[Surgical treatment of malignant tumor of the hepatic duct junction].

Seventy-nine patients with extrahepatic malignant cholangiocarcinomas were treated at the Surgical Clinic of the Charité from 1981 to 1989. Thirty-two of these patients suffered from Klatskin tumour. Exact preoperative staging includes sonography, CT scan, cholangiography (PTC or ERCP). Angiography can help to clear the diagnoses. Sixteen patients were considered inoperable. The therapy was surgical bypass, surgically or percutaneously placed catheters and endoscopically placed endoprostheses, a median survival time was 2 to 6 months. The other 16 patients underwent 12 resections and four liver transplantations. Two of these transplanted patients are still tumour-free, their survival times ranging from 2 to 6 years. The median survival time of the patients undergoing resection amounts to 22 months and operation mortality, 12%. Optimal treatment strategies by the Klatskin tumours is the radical resection.

[The atypical fibroxanthoma].

A 84-year-old female patient suffering from atypical fibroxanthoma is presented. A diagnosis of malignant melanoma or a cutaneous metastasis from an unknown primary tumor was suspected. The final diagnosis was established by immunohistological studies showing a strongly positive reaction to vimentin and a negative reaction to S-100 protein and cytokeratin.

[Diagnostic and therapeutic procedures in liver injuries in children].

Despite of severity of posttraumatic liver lesions after blunt abdominal trauma the availability of modern imaging diagnostic methods has changed the therapeutic concept. Nonoperative treatment attained an alternative place to operative procedures. For this kind of treatment personal and technical premises are mandatory. With ultrasound and computed tomography are non-invasive methods for diagnosis available. Paracentesis has lost of importance. This report of 7 children with liver laceration within 6 years discuss diagnostic and therapeutic practice. Decision for operative or nonoperative treatment depends on critical evaluation.

[Experiences with fibrin glue-coated collagen fleece].

Fibrin-adhesive coated collagen fleece (Tachocomb) was used for haemostasis on 57 patients on whom different types of operations had been performed. These had been partial hepatectomies (21 cases), liver transplantations (six), and partial pancreatectomies (nine). Haemostatic effect on diffuse oozing haemorrhage was satisfactory. However, collagen fleece patches were only 8 cm X 4 cm in size and should be available in larger dimensions, particularly when it comes to larger sealing areas.

[Surgery of pancreatic necrosis].

Surgical intervention in the presence of pancreatic necrosis was indicated if infection of necrosis developed. The demonstration of necrosis by CT, without clinical evidence of infection was not sufficient for operation. From 155 patients with necrotizing pancreatitis 134 had an operation upon. Surgical technique was the necrotectomy, drainage formation of a compartment and the periodic local lavage of the pancreatic cavum. 53 (39%) of 134 operated patients died.

[Psychological aspects of traffic fitness of aging car drivers].

With the increase in the proportion of older car drivers in road traffic of the G.D.R. it becomes more necessary to carry out traffic psychological fitness examinations stronger related to health indications. The present paper is occupied with accident frequency, accident causes, indications of age-induced changes in relevant traffic psychological performance demands, and it gives references with regard to a self-assessment of age-induced impairments in driving behaviour.

Leukocytospermia is associated with poor semen quality.

Semen samples from 179 infertility patients were analyzed for type and number of white blood cells (WBC) by a combination of immunologic techniques. Forty-one (23%) had more than 10(6) WBC/mL semen (leukocytospermia). Semen parameters in patients with less than 10(6) WBC/mL were similar to those of 15 fertile donors. In contrast, the leukocytospermic group had significant reductions in total sperm number, percent sperm motility, sperm velocity, motility index, and total number of motile sperm. Patients with high concentrations of monocytes/macrophages (greater than 5 X 10(5)/mL; n = 27) had significantly reduced ejaculate volume; patients with high numbers of T lymphocytes (greater than 10(5)/mL; n = 19) had a significant reduction in sperm velocity; and patients with high levels of granulocyte elastase in semen (greater than 1,000 ng/mL; n = 26) had significant reductions in ejaculate volume, total sperm number, and total motile sperm number. There was no correlation between the presence of antisperm antibodies and leukocytospermia. Our data suggest that leukocytospermia may occur frequently in male infertility patients and show that elevated levels of WBC in semen are associated with poor semen quality. This provides further rationale for white blood cell testing in semen of male infertility patients, since antibiotic or anti-inflammatory therapy may be helpful in appropriately selected cases.

[The liver--an immunologically privileged organ?].

Some rat strains permanently accept allogenic livers without pharmacological immune suppression. In the same donor/recipient combinations, other organs as heart, kidney or skin are normally rejected. The transplanted liver not only withstands its own rejection, but also induce donor-specific immune tolerance. The mechanisms responsible for this effect may be clonal deletion, activation of suppressor cells or immune suppressive circulating factors as anti-class II-antibodies. An "immune-privilege" of an orthotopically transplanted liver exists also in pigs and, however to a lesser extent, in other large mammalian species including man. Regeneration of liver tissue altered by the transplantation procedure may produce the signals for induction of tolerogenic mechanisms.

[Which surgical methods are justified in breast cancer?].

Between 1978 and 1987, operations were performed for primary mammary carcinoma on 1,112 patients, 59.2 years in average age. One pre-menopausal and one post-menopausal peak were revealed by subdivision by age groups. General five-year survival rates were 72.95 per cent for all patients who had undergone surgery between 1978 and 1983 (n = 593) and 88.18 per cent for those with a tumour up to 20 mm in size (n = 220). Modified radical mastectomy is recommended as standard therapy, while small tumours should be tackled by lumpectomy.
