In vivo regulation of rat neutrophil apoptosis occurring spontaneously or induced with TNF-alpha or cycloheximide.

We previously demonstrated that human TNF-alpha induces rapid apoptosis of human neutrophils. To understand better the in vivo significance of neutrophil apoptosis, we examined spontaneous, recombinant human and mouse TNF-alpha- or cycloheximide-induced apoptosis of normal peripheral blood neutrophils (PBN), PBN from rats injected i.p. with proteose peptone or a streptococcus preparation, OK-432 (inflammatory PBN), peritoneally exudated neutrophils (PEN) obtained after a proteose peptone injection, and normal bone marrow neutrophils. The following observations were made. 1) Normal PBN responded to TNF-alpha, but PEN, normal bone marrow neutrophils, and inflammatory PBN at 12 h after stimulation did not. 2) The sensitivity to TNF-alpha of the inflammatory PBN started to decrease at 3 h, was lowest at 12 h, and was almost restored at 52 h after stimulation. 3) Spontaneous apoptosis of normal and inflammatory PBN reached 25% at 12 h after in vitro incubation, but that of PEN and normal bone marrow neutrophils was very low over this period. 4) The sensitivity to cycloheximide (6 h incubation) was high for normal PBN and bone marrow neutrophils, but low for PEN and inflammatory PBN after 12 h. 5) 125I-rhTNF-alpha binding of bone marrow neutrophils was significantly lower than that of normal and inflammatory PBN and PEN. 6) TNF-alpha-induced apoptosis of normal or inflammatory PBN and bone marrow neutrophils was enhanced by treatment with low doses of cycloheximide that alone were barely able to induce neutrophil apoptosis; however, apoptosis of PEN was not. The mechanisms and in vivo significance of these phenomena are discussed.

Screening for abdominal aortic aneurysm and occlusive peripheral vascular disease in Japanese residents.

To evaluate the prevalence of abdominal aortic aneurysm (AAA) and occlusive peripheral vascular disease (PVD) in Japanese residents, and to examine the correlations between these diseases and the risk factors of atherosclerosis, 348 residents of a village in central Japan aged between 60 and 79 years were screened. The screening for AAA was performed using ultrasonography (US) and that for PVD was performed by palpation and Doppler US. No AAA was found, and a right common iliac arterial aneurysm was detected in a 79-year-old man (0.3%). The mean diameter of the infrarenal abdominal aorta was 18.7 mm and an abdominal aorta of 25 mm or greater in diameter was seen in 16 participants (4.6%), all of whom need to be followed up. PVD was suspected in two patients (0.6%) with a low ankle brachial pressure index. Of a total of five patients diagnosed or suspected of having a common iliac arterial aneurysm or PVD, four (80%) had at least one risk factor for atherosclerosis. Thus, we conclude that Japanese residents with risk factors predisposing them to atherosclerosis such as hypertension, obesity, abnormal serum lipid levels, and a history of smoking should be selectively screened for AAA and PVD due to the low prevalence of these diseases and from the viewpoint of cost-effectiveness.

Transesophageal echocardiographically detected atherosclerotic aortic debris in a patient with systemic embolism following coronary angiography.

Transesophageal echocardiography (TEE) has enabled detection of the cardiac source of systemic emboli. We report the case of a patient who manifested systemic, multiple embolization in the kidney, skin, and upper gastrointestinal tract following coronary angiography. TEE allowed visualization of the atherosclerotic debris in the thoracic aorta. The clinical picture of the patient was consistent with that of cholesterol embolism. We recommend that patients with extensive atherosclerotic disease should undergo TEE before cardiac catheterization or other invasive procedures involving the aorta are carried out.

Seventeen alpha-hydroxylase deficiency with one base pair deletion of the cytochrome P450c17 (CYP17) gene.

Mutation of the cytochrome P450c17 (CYP17) gene causes 17 alpha-hydroxylase deficiency (17OHD). Recently, several researchers have elucidated the molecular basis of 17OHD by gene analysis. We experienced a case of 17OHD and intended to reveal the abnormality of the CYP17 gene in this Japanese female with 17OHD. Leukocytes were obtained from the patient, her mother and sister, and normal control subjects. We amplified the CYP17 gene using polymerase chain reaction and performed the sequence analysis using the dideoxy terminator method and restriction enzyme analysis. We found that the patient had one base-pair deletion at the position of amino acid 438. An identical result was obtained with restriction enzyme analysis. This G deletion altered the reading frame and resulted in a premature stop codon at position 443; the ligand of heme iron (Cys: cystine 442) was absent. This small mutation may account for the patient's clinical manifestations of 17OHD. This is the first case of 17OHD with only one base pair deletion of the CYP17 gene.

Cavernous sinus invasion by pituitary adenomas--relationship between magnetic resonance imaging findings and histologically verified dural invasion.

The relationship between magnetic resonance (MR) imaging findings and histologically verified invasion of the cavernous sinus by tumor cells was studied in 26 patients treated surgically for pituitary adenoma. Dural invasion of the sellar floor by tumor cells was found in 10 cases (38%). All patients were classified according to MR imaging findings into three types. Type I showed a gadolinium-enhanced stripe medial to the carotid artery (5 patients), none of which showed dural invasion. Type II showed no enhanced stripe (17 patients), six of which showed dural invasion. Within this type, tumor size and dural invasion showed no correlation. Type III showed displacement or encasement of the carotid artery by the tumor with or without extracranial extension (4 patients), all of which showed massive infiltration of the tumor cells into the dura mater. This study shows that preoperative MR imaging can provide information for assessment of invasion into the cavernous sinus in patients with pituitary adenoma.

Amino acid sequence of alpha- and beta-polypeptide chains of turkey (Meleagris gallopavo) hemoglobin.

Two hemoglobin components are recognized in erythrocytes of the adult turkey (Meleagris gallopavo). We determined the amino acid sequences of turkey alpha A-, alpha D- and beta-globin from intact globin chains and chemical cleavage fragments. The sequences are highly similar to the hemoglobin of the Phasianidae, chicken, Japanese quail and pheasant. Turkey and pheasant beta-globin are identical. The amino acid sequence of turkey alpha A-globin differs by only one residue from chicken alpha A-globin. Phylogeny trees from alpha A-, alpha D- and beta-globin were constructed by the neighbor-joining method. Although the trees generated from alpha A- and beta-globin were similar, that from turkey alpha D-globin differed.

[Higher cortical dysfunctions and image diagnosis in a 9-year-old girl with complex partial epilepsy caused by focal neural migration disorder].

A 9-year-old right-handed girl was admitted because of complex partial seizures, left-right disorientation and finger agnosia. At the age of 2 years, she began to have seizures, which were exacerbated by carbamazepine and diazepam. Subsequently she was treated with phenytoin and phenobarbital, and remained seizure-free for four years. After age 7, she began to have attacks of alteration of consciousness, which lasted 10 seconds and occurred every two or three months. At age 9, neuropsychological testing revealed borderline intellectual functioning (WISC-R:FSIQ 83, VIQ 94, PIQ 73), but selective deficits were found in tests of calculation and spatial-figural relationships. Axial spin echo (3,000/22) image showed an abnormal high signal intensity in the left temporo-parieto-occipital lobe, in which interictal IMP-SPECT demonstrated decreased cerebral blood flow. These results seemed to explain the relationship between clinical findings and focal migration disorder. It is important to detect a focal migration disorder in patients with intractable focal epilepsy or higher cortical dysfunctions.

Prognostic significance of c-erbB-2 expression in invasive ductal carcinoma of the breast.

The expression of c-erbB-2 oncoprotein was studied in relation to the histological type of invasive ductal carcinoma. The expression of c-erbB-2 was found in 26 (17.6%) of 148 cases, and was a significant prognostic indicator in patients with lymph node metastasis, but not in those without. The patients were divided into a scirrhous group and a non-scirrhous group on the basis of histological type. There was no significant difference between the two groups in age, clinical stage, lymph node status, estrogen receptor status, operation method or c-erbB-2 expression. The expression of c-erbB-2 was a significant prognostic indicator in the scirrhous group, irrespective of lymph node metastasis, but not in the non-scirrhous group. We conclude that the prognostic significance of c-erbB-2 expression differs among histological types of invasive ductal carcinoma, and that c-erbB-2 expression is a prognostic indicator in patients with scirrhous carcinoma.

[Adjuvant endocrine therapy for breast cancer].

As reported by the Early Breast Cancer Trialists' Collaborative Group (EBCTCG) in Lancet (1992), the 10-year relapse-free survival or overall survival was significantly better in the tamoxifen (TAM) treatment group than in the control group in both node-positive and -negative groups. Adjuvant TAM treatment works better on patients over 50 years of age. Adjuvant TAM treatment for over 2 years is more effective than less than that time. Some reports demonstrated that long-term TAM treatment caused an increase in the circulating estrogen levels of premenopausal patients. This increase blocked the antitumor effect of TAM in a dose dependent fashion in our animal models. Similar results were obtained in toremifene or TAT-59 in our animal models. These results show that antiestrogens are more effective for postmenopausal than premenopausal women. LH-RH agonist or aromatase inhibitor may be a promising adjuvant endocrine agent of breast cancer. Combination therapy of TAM and LH-RH agonist, or LH-RH agonist and aromatase inhibitor will be an effective new adjuvant endocrine therapy for breast cancer. The combination of endocrine therapy and biological response modifiers, or of endocrine therapy with vitamin D3 will be clinically used in the future.

[Parathyroid carcinoma].

We collected 60 cases of parathyroid carcinoma, which was a rare disease of primary hyperparathyroidism, from 1980 to 1989, at several institutes in Japan by a questionnaire. The incidence of parathyroid carcinoma among the cases of the primary hyperparathyroidism in Japan was 4.8%, which was a little higher than that in the western countries. Most of the patients with parathyroid carcinoma show bone disease, high level of serum calcium and palpable anterior neck mass. Echogram is useful for the diagnosis of parathyroid carcinoma and its localization. Cure of the disease depends on the initial operation methods. En bloc resection of the tumor should be performed in each suspected case of a parathyroid carcinoma. The incidence of distant metastasis was 24%, and local recurrence 20% in Japan. Compared with the western countries, the lower incidence of local recurrence in Japan may be due to the operation method. The resection of local recurrence or distant metastases is effective for the control of recurrent hypercalcemia. We should always consider parathyroid carcinoma at the time of diagnosis and treatment of the patients with primary hyperparathyroidism.

Active site lysine backbone undergoes conformational changes in the bacteriorhodopsin photocycle.

Results are presented demonstrating that the backbone of the active site lysine of bacteriorhodopsin undergoes light-induced structural alterations during bacteriorhodopsin-mediated light-induced proton pumping. This conclusion is based on difference Fourier transform infrared spectroscopy of isotopically labeled bacteriorhodopsin. The data demonstrate that the backbone carbonyl of lysine achieves an extremely low vibrational frequency during M412 intermediate formation. This is preceded by a structural transition in the lysine backbone that leads to an active site lysine carbonyl with the observed low vibrational frequency, probably due to a high degree of solvation.

A new triphenylethylene derivative, TAT-59; hormone receptors; insulin-like growth factor 1; and growth suppression of hormone-dependent MCF-7 tumors in athymic mice.

TAT-59 ((E)-4-[1-[4-[2-(dimethylamino)ethoxy]-phenyl]-2-(4- isopropyl)phenyl-1-butenyl]-phenyl-monophosphate) treatment was performed on hormone-dependent MCF-7 tumors in athymic mice. TAT-59 given at 1, 5, and 20 mg/kg inhibited the estrogen-stimulated growth of MCF-7 tumors in athymic mice in a dose-dependent fashion. The most clear decrease in tumor growth was shown in the TAT-59 alone group, although it was not dramatic. Average serum concentrations of DP-TAT-59((Z)-[1-[4-[2-(dimethylamino)- ethoxy]phenyl]-2-(4-isopropyl)phenyl-1-butenyl]-4-hydroxybenzene) and DM-DP-TAT-59(desmethyl-DP-TAT-59), metabolites of TAT-59, increased in a dose-dependent manner. Much higher levels of DP-TAT-59 and DM-DP-TAT-59 were shown in tumors (target tissues of estrogen) as compared with muscles (nontarget tissues of estrogen) or serum. A serum concentration of DP-TAT-59 or DM-DP-TAT-59 corresponding to the physiologic levels of serum estradiol in premenopausal women was sufficient to inhibit the estrogen-stimulated growth of MCF-7 tumors in mice. TAT-59 induced a dose-dependent increase in estrogen receptor levels in the MCF-7 tumors. In contrast, it prevented the estradiol (E2)-induced increase in progesterone receptor levels in a dose-dependent manner. Insulin-like growth factor 1 levels measured in the MCF-7 tumors significantly decreased in the TAT-59 alone group and in the no treatment group as compared with the E2 alone group. These results show the pronounced antiestrogenic action of TAT-59 on hormone-dependent MCF-7 tumors in athymic mice.

Amino acid sequence of the alpha A- and beta-polypeptide chains of the Ryukyu rufous turtle dove (Streptopelia orientalis Stimpsoni) hemoglobin.

Only one hemoglobin component is recognized in the erythrocytes of the adult Ryukyu rufous turtle dove. We have determined the amino acid sequence of the alpha A-globin and the beta-globin by conventional protein sequence analysis. The sequences of both the alpha A- and the beta-globins were highly similar to domestic pigeon hemoglobin. The alpha A-globin has 6 exchanges and beta-globin has 4 exchanges compared with the corresponding domestic pigeon chains.

Expression of proliferating cell nuclear antigen in invasive ductal carcinoma of the breast.

Formalin-fixed, paraffin-embedded sections from 92 breast cancers (invasive ductal carcinomas) were immunostained with a monoclonal antibody against proliferating cell nuclear antigen (PCNA). The labeling index of PCNA ranged widely from 2 to 76 (mean 24.8)%. The labeling index was classified into three groups: low (< 25%), intermediate (25-50%), high proliferation (> 50%). Younger patients seemed to have a higher labeling index than older ones. The labeling index for tumors < or = 2 cm was lower than that of tumors larger than 2 cm. The labeling index in patients with high estrogen receptor (ER) levels (> 100 fmol/mg cytosol protein) was significantly lower than that in patients with low ER levels (< 10 fmol/mg cytosol protein). In relation to histological type, the labeling index of scirrhous carcinoma was significantly lower than that of papillotubular carcinoma or solid-tubular carcinoma. The high proliferating group had a significantly worse overall survival rate than the low proliferating group. Labeling index was shown to be a possible prognostic indicator.

[Expression of PDGF-A and -B in human coronary atherosclerotic lesion: immunohistochemical and in situ hybridization study].

We studied the role of platelet-derived growth factor (PDGF) on the development of atherosclerosis of human coronary arteries of 63 autopsied cases. Smooth muscle cells in fibrocellular intimal thickening lesion showed no significant immunohistochemical reaction of antibodies for PDGF-A or PDGF-B or PDGF-receptor. In atherosclerotic lesions, foam cells derived from macrophages and smooth muscle cells showed intense immunohistochemical reaction with antibodies of PDGF-B and PDGF-receptor, but not with that of PDGF-A. By in situ hybridization, no significant signals of PDGF-A or PDGF-B or PDGF-receptor were demonstrated in proliferating intimal smooth muscle cells in fibrocellular intimal thickening lesions. Uncomplicated atherosclerotic lesions expressed significant amount of m-RNA of c-sis protooncogene and PDGF-B receptor in foam cells. However, no significant signals of PDGF-A were observed in uncomplicated atherosclerotic lesions. These results suggest that foam cells-producing PDGF-B play an important role for the progression of atherosclerotic lesions in human coronary arteries.
