy of early colonic cancer. Complications are rare and most can be controlled by endoscopy. Recurrences of the polyp are possible, and therefore, follow-up is necessary.

[Malignant double tumor of the lung: endobronchial carcinosarcoma in the left and polypous squamous cell carcinoma in the right upper lobe].

According to MOORE, two types of carcinosarcoma in the lung may be distinguished: one with a peripheral and one with an endobronchial localization. The latter type is characterized by minimal local infiltration, absence of distant metastases and early symptomatology of bronchial obstruction. This report deals with the autopsy findings in a 67-year-old smoker in whom a typical endobronchial carcinosarcoma was found in the left upper lobe. In all the published cases, as well as in our patient, the epithelial part of the tumor was found to be a squamous cell carcinoma with varying grades of differentiation. On the other hand, the mesenchymal part has not been uniformly described. In the present case, it occurred as a spindle-cell shaped sarcoma. Morphologically, carcinosarcomas are to be distinguished from carcinomas associated with pseudosarcoma. Prognosis of successfully operated patients seems to be favourable. Survivals of up to 6 years without recurrence have been reported. In our patient the carcinosarcoma was associated with a polypoid squamous cell carcinoma in the right upper lobe. No other case with such a combination has been reported.

[Waldenström's macroglobulinemia with immunocytoma of the lung].

A case of Waldenström's macroglobulinemia in a 63-year-old man is presented. The clinical course was complicated by pulmonary infiltration classified by biopsy as immunocytoma. The clinical findings and therapeutic possibilities in this rare case of Waldenström's macroglobulinemia with with pulmonary complications are discussed in the light of the literature.

[Malignant endothelial vascular tumor with distal embolization (author's transl)].

This case report describes a malignant endothelial vascular tumor originating in the superficial femoral artery with distal embolization and formation of metastases in bones and soft tissue of the foot and lower leg. Clinically the case was considered to be microembolism of the foot. Diagnosis was established by biopsy of a cutaneous necrotic lesion in the lateral foot region. The histologic picture of the cutaneous and subcutaneous metastases is similar to that of systemic proliferating angioendotheliomatosis.

Identification of uncommon amino acids in the lentil seed (Lens culinaris Med.).

The fraction of the free basic amino acids in the lentil seed was shown to contain gamma-hydroxyarginine, gamma-hydroxyornithine and homoarginine besides the common amino acids. Similar distribution was found in 5 varieties of lentil, with hydroxyarginine and arginine dominating. The significance of these findings with regard to chemotaxonomy and lentil consumption is discussed.

["Senile" chronic pancreatitis; a new nosologic entity? Studies in 38 cases. Indications of a vascular origin and relationship to the primarily painless chronic pancreatitis].

47 of 170 cases of chronic pancreatitis observed and controlled in the last 12 years in this department were of unknown etiology (idiopathic form). The age distribution of these 47 cases showed 2 peaks, a "juvenile" group of 9 cases (average age 29 years) and a "senile" group of 38 cases (average age 62 years). Idiopathic "senile" chronic pancreatitis (ISCP) is characterized by advanced age at the time of first manifestation (95% over 50 years), a prevalence of males (81%), a high incidence of calcifications (60%) and a rather benign, often painles course (74%). Clinically there is some overlap between ISCP and painless chronic pancreatitis (PSCP). In contrast to PSCP, ISCP seems, however, to comprise a homogeneous group of patients as far as etiology is concerned, and typical episodes of pancreatitis occurred in about 25% of cases with ISCP. Ther is an unusually high incidence of signs of arteriopathy associated with ISCP. Arteritis of the lower limbs and/or coronary heart disease was detected in 42% of cases, 14 of 17 deaths were due to cardiovascular disease and arteriosclerotic risk factors were found in 87% of the patients. In our experience ISCP seems to be a special form of chronic pancreatitis which can be differentiated from alcohol-induced chronic pancreatitis and from the other forms with rare causes. Furthermore, our findings suggest that a vascular factor is of importance in causing ISCP.

[Juvenile colonic mucosal polyps in adults].

The histology of all intestinal polyps removed endoscopically during the last 30 months has been reviewed. Among the 90 patients with a total of 154 polyps, 3 cases of juvenile polyps were found. All three were male adults who were aged 29, 34, and 50 years. In two patients the lesion was solitary, while one had polyposis with about 100 polyps. Macroscopically the juvenile polyps cannot be distinguished from adenomatous polyps, but their histologic appearance is typical. They are considered to be hamartomas and have no tendency to become malignant. Thus, juvenile polyposis does not represent a precancerous lesion as adenomatous polyposis does, and must therefore be treated differently. Therapy consists of endoscopic removal of all polyps. In order to differentiate juvenile from adenomatous polyps with certainty, histological examination of at least one polyp is necessary. This differential diagnosis is the indispensable precondition for appropriate therapy.

Gastrointestinal involvement in systemic mastocytosis.

Four consecutive patients with systemic mastocytosis were studied. One patient had a malabsorption syndrome with only minor histological changes of the intestinal mucosa. Another patient with ulcer diathesis had a gastric secretory pattern resembling Zollinger-Ellison syndrome. Serum gastrin and histamine levels were consistently normal in all patients. Endoscopy of stomach and colon disclosed urticaria-like papulae either spontaneously or after topical provocation in all patients. No increase of mast cells was found in multiple mucosal biopsies. A markedly increased gastric tissue content of histamine was found, however, in the three patients studied. The findings suggest that urticaria-like lesions associated with a high tissue content of histamine may be more important that hyperhistaminaemia in causing the various gastrointestinal symptoms.

Familial 'minor' adenomatous polyposis coli associated with gastric carcinoma - a hiterto undescribed phenotype of polyposis coli?

A clinical study of a family (Sch.) with a high incidence of death from gastric cancer in the second generation is presented. Endoscopic and/or surgical evaluation of the stomach, upper small intestine, and colon in 14 mainly asymptomatic members of the third and fourth generations revealed single or multiple (solitary) papillary adenomas in the colon or jejunum in 2 members each of the two generations. In addition, single or multiple hyperplastic polyps in the stomach or colon were found in 5 members, in 4 of them in association with papillary adenomas. The findings in family Sch. are consistent with 'minor' adenomatous polyposis coli associated with gastric carcinoma. Gastric carcinoma probably represents another phenotype among the vast variety of extra-colonic and extra-alimentary neoplastic changes known to occur in association with familial polyposis coli (FPC). The relationship of hyperplastic polyps to papillary adenoma in the colon and to gastric carcinoma respectively is discussed. Finally, evidence for a genetic relationship between family Sch. and a case with probable FPC is presented.
