Role of dextran sulfate in urokinase therapy and evaluation of the effects by estimation of plasmin inhibitor, fibrinogenolytic degradation products, and fibrinolytic degradation products.

The present study was concerned with changes in APTT and coagulation-fibrinolytic inhibitors in 2 groups of patients receiving either UK with heparin or UK with heparin and dextran sulfate (DS). APTT just after UK treatment showed a significant prolongation in patients receiving combination treatment with DS, but contrarily a shortening in patients receiving those with DS. This result indicated the usefulness of DS combination in UK treatment. There was a marked difference between the responses of alpha 2-PI and antiplasmin to the treatment, presumably as a result of immune complex formation of the alpha 2-PI with plasmin. However, no marked difference in alpha 2-PI and AT-III existed both just after and at 24 hours after UK treatment in patients receiving UK (240,000 units) with both heparin and DS and those receiving UK (480,000 units) with heparin. Those results suggest that the dosage of UK can be reduced by combination of DS. Successful UK treatment in clinical cases resulted in a decrease of alpha 2-PI and an increase of fibrinogenolytic or fibrinolytic degradation products. For these reasons, effective UK treatment appears to require combination with DS and several treatment days under the condition with a decrease of alpha 2-PI (50%) and fibrinogenolysis with fibrinolysis, which are followed by administrations of UK.

[Computed tomography in developmental anomalies of craniocervical junction].

We reviewed 27 patients with developmental anomalies in craniovertebral junction, and special attention was paid to computed tomography (CT) findings in congenital atlantoaxial dislocation (AAD), basilar invagination and Chiari malformations. In ADD, CT clearly demonstrated the relationships of the atlas to the axis in axial plane. Four major types were distinguished; anteroposterior (6 cases), transverse (0 case), anteroposterior-transverse (4 cases) and rotatory (5 cases) dislocations. It was feasible by CT to analyse the pathomechanics in each individual with AAD. Soft tissue wad was shown on the posterior aspect of the odontoid in 7 out of 15 patients with AAD. Those two features, pathomechanics of AAD and intraspinal soft tissue wad, were felt to be important for patient's symptomatology. Basilar invagination presented in no patient as a single deformity and was always associated with other bony and neural abnormalities in 14 patients. Basilar invagination might be suggested of its existence in such a case as CT showing; (1) the odontoid at or above the level of the foramen magnum, (2) distinctive margin of the foramen magnum above the bottom of the posterior fossa, and (3) the posterior arch of the atlas at the same level as foramen magnum. Nevertheless, it is essential to confirm occipital hypoplasia by using either reformatting CT images in coronal and sagittal plane or conventional tomography; whereas high resolutional CT machine provides new dimension for demonstration of bony details. In Chiari malformations, plain CT scanned by high resolutional machine often makes it possible to delineate caudal migration of the cerebellar tonsils, so that it may be a choice of examination as a screening of the anomaly. In addition, metrizamide CT clearly demonstrated caudal migration both of cerebellar tonsils and medulla oblongata. Therefore, CT apparently exceeds other diagnostic modalities in evaluating this anomaly.

[Reconstructive operation of Moyamoya disease and its problems (author's transl)].

Cerebrovascular moyamoya disease is a chronic occlusive disease of unknown etiology, 90% of which are found among Japanese. At the 3rd International Symposium on Microsurgical Anastomosis held in 1976, Kikuchi and Kawasawa reported an preventing effect of STA-MCA bypass surgery for ischemic episode of this disease. Thereafter, they proposed a new operation to place temporal muscle on the brain surface, namely Encephalo-Myo Synangiosis (EMS), for the patients lacking suitable recipient artery of middle cerebral artery territory. The authors report clinical experience of eight STA-MCA anastomosis, two STA-MCA anastomosis with EMS and one EMS performed on six cases with moyamoya disease. The follow up period after operation ranged from seven to 18 months. Four patients had excellent results, and one good. One died of subdural hematoma of non-operated side seven months after contralateral bypass procedure. STA-MCA bypass surgery is an effective treatment for moyamoya disease. Also, EMS is expected to be a replaceable surgical procedure for STA-MCA bypass under absence of suitable recipient artery.

Assay of heparin in plasma using a chromogenic substrate and its clinical applications.

The concentration of heparin in plasma was measured using a chromogenic substrate. It appeared that the measurement of heparin concentration in plasma was important in therapeutic control and evaluation of heparin. There was a correlation between the activated partial thromboplastin time (APTT) and heparin concentration in plasma, but since the gradients of regression line differed in each of the cases, measurement of APTT alone would give a different estimate of heparin concentration in each case. Fibrinopeptide A (FPA) is considered as the best indicator for evaluatin of therapeutic effects of heparin. Therapeutic heparin concentrations were defined as ranging from 0.2 to 1.2 IU/ml because the normalization of FPA was observed and there happened no hemorrhagic accidents in that range.

Computerized tomography findings in dural arteriovenous malformations. Report of three cases.

Changes in the brain parenchyma and cerebrospinal fluid spaces were demonstrated in three cases of dural arteriovenous malformations by computerized tomography (CT). In various combinations, there were 1) vermiform or patchy enhancement after intravenous contrast infusion, 2) abnormal decreased density mainly in the white matter, 3) local mass effect, 4) hydrocephalus, 5) dilatation of the major venous sinus system, and 6) prominent vascular grooving of the skull inner table. Most of these changes were secondary to raised sinus pressure caused by arteriovenous shunt. It was also demonstrated by serial CT that some elements improved after artificial embolization or surgical excision of the malformations.

Catheter cervical vertebral venography.

Cervical vertebral venography using catheterzation via the femoral vein was carried out in 78 cases. In disc disease the internal vertebral venous plexus (IVVP) are interrupted at single or multiple levels, and sometimes completely blocked. In ossification of posterior longitudinal ligament, the IVVP are separated bilaterally and interrupted at multiple levels due to centrally situated ossification and associated osteophytes. Venography helps to determine the upper and lower limits of laminectomy, and the necessary sites of facetectomy. The IVVP in the thoracic region were clearly opacified in 86% of 37 normal selective azygograms.

Sequential delayed enhanced CT in brain tumors.

Delayed enhanced CT was performed in 53 cases and these findings are compared with those of delayed radioisotope scan and with operative and histopathologic findings. We have classified the results of enhancement into four types as follows: (1) immediate enhancement pattern due to rich vascular bed, typical in meningiomas; (2) delayed enhancement pattern due to exudation, often seen in cystic astrocytomas; (3) intermediate sustained pattern due to both mechanisms in glioblastoma multiforme; (4) no enhancement pattern as in diffuse astrocytomas. The results of delayed enhanced CT correlated well with those of radioisotope scan.
