Report on a 30-year old primigravida with a family history of bleeding and a personal history of nasopharyngeal bleeding since age four. Severe menorrhagia since her menarche. First seen at 20 weeks gestation with naso-pharyngeal bleeding and a twin pregnancy. The patient was found to have Glanzmann's disease (congenital thrombasthenia) with an excessive lack of factor XIII. Ambulatory treatment with packed cell transfusions until 35 weeks gestation. The thrombelastogram did not improve with transfusions of platelet concentrates. In-patient treatment from 35 to 37 weeks gestation, no vaginal bleeding throughout the pregnancy. Elective Caesarean hysterectomy at 37 weeks gestation with lo units fresh blood, 3 units platelet concentrate and 10 ampules factor XIII concentrate in readiness. Delivery of twin girls 3180 and 3050 g without signs of Glanzmann's disease. Treatment of the maternal bleeding by replacement. Factor XIII 80% of normal 8 weeks postpartum. This appears to be the first case of twin pregnancy in the presence of Glanzmann's disease and the fourth case in the world literature.
The AFP concentration in the amniotic fluid is elevated after contamination of the sample with fetal blood during a traumatic amniocentesis. Though the calculation of AFP due to fetal hemorrhage is possible by counting the fetal red cells there may be execssive errors resulting from the variability of fetal red cell count, amniotic volume, fetal serum AFP concentration, AFP concentration in the amniotic fluid changing with fetal age. Therefore it is impossible to interpretate elevated AFP values from a sample stained with fetal blood. A repeated amniocentesis is unavoidable. The correct time to do a second punction is still a matter of question.
Maternal serum concentrations of AFP, HPL, SP1 and Estriol were measured in the course of prostaglandin-induced abortions. Termination of pregnancy between the 13th and 25th week of pregnancy was done for medical reasons. Patients were treated with PGE2, PGF2alpha and with 16-phenoxy-PGE2-methansulfonamid, respectively. Prostaglandins were administered systemically, i. e. intravenously. Current measurement of HPL and AFP-concentrations showed a contrary sense. These changes of concentrations were related to fetal death and membrane rupture in some cases as is demonstrated in four diagrams. Typical and sharply decreasing values of Estriol followed fetal death in patients with intact pregnancies before. As for an anencephalic fetus, AFP-concentrations increased slightly too, HPL values decreased like in other abortions. Only minimal changes of SP1 concentrations were found generally in all patients and during abortion time.
The hidden intrauterine device (IUD) may result because of disapperance of the tails into the uterus, unobserved loss of the device and perforation. The localization can be estimated by sonography, x-ray and probing of the uterine cavity. In most of the cases the device can be removed without hospitalization, surgical procedures are rarely neccessary. If pregnancy did occur the hidden intrauterine device cannot be removed without termination of the pregnancy in most cases.
Rates of success after surgical treatment of tubal occlusions are limited by the etiologic factros of the female tubal sterility and by the operative techniques. The results of tuboplastic surgery have been improved during the last decade by critical preoperative selection of patients and subtile perioperative therapy. Nevertheless, rates of success are by no means satisfactory. Statistical evaluation of 5,056 cases reported in the literature shows an overall success rate of 26.5% intrauterine pregnancies with 19% live births. The classical surgical techniques are described. Preoperative examination and indications for the operations are discussed. Complementary surgical procedures may be helpful to prevent postoperative adhesions. Further improvement of tuboplastic surgery can be achieved by the introduction of microsurgery and endoscopic methods in the treatment of tubal sterility.

Cord blood of 686 newborns was tested for IgM-levels in connection with an epidemic occurrance of rubella infection in Bonn and its vicinity. In 17 samples IgM was higher than 30 mg/100 ml and in 102 samples higher than 20 mg/100 ml. In 121 cases the cord blood was examined for rubella antibodies and in 85 children it was also checked for the persistence of rubella antibodies at the end of the first six months of life. No unexpected prenatal rubella infection was found. Two children of this group had clinical and serological criteria for rubella embryopathy at birth. In addition 39 cases of verified rubella infections during pregnancy were investigated. In 13 cases abortion was performed during the first trimenon following a rubella infection; the presence of rubella virus embryotic tissue could be proven in 9 cases. A prenatal rubella infection became apparent by illness of the mother during the pregnancy and/or embryopathy of the newborn in 26 children. In 16 of these children prenatal rubella infection was confirmed by the detection of rubella specific IgM which was associated in all instances with elevated levels of IgM, and/or the persistence of rubella antibodies after the sixth month of life. The remaining 10 children had neither clinical nor serological signs. The mothers of 9 of these children had been ill after the 16th week of pregnancy. In at least 21 cases (81%) of 26 maternal rubella infections during the first trimenon of pregnancy the virus passed the placenta. In 13 cases the pregnancy was interrupted; of the remaining 13, 6 children (46%) had embryopathy.

The sera of 327 patients were investigated for rubella specific IgA after separation of the IgM fraction by sucrose density gradient centrifugation. 1. In 96 out of 137 cases suspicious of clinical rubella, where the time for demonstration of a significant rise of the antibody titer had been missed, the demonstration of rubella specific IgM confirmed the diagnosis of German measles. This demonstration was still possible in 9 out of 10 cases 5 to 7 weeks after the onset of the disease, but in only 2 out 7 cases 8 to 15 weeks after the onset of the disease. 2. In 48 patients who had contact with patients with German measles but did not develop any symptoms, a primary infection could not be excluded by the examination of the whole serum. In 5 out of them rubella specific IgM indicted a recent rubella infection. 3. Rubella specific IgM could not be demonstrated in 47 patients who had high antibody titers but no other hints for a recent rubella infection. 4. In 89 newborns, the investigation was prompted, when elevated IgM levels were found in umbilical cord blood, or when the newborn was sick, or when a rubella infection of the mother was suspected or proven. Rubella infection of the newborn was confirmed by rubella specific IgM only if the mother had contracted German measles during the first trimester of the pregnancy (6 out of 7 cases). The diagnosis of primary rubella infection during pregnancy is proven only if 2 out the fose in antibody titer, demonstration of rubella specific IgM.
