Fibromatosis of the Breast: A Case Report.

Fibromatosis develops in many anatomic sites, but it rarely arises as a primary lesion in the breast. This lesion is locally invasive and frequently recurs after a local excision, but it has no potential for distant metastasis. In this report, we present a case of mammary fibromatosis which was closely similar to carcinoma in clinical, mammographic and ultrasonographic findings, thus leading us to breast conserving surgery. Despite being a rare disease, fibromatosis should be included in the differential diagnosis of younger patients (the age of the present case was 51, and the mean ages of patients with fibromatosis ranges from 37 to 49) with abnormal changes on physical examinations and imaging studies.

Isolation and sequencing of two alpha-globin genes alpha(A) and alpha(D) in pigeon and evidence for embryo-specific expression of the alpha(D)-globin gene.

By screening a pigeon genomic DNA library, we isolated a recombinant phage clone containing the alpha(A)-globin gene. The DNA sequence of the approximately 6kbp-long insert fragment of the phage clone was determined. The sequence suggested the existence of pigeon alpha(D)-globin gene located 3.1 kbp upstream from the alpha(A)-globin gene. The expression of the alpha(D)-globin in late embryo was also shown by the N-terminal amino-acid sequence of the intact globin chain. These results show that two adult alpha-globin genes, alpha(A) and alpha(D), exist in the pigeon genome, and the alpha(D)-globin is expressed at the late embryo stage. The stage-specific expression suggests the existence of regulatory elements and factors interacting to inhibit transcription at the adult stage.

External calcium-dependent, F-actin-independent and pertussis toxin-insensitive novel neutrophil locomotion induced by a mAb.

We previously demonstrated that a mAb to human neutrophils, designated 3H9, which was established by screening for inhibition of neutrophil adherence to plastic plates containing fetal bovine serum, enhanced both n-formyl-methionyl-leucyl-phenylalanine (FMLP)-induced chemotaxis and random migration of neutrophils. In the present study, we examined the mechanisms of 3H9-induced enhancement of neutrophil locomotion in the phagokinetic track assay. 3H9-induced neutrophil locomotion maintained a straight path which was different from the track resulting from FMLP-stimulated locomotion. This 3H9-induced migration required extracellular Ca2+. beta 2-Integrin activation was a prerequisite for the increase in cytosolic free calcium induced by 3H9 treatment. However, stimulation by 3H9 did not induce an increase in F-actin, even after CD18 activation. Signal transduction after molecular recognition by 3H9 was not mediated by pertussis toxin-sensitive, heterotrimeric G proteins. These results suggest that 3H9 enhances neutrophil migration by mechanisms which are different from those involved in usual chemoattractant-induced migration. Neutrophil surface molecules recognized by 3H9 may play a crucial role in the regulation of transendothelial migration of leukocytes.

Spindle cell carcinoma of the breast.

Spindle cell carcinoma is a rare breast tumor. We present herein three cases of spindle cell carcinoma of the breast and review its characteristics from the literature. Spindle cell carcinoma frequently forms a large and well-circumscribed tumor with gross cyst formation. Histologically, its dominant component is of sheets of spindle shaped cells, and it includes such contiguous carcinoma components as squamous differentiation or invasive ductal carcinoma. Estrogen receptor expression and lymph node metastasis tend to be low. Despite the sarcomatous features, spindle cells are likely to be derived from epithelial cells of mammary glands. Immunohistochemical and ultrastructural examination demonstrated the expression of keratin and the desmosome-like junctional structure in the spindle cell components. Relatively favorable prognosis is expected in spindle cell carcinoma of the breast compared to common breast carcinoma.

A monoclonal antibody modulates neutrophil adherence while enhancing cell motility.

A monoclonal antibody (MoAb) to human neutrophils, designated 3H9, was established by screening for the inhibition of neutrophil adherence to plastic plates containing a medium supplemented with fetal calf serum (FCS medium). The antigen recognized by 3H9 was shown to be present on human leukocytes and found at the highest levels on granulocytes. On Western blotting, 3H9 reacted with a molecule having a molecular weight of 80 kDa. When this MoAb was added at the same time as a neutrophil stimulant (fMLP), the inhibition of neutrophil adherence to plastic plates in the presence of FCS medium was observed after 60 min incubation. Furthermore, this MoAb enhanced not only fMLP-induced chemotaxis but random migration of neutrophils as well. The mechanisms of these phenomena are discussed.

Rapid killing of human neutrophils by the potent activator phorbol 12-myristate 13-acetate (PMA) accompanied by changes different from typical apoptosis or necrosis.

To elucidate the relationship between activation of neutrophils and their subsequent death, the effect of phorbol 12-myristate 13-acetate (PMA), a potent activator of neutrophils, was examined. PMA-treated neutrophils showed morphological changes quite different from those of typical apoptosis or necrosis. After fusion of the lobate nucleus, nuclear contents of chromatin uniformly decreased in compactness and soon after the nuclear envelope was broken. Even at this stage, cytoplasmic organelles did not undergo degeneration. Membrane permeability began increasing at 3 h of incubation with PMA, subsequent to nuclear change. Conventional agarose gel electrophoresis and pulsed field gel electrophoresis of DNA from PMA-treated neutrophils revealed no DNA degradation products smaller than 300 kbp. PKC inhibitors, staurosporine and H-7, prevented cytotoxicity by PMA. Furthermore, antioxidants, thiourea, dimethylthiourea, pyrrolidinethiocarbamate, and N-acetyl-L-cysteine, but not superoxide dismutase, were also active in preventing PMA cytotoxicity, suggesting that cell suicide resulting from PMA treatment is due to oxygen radicals, especially the hydroxyl radical. A certain population of neutrophils phagocytosing opsonized zymosan also showed changes similar to those observed in PMA-treated cells.

Continuous monitoring of short-latency somatosensory evoked potentials during cardiac and aortic surgery.

The effectiveness of monitoring somatosensory evoked potentials (SEPs) intraoperatively to detect brain damage early remains controversial. To assess the diagnostic accuracy of this modality, a study was conducted between 1991 and 1994, recording SEPs in 287 consecutive patients undergoing cardiac and aortic surgery using cardiopulmonary bypass (CPB) with moderate hypothermia or deep hypothermic circulatory arrest. From P1 to N2 of the SEPs occurring within 50 ms latency in response to electrical stimulation of the median nerve were recorded over the contralateral postcentral cortex at 5-min intervals using a Neuropack-2 (Nihon Koden, Tokyo, Japan). Normal SEPs were recovered in 247 patients postoperatively; however, 2 of these patients had suffered a cerebral infarction and 1, a transient stroke intraoperatively, demonstrating a false-negative incidence of 1.2%. On the other hand, three different types of abnormal SEPs were recorded postoperatively. P1 and N1 absence, probably caused by a subcortical lesion, was observed in 4 patients; P2 and N2 absence, probably caused by a cortical lesion, was observed in 8 patients; and a flat SEP, representing diffuse damage, was observed in 2 patients. Among these 14 patients with abnormal SEPs, 7 showed no neurologic disturbance at all, demonstrating a false-positive incidence of 50%. Thus, we concluded that when normal SEPs are recovered during weaning from CPB, the incidence of brain damage could be predicted at below 5%. Conversely, when abnormal SEPs are demonstrated, the incidence of brain dysfunction impeding a return to active life is estimated to be about 70%.

Amino acid sequences of hemoglobin beta chains of five species of pinnipeds: Neophoca cinerea, Otaria byronia, Eumetopias jubatus, Pusa hispida, and Pagophilus groenlandica.

Pinnipeds (Otariidae, Odobenidae, and Phocidae) in the order Carnivora have one or two types (Hb I and Hb II) of hemoglobin components. These hemoglobins consist of identical beta chains and different alpha chains. We determined the complete amino acid sequences of the hemoglobin beta chain of three species of Otariidae (Australian sea lion, South American sea lion, and northern sea lion) and two species of Phocidae (ringed seal and harp seal) from intact beta chain and chemical cleavage fragments. The sequences are similar to beta chains of the already known sequences of pinnipeds. These sequences were compared with those of other carnivores (Mustelidae, Ursidae, Canidae, and Felidae) and adult human hemoglobin beta chain. Using Artiodactyla (pig) as an outgroup, we find that the tree constructed by means of phylogenetic analysis shows that Odobenidae is closest to Otariidae, and that Otariidae and Odobenidae are closer to Mustelidae than to Phocidae.

Brain perfusion SPECT in neuro-Behçet's disease: discordance between Tc-99m-HMPAO and Tc-99m-ECD.

A patient with neuro-Behçet's disease was studied with both Tc-99m-HMPAO and Tc-99m-ECD brain perfusion SPECT during the same time period. In Tc-99m-HMPAO SPECT, focal high uptake was observed in the left basal ganglia where MRI depicted abnormal signal intensity. Conversely, Tc-99m-ECD SPECT did not show corresponding high uptake, but demonstrated rather low uptake in contrast to the Tc-99m-HMPAO SPECT. This case suggests that Tc-99m-HMPAO and Tc-99m-ECD may show discordant distribution in inflammatory brain disease such as neuro-Behçet's disease.

[Clinical analysis of embolism with left atrial myxomas].

We conducted a retrospective analysis examining the relationship between the tumor quality and the occurrence of systemic embolization in 16 patients who underwent surgical excision of the left atrial myxoma. Systemic embolization occured in 10 patients (63%) before surgery, in 5 of whom embolic episodes were the first manifestation of the tumor. Cerebral infarction had developed in 7 patients, TIA in 2, myocardial infarction in 2, limb ischemia in 2, and multiple infarctions in the kidney, adrenal gland, spleen and bone marrow in one. A permanent deficit remained in 6 patients (60%). Myxoma was a lobulated and gelatinous type in 9 (90%) of these patients with embolization and a solid type only in one case. When the dignosis of the left atrial myxoma with soft and fragile quality is made by echocardiography, surgical excision should be performed without delay to prevent systemic embolization.

[Traumatic thoracic aortic rupture--diagnosis and surgical repair].

Fifteen cases of traumatic thoracic aortic rupture (TAR) were treated at St. Marianna University Hospital from December 1980 to July 1995. Causes of TAR were due to vehicle accidents in 14 patients and fall in one patient. On diagnosis, contrast-enhanced CT scan was routinely performed in the patients with blunt chest trauma associated with superior mediastinal widening, loss of the aortic knob or right shift of the trachea on the initial roentgenogram. When CT scan demonstrated specific signs for TAR, pseudoaneurysm formation and/or extravasation of the contrast dye, aortography was eliminated before operation. As a role, operation was performed on an emergency basis as soon as the diagnosis was confirmed. Four cases died due to intrapleural rupture before or immediately after thoracotomy. Nine (82%) of the 11 patients in whom operation was completed survived and are doing well. In one of the 4 patients who underwent operation with simple aortic cross-clamping; paraplegia developed after 30 minutes of spinal ischemia. Left heart bypass with the Bio-Pump without heparin or with an antithrombin agent, argatroban, was used in recent 6 patients. Use of left heart bypass with the Bio-Pump without anticoagulant or with argatroban appears to be promising as a safe adjunct in the repair of TAR, preventing fatal bleeding of other injured organs.

[TNF/TNFR gene and apoptosis].

Intracellular signal transduction pathways induced by in the reaction of TNF and TNF receptors has been clarified through finding of various TNF receptor associated intracytoplasmic molecules. The possible role of these signal transduction pathways for apoptosis induced by TNF is discussed.

[A case of catamenial pneumothorax with elevated level of serum CA 125].

A 40-year old woman had experienced repeated spontaneous pneumothorax on the right side at the beginning of each menstrual period for one year. The level of serum CA125 was high at 60 ng/ml. Right thoracotomy revealed the presence of multiple darkly-pigmented spots and several pinhole like perforations near the central tendon of the diaphragm. The partial resection of the diaphragm was done, endometrial tissues were detected histologically. Tha patient has been asymptomatic after operation and the level of serum CA125 is normal.

Regulation of neutrophil apoptosis--its biological significance in inflammation and the immune response.

Neutrophils play a pivotal role in host defence against bacterial infection. Their life span is short compared with that of leukocytes of other lineages. Neutrophils are programmed to die by apoptosis at the time of differentiation. However, recent studies have demonstrated that environment also has a great influence on apoptosis of these cells. Based on our recent experimental results, we present here a review of studies on the regulation of neutrophil apoptosis and discuss its biological significance in inflammation and the immune response.
