[How many newborn infants have an increased risk of atopic disease?].

Children of atopic parents as well as newborns with an increased cord blood IgE-level have a increased risk to develop atopic symptoms later in childhood under the influence of certain environmental factors. Out of 1203 newborns there were 3% with a biparental history of atopy and 9% with a cord blood IgE higher than 0.9 kU/l. Boys showed elevated cord blood IgE-concentrations more frequently than girls. There were no differences in IgE-levels between Turkish and German neonates. The effectiveness of preventive measures against atopic diseases should be studied in defined high risk groups, before a screening for atopy is considered.

[Biopsy of the chorionic villi].

The biopsy of chorionic villi (BCV) has by now been applied in approximately 20,000 pregnant women world-wide in prenatal diagnosis. The first experience in the Federal Republic of Germany was gained in the centres supported by the Federal Ministry for Research and Technology. Based on a questionnaire sent to each of these centres, a survey of the situation is given up to July 1986; at that time, each centre had an average of 2 years of experience with this new diagnostic method. At the present time, approximately 10% of all amniocenteses are replaced by BCV, the transcervical, ultrasonic catheter control method being applied in most cases. In 89%, BCV also resulted in a reliable genetic diagnosis. Bleedings without induction of abortion (16.7%) were relatively frequent complications. Prediagnostic abortions occurred in 10.6% and 5.5% following BCV. However, every 5th abortion took a febrile course. Genetic diagnoses confirmed an evidently normal finding in 87.9% and a pathological finding in 4.4%. 6% of the findings made a control necessary. Abortion was performed in 3.8% of the cases following biopsy of chorionic villi.

Growth fractions and estrogen receptors in human breast cancers as determined in situ with monoclonal antibodies.

The growth fraction (GF) and estrogen receptor (ER) status of 76 cases of breast cancer were investigated on frozen sections of the same tissue block by immunostaining with monoclonal antibodies (Ki-67 and anti ER antibody). In 55 cases of this series, the ER status was also determined by standard biochemical methods. Our study revealed an inverse correlation between GF and ER status. This negative relationship was most significant when both variables were determined immunohistologically in the same tissue block and less significant when GF was analyzed by immunohistology and ER status by biochemical methods. These data suggest that the immunohistologically assessed ER values characterize the receptor status of a given carcinoma better than the biochemical values, which leads to the conclusion that the immunohistologically determined close negative relationship between GF and ER reflects the actual in vivo situation of a given breast carcinoma case. However, the immunohistologic analysis also revealed that there is a proportion of exceptional cases, ie, those with a positive ER status and a large or moderately large GF. Because only half of the ER-positive breast carcinomas respond to endocrine therapy with objective remission., it is hypothesized that it might be preferentially these mentioned exceptional cases that fail to positively respond to endocrine treatment protocols.

[Fine structure of the septum in twin pregnancy].

The structure of the septum in bigeminal pregnancy was examined via the electron microscope after normal pregnancy. The cellular junctions were desmosomes and "gap junctions" in the amnionic epithelium, whereas in the chorionic epithelium "gap junctions" and "tight junctions" were found occasionally besides desmosomes. The basement membrane was multilayered and discontinuous both in the amnion and in the chorion. Since, physiologically, the desmosomes and the gap junctions cannot be regarded as a diffusion barrier against proteins, and since the rare tight junctions are without any significance for the passage of the proteins through the septum, and, furthermore, since a discontinuous basement membrane cannot exercise any action in the nature of a barrier, these structural peculiarities of the septum seem to indicate that the intercellular spaces and the discontinuous basement membrane of the amnion and of the chorion in the septum may represent a paracellular path for the passage of the proteins between the two amniotic cavities.

[Improved diagnosis of pregnancy-associated gynaecological emergencies via rapid beta-HCG test (author's transl)].

A pregnancy-related disorder was identified by a rapid radioimmunoassay or serum chorionic gonadotropin in 151 out of 600 women (25%) with lower abdominal pain or bleeding, while a routine pregnancy test in urine was positive in only 7% of these cases. In 60 patients with ectopic pregnancy the rapid hCG-RIA was positive in 90% as compared with 10% for the routine pregnancy test. In patients with evidence of intrauterine pregnancy (80 cases) the rapid hCG-RIA was positive in 99% and routine pregnancy test in 39%. The clinical sensitivity of the hCG-RIA was 95%, specificity 96% and predictive value 88%. These figures should introduce a considerable improvement in the routine diagnosis of early intra- and extrauterine pregnancy-related disorders.

[The induction of abortion in the second trimester by combined administration of minprostin and sulproston, compared with the use of sulproston alone (author's transl)].

Sulproston was used to terminate 125 pregnancies from 13 to 41 weeks duration, between 1.1.1979 and 31.12.1980. The indications for termination of pregnancy included medical problems, genetic disorders and neural tube defects. Sulproston alone was used in 30 cases. In the remaining 95 cases the cervix uteri was treated with Minprostin F2a before Sulproston application. The failure rate was 20% when Sulproston alone was used at varying dosage. There were no failures using the combination therapy. Combined therapy reduced the time necessary for abortion by an average of four hours, the period of painful contractions was reduced, and less Sulproston was needed. The incidence of side effects was comparable in the two groups. There were no complications from the intracervical application of Minprostin F2a.

[Pheochromocytoma in pregnancy--pathophysiology and therapy (author's transl)].

In this paper pathophysiology of pheochromocytoma and its modern diagnostic and therapeutic management are discussed with special regard to the combination of pheochromocytoma and pregnancy. The preoperative treatment with phenoxybenzamine is evaluated. As for the anesthetic management barbiturates and neurolept anesthesia are recommended. In the acute treatment of extreme blood pressure values sodium nitroprusside is preferred to phentolamine. Whereas vaginal delivery is contraindicated in a patient with pheochromocytoma there is no definite superiority of tumorresection along with caesarean section against a separate abdominal procedure.

[Examination of placenta by light and electron microscope in a woman patient with renal transplant (author's transl)].

The placenta of a woman who had received a renal graft before she became pregnant was examined. The light- and electronmicroscopical results were as follows: 1. Enlargement of the granulated endoplamatic reticulum in the syncytium. 2. Increase of the syncytial knots. 3. Persistence of exceeding numbers of Langhans cells. These alterations coinciding with the immune suppressive therapy might play a role for the initiation of the growth retardation as observed in children of renal-grafted mothers.

[Investigation on the postoperative behavior of plasma amino acids in metabolically healthy patients. 1st communication (author's transl)].

In 12 patients before and after vaginal hysterectomy the plasma amino acid levels were determined. Without parenteral feeding the changes in plasma amino acid patterns can be explained by effects of operation and anaesthesia alone. The branched-chain amino acids decreased after operation. The following rise was never exceeding the control level. Even without substitution the concentrations of phenylalanine, methionine, and glutamic acid were increased. Tyrosine concentration remained within normal levels while cystine fell significantly.

[Plasma amino acids and nitrogen balance during the postoperative period in metabolically healthy patients. 2nd communication (author's transl)].

Under parenteral and following oral nutrition in ten metabolically healthy patients the plasma amino acids were determined before and after vaginal hysterectomy. Starting from normal preoperative values there is a fall in the plasma concentration of most amino acids. The branched-chain amino acids decreased significantly, but under oral feeding they rose above the initial values. With increased plasma phenylalanine and methionine levels the concentration of cystine and tyrosine remained nearly unchanged. Glycine showed no increased levels while serine concentration was decreased even under substitution.
