[Abdominal Burkitt's lymphoma developing the tumor lysis syndrome following chemotherapy].

A 16-year-old girl was referred to our hospital complaining of a huge abdominal mass and ascites. Cells from the ascites were morphologically compatible with Burkitt's cells. These cells had surface immunoglobulin (gamma and kappa chain) and were positive for B-cell monoclonal antibody (L21, 24). Small numbers of the same cells were present in the bone marrow. Soon after the beginning of chemotherapy, oliguria, hyperuricemia, hyperkalemia, hyperphosphatemia, and hypocalcemia caused by "tumor lysis" developed. Peritoneal dialysis was not effective, and the patient died of acute renal failure. Autopsy revealed no tumor cells in her body.

[Evaluation of various administration routes of the streptococcal preparation, OK-432, with regard to their in vivo effect by lymphocytes proliferation assay].

To evaluate the efficacies of various administration routes of the streptococcal preparation, OK-432, we studied the lymphocyte proliferation responses to lectins in patients with malignancies. OK-432 was administered intravenously (I.V.), intramuscularly (I. M.) or intradermally (I.D.) for 2 weeks. Lymphocyte proliferation responses to concanavalin A, PHA and SuM (crude extract of OK-432) were studied before and after OK-432 administration. Enhanced responses were observed in 7 out of 9 patients (77.8%) in the I.V. group compared with 3 out of 7 (44.2%) in the I.M. group and 4 out of 9 (44.4%) in the I.D. group. Ratios of stimulation index (S.I.) after administration over S.I. before administration were highest in the I.V. group. These results suggest that I.V. administration of OK-432 is most effective for stimulating host immune systems.

[Phase II study of VP-16 (capsule) in solid tumors. A cooperative study].

A phase II study of VP-16, a semisynthetic Podophyllotoxin, was performed in patients with solid tumors. VP-16 was administered orally at a dose of 200mg/day for 5 consecutive days at 3 to 4-week intervals. Out of 41 patients who were entered into the study, 35 patients comprising 17 lung cancer, 10 hepatoma and 8 other tumors were evaluable. There were 4 partial responses (23.5%) for lung cancer, 1 (10.0%) for hepatoma and 1 for rhabdomyosarcoma. Overall response rate was 18.2% for patients with prior chemotherapy and 15.4% for those given no prior chemotherapy respectively. Thus the results indicated VP-16 has no cross-resistance to other antitumor agents. Leukopenia (less than 4,000/mm3) and thrombocytopenia (less than 10 X 10(4)/mm3) were observed in 72.7% and 29.4% of the patients, respectively. Other toxicities were alopecia (59.5%) and gastrointestinal disturbances such as nausea (46.2%), vomiting (20.5%) and anorexia (20.5%), but these were all well tolerated.

A case of granular lymphocyte proliferation with T-cell phenotype.

Immunological studies were conducted on a case of granular lymphocytosis with benign clinical feature. A 60-year-old Japanese woman was found incidentally to have lymphocytosis when she had a common cold. A complete blood count showed 47,200 leukocyte per mm3 with 82% granular lymphocytes, 8% small lymphocytes and others. Hemoglobin was 11.5 g/dl and platelet count was 365 X 10(3)/mm3. Surface marker study revealed erythrocyte-rosettes 94%, Leu-1 59%, Leu-2a 70%, Leu-3a 14%, Leu-4 98%, Leu-7 57%, Leu-11 5%, HLA-DR 92%, BA-2 6%, common ALL antigen 4%, and surface immunoglobulin 2%. These results suggest granular lymphocyte proliferation with T-cell phenotype. Natural killer activity was 4.5%, but it was elevated to 11.4% after interleukin-2 stimulation by 2 days' culture. Human T-cell lymphotropic virus-I antibody was absent. No lymphadenopathy and no hepatosplenomegaly were seen, except for bone marrow infiltration of granular lymphocytes. The patient has been in good health without any acute distress. The leukocyte count has gradually decreased to 12,300/mm3 with 79% lymphocytes in 6 months of follow-up without any therapy. This case is suggestive of benign lymphocytosis, although similar cases have been reported previously as chronic lymphocytic leukemia with T-cell marker and/or natural killer function.

[Surface marker studies on lymphoma and leukemia cells with microcytotoxicity test using the UCLA Monoclonal Antibody Tray 5].

Surface marker studies of lymphoid malignancies are important in predicting the prognosis. We have investigated surface marker studies on lymphoma and leukemia cells with a new method of a microcytotoxicity test using monoclonal antibodies. The microcytotoxicity test (UCLA Monoclonal Antibody Tray 5) has the advantages of being performed in a few hours with an easy technique. The results showed that T-cell lymphomas and B-cell lymphomas could be clearly differentiated by monoclonal antibodies specific to T or B cells, respectively. Leukemia cells of ALL and AML were also tested for several monoclonal antibodies. However, the problem that most of the monoclonal antibodies used in this tray have not been characterized for specificity has yet to be solved.

[A case of Burkitt's lymphoma with total ophthalmoplegia].

Burkitt's lymphoma was first reported by Burkitt in 1958 as a sarcoma involving the jaw in African children with characteristic symptoms. Forty three Japanese cases have been reported since the first description by Oboshi et al. in 1969. We report a case of Burkitt's lymphoma with left total ophthalmoplegia. A 73-year-old Japanese female was admitted in Sadamoto Hospital on July 11, 1983 with a two-week history of headache, ptosis and double vision. The patient was exposed to the atomic bomb in Hiroshima and had ten-year history of hypertension. On admission, physical examination showed hypertension and neurological examination revealed only left total ophthalmoplegia (such as left ptosis, external ophthalmoplegia, mydriasis and deficit of light reflex). Plain X-ray film and enhanced CT scan showed no remarkable abnormalities. Laboratory examinations revealed high serum levels of GOT(51 K.U.) and LDH (1300 U.). Left carotid and right retrograde branchial angiograms showed no remarkable abnormal findings. While the patient was treated only conservatively, left abducent and trochleal nerve palsy appeared on August 5, 1983. On plain and enhanced CT scans at the time, abnormal density mass with bone destruction of the left sphenoidal sinus was demonstrated. Biopsy specimen from the left sphenoidal sinus showed lymphosarcomatous cells. Peripheral blood and bone marrow smears showed lymphoma cells which are compatible with L3-Burkitt's type according to FAB leukemia classification. The patient was diagnosed as leukemic transformation of Burkitt's lymphoma and treated with CHOP; Cyclophosphamide (C), Hydroxydaunorubicin(H), Vincristine (O), and Prednisolone (P).(ABSTRACT TRUNCATED AT 250 WORDS)

[Combination chemotherapy consisting of cis-platinum and adriamycin in non-small cell carcinoma of the lung].

Twelve patients with unresectable and recurrent non-small cell carcinoma of the lung were treated with a combination chemotherapy consisting of cis-platinum and adriamycin. The overall response rate was 42% with two complete responses and three partial responses. The durations of response were 9+ and 8.5+ months in two complete responses, and 7.5+, 5.5+ and 2.5 months in three partial responses. The overall median survival was 8 months ranging 3.5+ to 16+ months. Three patients had leukopenia less than 2,000/mm3, two patients had thrombocytopenia less than 100 X 10(3)/mm3, and four patients had hemoglobin decrease of 3 g/dl or more. Severe nausea occurred in nine patients, whereas vomiting did in seven patients. Nearly total alopecia was observed in all patients. Renal toxicity in four patients was also observed, but was reversible. We believe that the combination chemotherapy with cis-platinum and adriamycin is promising in the management of patients with non-small cell carcinoma of the lung.

[Combination chemotherapy with PAP (peplomycin, adriamycin and cisplatin) in recurrent uterine cervical cancer with pulmonary metastasis].

Six patients with recurrent carcinoma of the uterine cervix were treated with a combination of peplomycin, adriamycin and cis-platinum (PAP). The PAP regimen consisted of peplomycin at a dose of 5 mg/day, continuous i.v. drip on days 1-7, adriamycin at a dose of 40 mg/m2 i.v. on day 1, and cis-platinum at a dose of 40 mg/m2, continuous i.v. drip on day 1, repeating at five to six week intervals. Three complete responses and three partial responses were obtained in six evaluable patients. Toxicities included mild to moderate nausea and vomiting, alopecia, nephrotoxicity, and myelosuppression, were generally manageable. We have concluded that PAP regimen is one of the useful regimens for recurrent uterine cervical carcinoma with pulmonary metastasis.

[Combined chemotherapy and elemental diet in advanced lung cancer patients].

The nutritional and immune status were studied in advanced lung cancer patients receiving combined chemotherapy and an elemental diet to test the effectiveness of the latter. Parameters such as skin tests, lymphocyte count, total protein and albumin values, body weight, and nitrogen balance were improved or maintained by the administration of the elemental diet. A better response to chemotherapy was achieved in patients receiving the elemental diet. Elemental diet support is suggested as a useful addition to chemotherapy in advanced lung cancer.

[Effect of intravenous treatment with OK-432 on the bone marrow in patients with lung cancer].

We studied effects of OK-432 on the bone marrow and peripheral blood cells of lung cancer patients. The nuclear cell count of bone marrow increased in 5 to 7 patients upon intravenous treatment with OK-432 compared with 3 of 6 patients who were intramuscularly treated with OK-432. Serial neutrophil counts of bone marrow increased in all 7 patients treated intravenously compared with 3 of 6 patients treated intramuscularly. The mean nuclear cell count or the serial neutrophil count of bone marrow in intravenously treated patients was significantly higher than the pretreatment values (p less than 0.001). In the peripheral blood picture, the difference in white blood cells or neutrophils before and after intravenous treatment was also statistically significant (p less than 0.01). There was no change in the erythrocytic series count of bone marrow and the hemoglobin count. Our results support the superiority of intravenous OK-432 treatment over intramuscular treatment in the growth-accelerating effect on bone marrow cells, especially regarding the neutrophil series.

[Efficacy of intrapleural treatment with aclacinomycin combined with closed tube thoracostomy for malignant pleural effusion].

Intrapleural treatment with aclacinomycin combined with closed tube thoracostomy was used in 7 patients with malignant pleural effusion. Five patients had no recurrence of effusion 3 months after the treatment. Aclacinomycin levels were much higher in blood cells than in plasma, and metabolites were present as the active form. We posit that the local instillation of aclacinomycin is indicated in the management of malignant pleural effusion.

[Remission maintenance therapy of non-Hodgkin's lymphoma with CHOP protocol].

We have retrospectively studied the problems concerning duration of maintenance therapy and chemotherapy regimens for non-Hodgkin's lymphoma treated at Shikoku Cancer Center Hospital. Four out of 15 complete remission cases had been in complete remission more than 3 years. Maintenance chemotherapy consists of CHOP protocol administering every 4 weeks and the median maintenance duration was 14 months with a median cycle of 14. Thereafter, patients have been followed up without receiving any therapy. The prospective study on maintenance therapy should be designed considering conditions such as 1) sufficient drug doses for cytocidal effects, 2) shorter maintenance period as possible, and 3) minimal side effects and complications to be expected.
