[Endoscopic stone disintegration using the Lithoclast].

We present our experience with a new and unique device for performing intracorporeal lithotripsy known as the Swiss Lithoclast. This simple device uses compressed air to activate a solid probe in a manner of a jackhammer. This device was used to treat 20 patients with stones in the ureter (11), bladder (5) or a Kock pouch (4). The Lithoclast successfully fragmented 90% of the calculi independent of the composition and salvaged 2 failed ESWL treatments. At one month after the procedure, either alone or in combination of with ESWL, 17 of 20 patients became free of stones. There were no complications related to use of the device. We have found the Swiss Lithoclast to be a safe and effective means of performing intracorporeal lithotripsy for urinary calculi.

Effects of lymph node stromal cells on the differentiation of a cell line established from an atypical prolymphocytic leukemia with phenotypes of both chronic lymphocytic and hairy cell leukemias.

We investigated the effects of stromal cells, obtained from lymph nodes with various lymphoid disorders and characterized immunocytochemically as fibroblasts, on the maturation of a cell line "HN" established from an atypical PLL (prolymphocytic leukemia) having phenotypic characteristics of both CLL (chronic lymphocytic leukemia) and HCL (hairy cell leukemia). Coculture with stromal cells, irrespective of the kind of lymphoid disease affecting the lymph node, induced a plasmacytoid cytology in HN cells, an increase of cIg gamma, and decreases of sIg gamma, CD5, CD21, HC2, and B-ly7. In contrast, coculture with the stromal cells produced no marked effects on hairy cells from two HCL patients. Similarly, coculture with stromal cell supernatant or with various cytokines shown to be produced by bone marrow stromal cells produced no effects on HN cells. We propose that lymph node stromal cells play an important role in the differentiation of B cells through direct contact and that HN cells will be useful for investigating the differentiation pathway of chronic B cell leukemia cells.

[Prenatally diagnosed bilateral multicystic dysplastic kidneys associated with multiple anomalies: a case report].

A case of bilateral multicystic dysplastic kidneys with multiple anomalies is reported. Prenatal ultrasonography showed oligohydramnios, atrial septal defect, bilateral multicystic kidneys, omphalocele, and bowel dilatation. A male baby died of respiratory insufficiency immediately after premature delivery. Autopsy showed multiple anomalies of face, fingers, lung, heart, bowels, and genitourinary tract. Seven more cases with urinary tract anomalies prenatally detected by ultrasonography are also reported. Ultrasonography is useful to diagnose anomalies of fetus.

[Autonomic nervous function in progressive supranuclear palsy--comparison with Parkinson's disease and healthy controls].

This study evaluated the autonomic nervous function in 6 patients with progressive supranuclear palsy (PSP). The autonomic nervous functions in PSP patients were compared with those in 17 patients of Parkinson's disease (PD) and 9 age-matched healthy control subjects. Results were that all PSP patients and 59% of PD patients had sympathetic skin response (SSR) abnormalities. Significant abnormalities in cardiovascular response observed in PD patients suggested the presence of sympathetic and parasympathetic disturbances. There were no significant differences between PSP patients and control subjects in cardiovascular responses. But some of PSP patients showed abnormal cardiovascular responses compared with the results from the control subjects. In PSP patients mild disturbances in cardiovascular responses and sudomotor dysfunction were suggested. We consider that the high incidence of SSR abnormality is at least partially related to the presence of disturbances in the frontal lobes.

[Bladder hernia associated with bladder diverticulum in a female].

A 77-year-old woman with a previous history of pelvic fracture had suffered from recurrent cystitis. In the excretory urography, post-void upright film revealed the bladder hernia associated with the bladder diverticulum. Transurethral incision and fulguration of the bladder diverticulum and left inguinal herniography was performed. There has been no recurrence since then.

Efficacy of a novel injectable cephalosporin, Cefclidin, on the experimental complicated urinary tract infections with urinary stones caused by Pseudomonas aeruginosa and Proteus mirabilis.

We evaluated the effects of a novel cephalosporin, cefclidin (CFCL) and imipenem (IPM), on the eradication of bacteria from the urine, bladder stones and the kidneys, and also on the prevention of the infection stone formations, in our polymicrobial urinary tract infection model of rats associated with bladder stones using IMP-sensitive or IPM-resistant Pseudomonas aeruginosa and Proteus mirabilis as a causative pathogen. CFCL completely eradicated P. mirabilis from the urine and the stone in the short-term regimen (5 days). CFCL completely eradicated both IPM-sensitive P. aeruginosa and P. mirabilis from the urine, the stones and the kidneys as compared to IPM in the long-term regimen (11 days), reflecting the superior antibacterial activity of CFCL. CFCL also significantly prevented the development of infection stones as compared to IPM in the long-term regimen. There was no significant difference in the blood urea nitrogen (BUN) values between the CFCL or IPM-treated and the non-treated groups. The cumulative recovery rate of unchanged CFCL reached 47.3% of the total dosage (20 mg/kg) within 8 hours.

[Crossed cerebello-cerebral diaschisis in olivopontocerebellar atrophy].

We reported a case of crossed cerebello-cerebral diaschisis (CCCD) in olivopontocerebellar atrophy (OPCA). A 49-year-old male was admitted with complaints of titubation, dysarthria and tremor. Examination on admission revealed exaggerated triceps, patella and achilles tendon reflexes on both sides and rigidity in left wrist. Ocular movements were slightly saccadic and speech was scanning or explosive. Finger-nose and heel-knee coordination was poor on both sides (left dominant). Cardiovascular reflex tests showed abnormal findings, suggesting insidious autonomic dysfunction. Brain CT and magnetic resonance imaging (MRI) revealed mild atrophy of the pons and cerebellum. Brain single photon emission computed tomography (SPECT) showed reduced cerebellar N-isopropyl-P-(123I) iodoamphetamine (IMP) uptake more prominent on the left than on the right side. A reduction of 123I-IMP uptake was more striking in the right thalamus, basal ganglia and frontal lobe than on the left side. The cerebellar sign which was left dominant and the left extrapyramidal sign were consistent with the side where reduction of 123I-IMP uptake was more prominent. We suggest that CCCD in our case resulted from transneuronal deactivation in the classic anatomical (cerebello-thalamo-cortical) pathway and dopaminergic (cerebello-basal ganglia-cortical) pathway. There is a possibility that CCCD reflects the development of pathological changes in OPCA.

[A case of multiple sclerosis with intractable hiccup and acute respiratory arrest].

A 41-year-old woman with multiple sclerosis (MS) manifesting optic neuritis, cerebellar ataxia and myelopathy was admitted for intractable hiccup and aggravation of sensory disturbance in both lower limbs. Magnetic resonance imaging (MRI) revealed a Gd-DTPA enhanced lesion from the level of the medulla oblongata, involving the reticular formation, to that of the vertebral body C2. She became abruptly unable to breath following aggravation of bulbar palsy and tetraplegia and mechanical ventilation was immediately initiated. The next day, voluntary breathing re-appeared but automatic respiratory failure remained unchanged. The respirator was completely removed after one month, and then the breathing was almost normal. In MS patients who develop intractable hiccup as an early symptom, it is suggested that the causative lesion is in the medulla oblongata. In such cases, we must carefully observe the clinical course because expansion of the lesion may induce respiratory failure.

[A case of incidental renal leiomyosarcoma].

A 68-year-old man visited his home doctor with the chief complaint of left flank pain. Abdominal ultrasonography revealed left hydronephrosis and space occupying lesion in the right kidney. He was referred to our hospital because of suspected left ureteral stone and for further examination. Drip infusion pyelography revealed a left ureteral stone and retrograde pyelography showed extrinsic compression and deformation of the right pyelogram. A computed tomography showed a heterogeneous solid mass, 7 x 4 cm in size, compressing the pelvicocalyceal system and growing out of the kidney. Right radical nephrectomy was performed under the diagnosis of renal cell carcinoma. The pathological diagnosis was leiomyosarcoma. Seventy-five cases of renal leiomyosarcoma have been reported in the Japanese literature and this is the third case as an incidental tumor. Because of the absence of the characteristic diagnostic signs, this malignant tumor can present diagnostic difficulty. The diagnostic problems and clinical features of renal leiomyosarcoma are discussed.

Cardiac metastasis of colon adenocarcinoma diagnosed by magnetic resonance imaging.

We report a case with right intraventricular metastasis of colon carcinoma which was diagnosed antemortem by the use of magnetic resonance imaging. Metastatic colon carcinoma of the heart is a rare diagnosis, and an uncommon finding at autopsy. We conclude that MR imaging is a valuable method for the diagnosis of the heart tumors.

[Single agent carboplatin therapy for advanced seminoma].

Between May 1990 and August 1992, 5 patients with advanced seminoma were treated with single agent carboplatin at Kyoto University. The mean follow-up period of all patients was 23 months. The clinical stage was IIA: 1, IIB: 1, IIIA: 2, IIIB1: 1. Three to 4 courses of carboplatin at 400 mg/m2, were administered intravenously over 1 hour without hydration every 3 to 4 weeks. One patient achieved complete response (CR). Four patients with a residual mass were observed, in two of them the mass disappeared and they remained free of disease. Two of the 4 patients with a residual mass relapsed 11 and 19 months after the start of treatment, and were successfully salvaged with surgery and adjuvant EP (etoposide and cisplatin) therapy. Subsequently, the overall CR rate was 60% and overall survival rate was 100%. The side effects of carboplatin were compared with those of VAB6 (vinblastine, actinomycin-D, bleomycin, cisplatin and cyclophosphamide), with which another 5 patients were previously treated at our hospital. Leukopenia and alopecia were observed in the VAB6 group with a significant difference (p < 0.05). Severe thrombocytopenia, dyspnea, skin rash, tinnitus and numbness were observed in 4 patients in the VAB6 group. However, no other symptoms but nausea and vomiting were observed in the carboplatin group. It was concluded that single agent carboplatin therapy for advanced seminoma was effective and less toxic, and it would be beneficial for the quality of life of patients, but the residual mass should be treated with surgery, cisplatin-based chemotherapy, or radiotherapy because it possibly has a recurrent potential; otherwise it should be carefully observed.

[Clinical application of extracorporeal shock wave lithotripsy using Siemens Lithostar for upper urinary tract stones].

We performed extracorporeal shock wave lithotripsy (ESWL) monotherapy on 51 patients with upper urinary tract stones using a Siemens Lithostar (C-tube) from April 1992 to April 1993. On the x-ray film obtained three months after the final ESWL treatments, 24 patients (47%) were free from stone concrements and 22 (43%) had stone fragments less than 4 mm. The over-all success rate was therefore 90.2%. No severe side effects were observed. We also examined the relationship between the degree of fragmentation seen on the immediate post-ESWL x-ray and stone status at 3 months. There was no significant correlation between them and the post-ESWL x-ray did not reflect the stone status at three months. In our experience, ESWL monotherapy using a Siemens Lithostar (C-tube) was a safe and effective treatment for upper urinary tract stones.

[Clinical evaluation of cefpodoxime proxetil in otorhinolaryngological infections].

In this study, we evaluated the clinical efficacy of cefpodoxime proxetil (CPDX-PR) in otorhinolaryngological infections. The subjects were 205 patients (85 men and 120 women) with various otorhinolaryngological infections, aged from 16 to 81 years (mean 49.2 years): 113 patients had acute infections, 25 patients had chronic infections and 67 patients had acute exacerbation of chronic infections. 1. Clinical evaluation The overall efficacy rate was 75.6%. When classified by disease, the efficacy rate was 84.9%, 60.0%, 65.6% in acute infections, chronic infections and acute exacerbation of chronic infections, respectively. 2. Bacteriological evaluation Frequencies of isolation of different organisms were studied: 49 strains of Staphylococcus aureus, 27 strains of Staphylococcus sp. and 15 strains of Streptococcus sp. were found in the decreasing order of frequencies. Antibacterial activities against S. aureus, Staphylococcus sp. and several other organisms were compared among CPDX-PR, ampicillin, cefaclor, cefteram and norfloxacin: CPDX-PR showed the highest activity. 3. Side effect Mild urticaria was observed in only 1 patient. Abnormal laboratory test results were mild elevation of GOT and GPT in 3 of 43 patients. Based on the above results, we consider that CPDX-PR is useful for treatment of otorhinolaryngological infections.

[A case of adrenal myelolipoma consisting of two masses with different red and yellow colors].

A 52-year-old woman with asymptomatic gross hematuria visited a hospital, where computed tomography and ultrasonography revealed a left suprarenal mass, which was diagnosed as adrenal myelolipoma. After two years observation, she was admitted to our hospital due to enlargement of the tumor, and left adrenalectomy was performed. The tumor consisted of two separate masses with different macroscopic appearances. One was type I, while the other was type II according to Soos' classification. We found no similar case in the literature. We added a review of 119 cases of surgically resected adrenal myelolipoma in Japan.

[Multiple sclerosis with syringomyelia--case report].

A 32-year-old woman experienced subacute onset of weakness in her left leg, urinary retention and difficulty in extending her right middle and third finger. She subsequently suffered episodes of myelopathy, optic neuritis and cerebellar ataxia over a period of several years. Brain MRI showed multiple areas of high signal intensity on T2-weighted images, consistent with multiple sclerosis (MS). However spinal MRI revealed no abnormal findings. In her most recent episode, at age 40 she developed paraparesis. Neurologic examination revealed down beat nystagmus on gazing to the right, horizontal jerk nystagmus gazing to the left, weakness of the right middle and third fingers and paraparesis associated with spasticity of the right leg. Sensory disturbance below C3 and diminished vibration and position sense in both legs were also observed. The patient could not stand or walk, and urinary disturbance was present. Spinal MRI revealed syrinx formation at the level of vertebral bodies C2 to C6. The syrinx within the cervical cord diminished in size after four months, but the patient was unable to walk unaided and had moderate sensory disturbance as before. This finding suggests that the prognosis of MS with syrinx formation following repeated episodes of myelopathy is not always favorable. We believe that functional recovery in MS with syringomyelia is affected by the severity of the demyelination and/or gliosis caused by MS rather than by the presence of the syrinx.

An experimental transplantable osteosarcoma with spontaneous pulmonary metastasis in hamsters.

Animal models of osteosarcoma with spontaneous pulmonary metastasis which retain metastatic capacity and osteoid formation after serial passages have been reported infrequently. In this communication we describe some biological features of a transplantable osteosarcoma, Os515, induced by BK-virus in Syrian golden hamsters. The subcutaneously transplanted tumours in 2-week-old animals grew progressively until death, with a mean survival time of 32 days. Distant metastases occurred only in the lungs in all animals. The histological appearance was osteosarcoma of osteoblastic type. Enzyme-histochemical staining showed alkaline phosphatase activity in many cells and beta-glucuronidase activity in few cells. Tumours transplanted intramuscularly in the hind limbs were amputated radically at 5 or 11 days. A small number of animals died from lung metastases without local relapse during the observation period of 140 days after grafting. All the control hamsters bearing unamputated tumours died much earlier. Necropsy revealed large metastatic nodules in the lungs of limb-amputated animals and small diffuse nodules in the lungs of untreated control animals. The development of lung metastases was monitored by soft X-ray without sacrificing the animals. This model will be useful in studies of mechanisms of metastasis and for the experimental treatment of osteosarcoma.

[A case of Borrmann type 4 gastric cancer responding to sequential methotrexate and 5-fluorouracil combined with CDDP].

A case of Borrmann type 4 gastric cancer responding to sequential methotrexate and 5-fluorouracil combined with cis-diamminedichloroplatinum (CDDP) is reported. A 57-year-old woman was admitted to our hospital complaining of abdominal fullness. Upper gastrointestinal series and gastrofiberscopy revealed almost the entire stomach was involved with Borrmann type 4 cancer. Gastrectomy could not be performed because of peritoneal dissemination. However, the patient responded to sequential methotrexate and 5-fluorouracil therapy and CDDP administration. She enjoyed more than one year of hospital-free survival period and survived for almost two years.
